Molecule Description
Sunitinib is a prescription oral targeted anticancer medication that belongs to a class of drugs known as receptor tyrosine kinase inhibitors (TKIs). It is primarily used for the treatment of advanced renal cell carcinoma (kidney cancer), imatinib-resistant or intolerant gastrointestinal stromal tumor (GIST), and progressive, well-differentiated pancreatic neuroendocrine tumors (pNET). The medication works by inhibiting specific cellular enzymes (such as VEGF receptors) that are involved in blood-vessel formation and cancer-cell growth.
Treatment with Sunitinib requires medical supervision by an experienced oncologist. Because this medication can significantly affect blood pressure, heart function, thyroid levels, liver health, and blood cell counts, regular clinical monitoring and frequent laboratory blood tests may be required throughout therapy.
Patients receiving this treatment should be informed about potential risks, including hypertension, fatigue, skin and hair color changes, gastrointestinal disturbances, and hand-foot syndrome. Contraindications
When should one not use Sunitinib
- If you have a known hypersensitivity or severe allergic reaction to sunitinib or any of the inactive ingredients in the capsule formulation.
Side effects
Common side effects of this drug are:
- Fatigue and weakness
- Diarrhea, nausea, vomiting, indigestion, and mouth sores
- High blood pressure
- Skin discoloration and hair color changes
- Hand-foot syndrome, which may cause redness, pain, swelling, or blisters on the palms and soles
- Decreased appetite and weight loss
- Low white blood cell, red blood cell, or platelet counts
- Hypothyroidism (underactive thyroid gland)
- Taste disturbances
- Dizziness, headache, and dry skin
- Muscle and joint pain
- Nosebleeds
- Severe high blood pressure: Severe hypertension or a sudden rise in blood pressure may require urgent medical attention.
- Heart problems: Heart failure, reduced heart function, or abnormal heart rhythms may occur.
- Severe bleeding: Serious bleeding, including gastrointestinal bleeding or bleeding in the brain, may occur.
- Liver injury: Severe liver injury or liver failure may occur.
- Heart rhythm changes: Sunitinib may prolong the QT interval and, rarely, cause serious abnormal heart rhythms.
- Gastrointestinal complications: Rarely, gastrointestinal perforation or fistula may occur.

Talk to your doctor if
- You experience a sudden increase in blood pressure, severe headache, chest pain, or changes in vision.
- You develop shortness of breath, swelling of the feet or ankles, dizziness, fainting, or irregular or fast heartbeats.
- You develop signs of bleeding, such as vomiting blood, coughing up blood, black or sticky stools, or blood in your urine.
- You develop severe or persistent abdominal pain, especially if it does not go away.
- Monitor your blood pressure regularly as advised by your doctor and report unusually high readings or symptoms of high blood pressure.
- Report severe skin reactions or painful, blistering, peeling, or severe rash on the palms of your hands or soles of your feet promptly.
- Watch for unusual bleeding or symptoms of liver problems, such as yellowing of the skin or eyes, and seek prompt medical attention.
- Keep all scheduled thyroid function tests and recommended heart monitoring, such as ECGs or echocardiograms.
- Tell all your healthcare providers and dentists that you are taking sunitinib before any surgery, medical procedure, or dental procedure.
Mode of Action
How Does It Work?
Sunitinib is a multi-targeted receptor tyrosine kinase inhibitor. It blocks proteins such as vascular endothelial growth factor receptors (VEGFR) and platelet-derived growth factor receptors (PDGFR), which help cancer cells grow and help tumours develop new blood vessels. By blocking these signals, Sunitinib can slow tumour growth and reduce the blood supply that supports the tumour.
Interactions
Interactions with other medicines
- Ketoconazole, clarithromycin, and ritonavir may increase the amount of sunitinib in your body and increase the risk of side effects.
- Rifampicin, carbamazepine, and St. John’s wort may reduce the amount of sunitinib in your body and make it less effective.
- Inform your doctor about all prescription drugs, over-the-counter medicines, and herbal supplements you are taking, especially blood pressure medications, anticoagulants, or drugs affecting heart rhythm.
Interactions with food items
Sunitinib can generally be taken with or without food. Avoid grapefruit and grapefruit juice unless your doctor advises otherwise, as they may affect sunitinib levels. Maintain adequate fluid intake and follow your doctor's dietary advice.
Dosage
Overdose
If you think you have taken more Sunitinib than prescribed, contact your doctor or seek emergency medical attention. Excessive doses may increase the risk of severe side effects, including high blood pressure, blood-count abnormalities, and gastrointestinal symptoms. Treatment is supportive and based on your symptoms.
Missed a Dose
If you miss a dose of Sunitinib, take it as soon as you remember on the same day. Follow your doctor's instructions. Do not take a double dose to make up for a missed dose.
