Medical Description

Amitone Syrup is a prescription medication. It contains Amitriptyline, which belongs to a group of medications called tricyclic antidepressants. Apart from its use in the management of major depressive disorders, it is also commonly used in the management of neuropathic pain and migraine in varying doses. Each bottle of Amitone Syrup contains 200mL of product. Depression is a condition characterised by a pervasive low mood. Patients present with complaints like feeling low, decreased motivation, feelings of hopelessness and dread, and loss of interest in daily activities, which can be debilitating. Multiple brain pathways and chemical messengers are indicated in depression, chiefly serotonin, norepinephrine and dopamine. Antidepressant medications target these pathways and neurotransmitters to help relieve the symptoms of depression. The medication should be taken as recommended by the doctor. It should not be abruptly started or stopped, as it may cause a relapse of symptoms, withdrawal symptoms and worsening of an underlying condition. Taking medicine at the same time every day is advisable for maximum efficacy.

Contraindications

  • Hypersensitivity reactions – avoid using this medication in case of hypersensitivity reactions to these medications/ inactive ingredients.
  • This medication should not be used in patients with a recent history of heart conditions such as irregular heart rhythm or myocardial infarction (heart attack).
  • It is contraindicated in patients with angle-closure glaucoma (increased pressure in the eye)
  • It is recommended to discontinue this medication before elective surgery to decrease the risk of interaction with anaesthetic agents.
  • This medication decreases the seizure threshold and should be used cautiously in patients with epilepsy/ seizure disorders.
  • Contraindicated in children < 12 years of age.
  • It should be used cautiously in adolescents and young adults <24 years of age as it may increase suicidal ideation.

Directions for Use

  • This medication should be taken as prescribed by the treating physician.
  • It is recommended to take this medication on a regular schedule, i.e., at the same time every day, for optimal pharmacological effects of this medication.
  • Shake the bottle well before use.
  • Use a measuring cup and accurately measure the quantity of syrup as prescribed by your treating physician.
  • It is recommended to finish the entire course of this medication as prescribed by your treating doctor.
  • Abrupt stoppage of the drug can cause withdrawal side effects. Consult your doctor before stopping this medication.
  • This medication may require 2-4 weeks for onset of action. Do not increase the dose without explicit advice from your treating physician.
  • This medication can be taken with or without food.
  • Driving/ use of heavy machinery is not advised while using this medication.
  • Inform your doctor regarding other medications you may be taking to review and prevent any drug interaction associated effects.

Storage and disposal

Store in a cool and dry place away from direct light and moisture. Do not use the medicine after the date of expiration. Keep the medicines away from children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Overdose

Amitriptyline toxicity can be severe and even fatal. In the event of an overdose, the patient may present with sedation, seizure, coma, increased heart rate, low blood pressure, dilated pupils, dry mouth, difficulty in passing urine or stools, abnormal heart rhythms, low urine output. In case of a suspected overdose, immediately rush the patient to the nearest tertiary care hospital for emergency management as it is life-threatening.

Missed a Dose

If you miss a dose of this medicine, it is advised to take it as soon as possible. If the duration missed is too long, and it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular schedule. Do not double the dose.

Mode of Action

How Does It Work?

Amitriptyline belongs to a class of medications called Tricyclic antidepressants (TCAs) that acts on multiple brain pathways to increase levels of certain chemical messengers called neurotransmitters. It works by increasing the neurotransmitters serotonin, and nor-epinephrine levels, which affect the mood and personality. It also causes sleepiness. In the long term, it increases the levels of BDNF (brain-derived neurotrophic factor). This reduces the symptoms of Major Depressive Disorder. The action of this drug on serotonin and nor-epinephrine is also indicated in the management of neuropathic pain and migraine. It is important to note that Amitone Syrup as an antidepressant requires about 2-4 weeks of continued use for optimal onset of action to begin.

Interactions

Interactions with other medicines

  • This medication affects the heart rhythm. It should not be used in addition to other medications with the same effect, as it may result in potentially life-threatening conditions. Examples include astemizole, cisapride, etc.
  • This medication should not be used concomitantly with MAOIs (Monoamine oxidase inhibitors) or within 14 days of MAOIs and vice versa. Examples of MAOIs include Selegiline, Rasagiline.
  • This medication should not be used with other drugs known to increase serotonin levels, like isocarboxazid, as it may result in a potentially fatal condition.
  • This medication should not be used with alcohol or other CNS depressants.
  • This medication should not be mixed with opioid medications or benzodiazepines.

Frequently Asked Questions (FAQs)

Amitriptyline must be used for at least three months following management of presenting symptoms to prevent depression. Sudden stoppage can significantly cause a higher risk of your symptoms returning, as well as producing withdrawal symptoms.
