Medical Description

Cilapam LS Tab is a prescription medication. It is used in the treatment of depression, anxiety disorders, and panic disorders. Depression is a condition characterised by low mood, decreased motivation, feelings of hopelessness and dread, and loss of interest in daily activities, which can be debilitating. Anxiety is a condition characterised by increased worry and fear about routine situations. It can present with increased heart rate, sweating, restlessness, trembling, increased rate of breathing, etc. Panic disorder is also an anxiety disorder with a feeling of loss of control and fear. Cilapam LS Tab is a combination of two medications, citalopram and clonazepam. Citalopram is an antidepressant, whereas clonazepam is an anxiolytic agent. It is important to note that clonazepam can be habit-forming and has the potential to be misused. Careful monitoring while on this medication is required and necessary. Cilapam LS Tab should be taken as recommended by the doctor. It should not be abruptly started or stopped, as it may cause withdrawal effects. Taking the tablet at the same time every day is advisable for maximum efficacy.

Contraindications

  • If you have known citalopram, clonazepam, or benzodiazepine hypersensitivity or allergies to any components of Cilapam LS Tab.
  • If you have abnormal heart rhythms.
  • If you have been diagnosed with significant liver dysfunction.
  • If you have acute narrow-angle glaucoma.
  • If you have been diagnosed with respiratory insufficiency.
  • If you have congenital blood dyscrasias known as porphyrias.
  • Cilapam LS Tab should be used cautiously in elderly patients and those with a history of frequent falls.
  • Cilapam LS Tab should not be administered to children and adolescents under 18 years of age.

Driving

Alcohol

Using clonazepam(one of the active ingredients of Cilapam LS) with other CNS depressants, like alcohol, increases the risk of respiratory depression, low blood pressure, and death. It is strictly contraindicated.

Other General Warnings

Talk to your doctor if
  • You are taking other prescription medications. This tablet interacts with many other medications that use the same metabolic enzymes.
  • You experience worsening symptoms such as persistent low mood, increased anxieties, worsening obsessive behaviour, and suicidal ideation.
  • You conceive during the course of Cilapam LS Tab or are planning a pregnancy.

Directions for Use

  • Cilapam LS Tab should be taken as prescribed by the treating physician.
  • It is recommended to take Cilapam LS Tab at the same time every day for optimal pharmacological effects.
  • Swallow Cilapam LS Tab as a whole. Do not crush, chew, or break it.
  • It is recommended to finish the entire course of Cilapam LS Tab as prescribed by your treating doctor. It can take up to 2 weeks for maximum efficacy of this medicine to manifest.
  • Discontinuing this medicine abruptly can cause withdrawal side effects. Consult your doctor before stopping this medication.
  • Driving and use of heavy machinery are not advised while using this medication.

Storage and disposal

Store Cilapam LS Tab in a cool and dry place, away from direct light and moisture. Do not use Cilapam LS post the date of expiration. Keep the tablets away from the reach of children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Overdose

The symptoms of an overdose of Cilapam LS Tab may include excessive sleepiness, double vision, slurring of speech, poor motor functions, respiratory failure, fall in BP, increased heart rate, abnormal heart rhythms, coma, and cardiac arrest. In case of a suspected overdose of Cilapam LS, rush the patient to a hospital for further evaluation and treatment.

Missed a Dose

If you have missed a dose of Cilapam LS Tab, it is advised to take it as soon as you recall. If the gap is too long, and it is almost time for the next dose, take the scheduled dose and skip the one you have missed. Do not double the dose of the tablet, as it can lead to an overdose.

Mode of Action

How Does It Work?

Citalopram is an antidepressant. It works by acting on brain pathways that involve the neurotransmitter(chemical messenger) called serotonin. It prevents the reabsorption of serotonin by nerves, a process called reuptake, causing serotonin accumulation. It also has some effect in preventing the reuptake of norepinephrine and dopamine. Clonazepam is a benzodiazepine that acts on the brain pathways to increase the action of a chemical messenger called GABA (gamma-aminobutyric acid) and increases its activity. This results in sedation, calming effects, and muscle relaxation. Clonazepam also increases the levels of serotonin in the brain by increasing its synthesis. These effects are beneficial in the treatment of anxiety.

Interactions

Interactions with other medicines

  • Cilapam LS Tab should not be used with or within 2 weeks of use of monoamine oxidase inhibitors (MAOIs) like selegiline, rasagiline etc., as it may cause serotonin syndrome, which is potentially life-threatening.
  • Patients who are taking other medications that increase the levels of serotonin should be advised regarding the risks.
  • Cilapam LS Tab should not be mixed with medications that stop bleeding, such as aspirin, NSAIDs, heparin, warfarin, other anticoagulants, and other medicines that cause decreased platelet activity. This may lead to bruising, nose bleeds, or internal bleeding.
  • The concomitant use of Cilapam LS with opioids (like morphine, tramadol etc.) is contraindicated, as it may cause respiratory depression, coma, and death.
  • It should also not be used with other CNS depressants like alcohol can also cause similar effects and present life-threatening adverse effects.
  • Inform your doctor about all medicines and supplements you are currently taking at the time of prescription of Cilapam LS Tab.

  • Cilapam LS Tab does not have any known interaction with food items. Unless advised otherwise by the doctor, a regular diet can be continued.
