Medical Description

Gospas Tablet is a fixed-dose combination of three active ingredients, namely Clidinium (2.5mg), Chlordiazepoxide (5 mg) and Dicyclomine/Dicycloverine (10 mg). It is commonly prescribed for medical conditions such as Irritable bowel syndrome, ulcers in the stomach or intestine and enterocolitis (intestinal swelling). Irritable bowel syndrome is a condition that interferes with the normal movement of the intestines, thereby leading to severe, intermittent abdominal pain due to muscle spasm, flatulence, constipation, or even diarrhoea. Some common side effects that may occur after the intake of Gospas Tablet generally subside with time. However, the doctor must be informed if it starts affecting your daily activity or in case of serious side effects.

Uses

  • Treatment of Irritable bowel syndrome.

Contraindications

  • If you have a known hypersensitivity (allergy) to any of the constituent ingredients or to benzodiazepines in general.
  • If you have a pre-existing condition of respiratory issues like pulmonary insufficiency (decreased and insufficient lung function) or sleep apnea (sudden breathlessness while asleep).
  • If you are a diagnosed case of liver disease, especially severe insufficiency of the liver.
  • If you are a known case of chronic or long-standing psychosis.
  • If you have Myasthenia gravis, a condition wherein there is rapid fatigue of voluntary muscles (muscles under your control) is also a contraindication.
  • If you have Ataxia (loss of control over voluntary muscles).
  • If you have an enlarged prostate, glaucoma (increase pressure inside the eye).
  • If you are pregnant or have future plans of pregnancy.

Alcohol

The active ingredient of Gospas Tablet can have a sedative effect on the patient, leading to increased drowsiness, dizziness, and impaired muscular coordination. Alcohol by itself is essentially a depressant of the central nervous system. Hence, it can cause further damage when consumed with Gospas Tablets. Additionally, both alcohol and Gospas Tablet are metabolised in the liver, leading to obtundation of the effect of the medication. Consult your doctor in case of any eventuality.

Talk to your doctor if
  • You are consuming any allopathic or non-allopathic medication for the same or a different disease condition.

Directions for Use

  • Gospas tablet is to be consumed by the oral route.
  • Swallow it whole with a glass of water or as directed by your doctor. It should not be crushed or chewed as this may decrease the efficacy of the medication.
  • It is advisable to consume this medication on an empty stomach.
  • The dosage and frequency are to be followed strictly as per the doctor’s advice.
  • The dosage changes according to age, response, induction, maintenance, other medications and systemic conditions.
  • Do not discontinue the treatment schedule abruptly or change the dosage without informing your doctor.
  • Do not self medicate.

Storage and disposal

Overdose

Overdose of Gopas tablet is typically characterised by profound depression of the central nervous system resulting in an exaggeration of the common side effects like drowsiness, generalised confusion and lethargy. In case of severe overdose, low blood pressure, breathing issues, loss of control over body movements may be precipitated. If you have overdosed, tell a doctor or contact your nearest hospital casualty department immediately. Take the medicine wrapper with you.

Missed a Dose

If you miss a dose of Gospas Tablet, take it as soon as you remember it. However, never double the dose. In case you remember the missed dose very close to the next dose, skip the missed dose, and continue your schedule. Keep your doctor informed at all times about any change or multiple missed doses.

Mode of Action

How Does It Work?

The primary action of Gospas Tablets is by stopping the sudden cramping of muscles in the abdomen and facilitating more coordinated intestinal contractions. This not only provides symptomatic relief in terms of abdominal pain but also allows a passage for the built-up gas giving long term relief. Gospas Tablet has three primary active ingredients. Clidinium and Dicyclomine/Dicycloverine are antispasmodics that relax the muscles of the stomach and the intestines, countering the sudden cramps and the associated pain. Chlordiazepoxide actively metabolises the liver. It acts by inhibiting the signals to and from the muscles to the spine by increasing the secretion of GABA (a chemical messenger of the nervous system that facilitates muscle contraction). Hence, Chlordiazepoxide also relaxes the intestinal muscles and relieves the abdominal cramps, the accumulated gas, and the associated pain and discomfort.

Interactions

Interactions with other medicines

  • Medicines used in the treatment of epilepsy or seizures (e.g. phenytoin, phenobarbitone) or barbiturates or combinations including them can also increase the severity and frequency of side effects associated with Gospas tablet.
  • Medicines that affect the P450 enzyme in the liver. For example, cimetidine (used to treat acid indigestion and ulcers) and omeprazole (used to treat stomach problems) reduce the activity of the enzyme, thereby increasing the concentration of Chlordiazepoxide in blood,
  • Rifampicin (a medicine used in the treatment of tuberculosis) increases the action of the enzyme, hence reducing the concentration of Chlordiazepoxide in blood.
  • The effect of medicines used in Parkinsonism, such as Levo-dopa, is reduced by the concomitant use of Gospas Tablets.
  • Pain killers and anaesthetic agents like codeine and morphine potentiate the adverse effects of Gospas Tablets. It can also cause psychological dependence due to the associated euphoria.
  • Muscle relaxants like Baclofen, and medications used to treat constipation and heartburn, like cisapride, can hamper the sedative effects of Gospas tablet.
  • Medicines used to treat high blood pressure and diuretics (medicines that increase the loss of salt and water from the body) like furosemide tend to increase the hypotensive propensity of Gospas Tablets.
  • Sodium oxybate, used in patients with narcolepsy (increased tendency to fall asleep), is to be avoided with Gospas Tablet.
  • Theophylline, a medication used to ease breathing, can decrease the effect of Gospas Tablets.
  • Antihistamines like chlorpheniramine can increase drowsiness when administered with Gospas tablet.
  • Monoamine oxidase inhibitors (MAO) like isocarboxazid, linezolid, selegiline, and methylene blue should not be prescribed along with Gospas Tablets.
  • While consuming anti-coagulants like warfarin or cardiac medications like digoxin with Gospas tablet, care should be taken.

Interactions with food items

  • Unless specifically directed by your doctor, a normal diet can be continued while on Gospas Tablet. However, it is advisable to avoid fatty or fried food when on medication with Gospas tablet. Kindly consult your doctor in case of any eventuality.
