Product Summary

Uses
Apnea of prematurity: Premature newborns may have pauses in breathing because breathing control is immature; caffeine stimulates breathing and reduces these episodes under neonatal care.
Side effects
Restlessness or sleep disturbance: Caffeine stimulates the baby's developing nervous system and may make settling difficult or the baby unusually wakeful. Tell the neonatal team if this is marked., Fast heartbeat: Caffeine can increase heart rate while stimulating breathing. The neonatal team should assess persistent fast heartbeat, pallor or worsening feeding immediately., Frequent urination: Caffeine may increase urine output in some babies. Staff should monitor wet nappies and fluid balance rather than giving extra fluids at home., Dehydration: Increased fluid loss may cause fewer wet nappies, a dry mouth or unusual sleepiness. Alert the neonatal team promptly rather than giving fluids without advice., Fever or unusual confusion: Fever or mild delirium can occur during treatment and may be difficult to recognise in a premature baby. Tell the neonatal team immediately.

Medical Description

Cafirate injection is used to reduce repeated pauses or shallow breathing in premature newborns with apnea of prematurity. It contains caffeine, a stimulant that activates the immature breathing centre in the brain and helps the baby breathe more regularly. Caffeine also makes the breathing centre respond better when carbon dioxide rises during a breathing pause. This injection is given by a doctor or nurse in a neonatal intensive care setting, with close monitoring of the baby's breathing, heart rate and feeding.

Uses

  • Apnea of prematurity: Premature newborns may have pauses in breathing because breathing control is immature; caffeine stimulates breathing and reduces these episodes under neonatal care.

Contraindications

If you are allergic to any ingredient of this medicine, do not give Cafirate injection. Tell the neonatologist before treatment if the baby has seizures, heart rhythm problems, kidney disease, liver disease or feeding problems. The injection must be given only when the neonatal team has assessed the baby and can monitor the treatment.

Side effects

  • Restlessness or sleep disturbance: Caffeine stimulates the baby's developing nervous system and may make settling difficult or the baby unusually wakeful. Tell the neonatal team if this is marked.
  • Fast heartbeat: Caffeine can increase heart rate while stimulating breathing. The neonatal team should assess persistent fast heartbeat, pallor or worsening feeding immediately.
  • Frequent urination: Caffeine may increase urine output in some babies. Staff should monitor wet nappies and fluid balance rather than giving extra fluids at home.
  • Dehydration: Increased fluid loss may cause fewer wet nappies, a dry mouth or unusual sleepiness. Alert the neonatal team promptly rather than giving fluids without advice.
  • Fever or unusual confusion: Fever or mild delirium can occur during treatment and may be difficult to recognise in a premature baby. Tell the neonatal team immediately.

Precautions and Warnings

Pregnancy

Pregnancy

This medicine is given after birth to premature newborns, not during pregnancy. Tell the treating team about the pregnancy, gestational age and birth history when the baby is assessed.

Breast Feeding

Breast Feeding

Caffeine can pass into breast milk. If the baby is receiving Cafirate injection, ask the neonatal team whether the breastfeeding parent should limit coffee and other caffeine-containing products.

Alcohol

Alcohol

No specific alcohol safety information is available for Cafirate injection. If the baby receives breast milk, ask the neonatal team about alcohol and caffeine intake.

Directions for Use

This injection will be given by a doctor or nurse in a hospital or clinic. Do not self-inject. It must be administered and monitored by neonatal healthcare professionals. Caregivers should follow the unit's instructions and should not give caffeine products to the baby unless specifically advised.

Storage and disposal

The hospital stores Cafirate injection according to its handling requirements. Keep it away from children and do not use a vial that is damaged, cloudy or contains visible particles. Healthcare staff should dispose of unused medicine safely.

Quick Tips

  • Cafirate injection must be given only in a neonatal intensive care setting.
  • Tell the neonatologist about every medicine or supplement given to the baby.
  • Do not give the baby coffee, tea or other caffeine products.
  • If breastfeeding, ask the team about caffeine intake from drinks and supplements.
  • Report unusual jitteriness, vomiting, fast heartbeat or seizures immediately.
  • Keep follow-up monitoring appointments arranged by the neonatal team.

Dosage

Overdose

Excess caffeine may cause marked restlessness, vomiting, fast heartbeat, tremors, high blood sugar, seizures or worsening breathing problems. If you think you have taken too much of this medicine, contact your doctor immediately or visit the nearest hospital.

Missed a Dose

If you missed any dose of medicine, take it as soon as you remember. If it is time for your next dose, then skip the missed dose and continue with your regular dose schedule. Do not take a double dose of medicine to compensate for the missed one.

How does this medicine work?

Caffeine stimulates areas of the premature baby's brain that control breathing. It makes the breathing centre more responsive to signals that breathing needs to increase, helping reduce pauses and improve the regularity of breathing.

Interactions

Interactions with other medicines

Some medicines can affect the way Cafirate injection works. Tell your doctor about all the medicines, vitamins and supplements given to the baby, and tell the neonatologist as well. In particular, mention medicines for seizures such as phenobarbital, medicines that change caffeine breakdown, other stimulants, and a group of antidepressants called monoamine oxidase inhibitors, such as phenelzine or tranylcypromine, which can raise blood pressure when combined. Also report paroxetine, amoxapine or axitinib so the team can review the interaction.

Interactions with food items

No specific food interaction is established for the injection. If the breastfeeding parent is expressing milk for the baby, the medical team may advise avoiding coffee and other caffeine-containing products because caffeine can pass into breast milk.
