Medical Description

Stalopam plus is a prescription medication. It is a combination of two drugs, Escitalopram (10mg) and Clonazepam (0.5mg). It is used to treat generalised anxiety disorder and depression. Escitalopram belongs to a class of drugs called Selective Serotonin Re-uptake Inhibitors (SSRIs), which act on the brain to prevent the re-uptake of serotonin, thereby increasing the level of serotonin in the nerve cells. This increases the level of serotonin in the central nervous system. Serotonin is responsible for modulating various behavioural processes, including mood, anger, memory, fear, appetite, stress response, etc. Clonazepam is a benzodiazepine that causes calming effects on the brain by being an agonist of an inhibitory neurotransmitter called GABA and increasing the synthesis of serotonin. The medication should be taken as recommended by the doctor. It is generally advisable to take the tablet at the same time every day as this ensures the most productive action of the medication, as well as decreasing the chances of relapse significantly. It is also recommended to complete the entire course of the medication as prescribed by the treating physician and not abruptly stop taking the drug.

Contraindications

  • It is contraindicated in case of hypersensitivity to the ingredients in this combination.
  • It is contraindicated in patients with severe liver disease, as the liver metabolises these drugs.
  • This drug is not recommended during pregnancy and lactation.
  • Clonazepam is contraindicated in patients with acute narrow-angle glaucoma.

  • It can be taken individually or can be clubbed with food as well.

Dosage

Overdose

The highest recommended dose of Escitalopram is 20 mg orally daily. The therapeutic range of Clonazepam is from 0.02 to 0.08 mcg/mL. A level greater than 0.08 mcg/mL is toxic. The overdose of this medication can cause irregular heartbeat, sleepiness, double vision, difficulty breathing, low blood pressure, slowing of heart rate, cardiac arrest, and coma. In case of a suspected overdose, visit an emergency room immediately for urgent intervention.

Missed a Dose

The recommendation in case of a missed dose is to take it as soon as possible. If the duration missed is too long, and it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular schedule. The dose of the tablet must not be doubled in this case.

Mode of Action

How Does It Work?

Escitalopram is a drug belonging to a class called Selective Serotonin Re-uptake inhibitors (SSRIs). Serotonin or 5-hydroxytryptamine (5-HT) is responsible for modulating various behavioural processes, including mood, anger, memory, fear, appetite, stress response, sexuality, addiction, etc. Increased serotonin levels directly improve the mood and relieve the user’s symptoms. It acts by preventing the re-uptake of serotonin by the nerve cells, thereby increasing its concentration. This process results in an increased effect of serotonin in the central nervous system. Clonazepam acts dually by increasing serotonin synthesis and acting as a GABA-A receptor agonist. GABA stands for Gamma Amino Butyric Acid, and it is an inhibitory neurotransmitter. By binding to the GABA-A receptor, it enhances the inhibitory effects of GABA, resulting in sedative, anti-anxiety, muscle relaxant properties.

Interactions

Interactions with other medicines

  • Serum levels may be reduced by CYP3A4 inducers (e.g. nafcillin, nevirapine), or CYP2C19 inducers (e.g. carbamazepine, rifampin, and phenytoin).
  • Serum levels may also be increased by CYP3A4 inhibitors (e.g. azole antifungals, clarithromycin) or CYP2C19 inhibitors (e.g. fluconazole, fluvoxamine, omeprazole).
  • It may cause an increased risk of bleeding when used with aspirin, NSAIDs, and other drugs which affect coagulation.
  • The concomitant use of this medication with MAOI (monoamine oxidase inhibitors) like Selegiline, rasagiline, isocarboxazid, etc., is strictly contraindicated as it may cause serotonin syndrome. In case of use of these drugs, Stalopam Plus should not be taken until after 2 weeks of stopping their use and vice versa, as it can be potentially fatal.
  • Serotonin syndrome can also be induced by antidepressants, triptans, fentanyl, lithium, tramadol, tryptophan, buspirone, amphetamines. These are also contraindicated.
  • Intravenous methylene blue, linezolid, and other psychiatric medications are also contraindicated as they impair the metabolism of serotonin.
  • Escitalopram may cause prolongation of QTc interval, hence concomitant use with other drugs, which may also cause this to be contraindicated. E.g. – typical antipsychotics.
