Medical Description

Zolpirest is a prescription medication. It is used in the short term management of insomnia or sleeplessness. The active component of Zolpirest is Zolpidem (10mg). Insomnia is a condition characterised by persistent problems pertaining to falling and staying asleep. Zolpidem is particularly useful in the short term management of insomnia where there is difficulty in falling asleep. It prevents frequent awakenings at night and maintains adequate sleep duration. Zolpidem is a non-benzodiazepine that can cause sedation. It is essential to note that Zolpidem has habit-forming potential. It also has the potential to be misused. Hence, careful monitoring while on this medication is required and necessary. The medication should be taken as recommended by the doctor. It should not be abruptly started or stopped, as it may cause withdrawal effects. Taking the tablet at the same time every day is advisable for maximum efficacy.

Contraindications

  • It is contraindicated in patients with known zolpidem hypersensitivity or known allergies to any components in this medication.
  • This medication should not be used in patients with any lung condition that results in difficulty in breathing or obstructive sleep apnea (a condition associated with snoring or trouble breathing while sleeping).
  • It is contraindicated for use in children.
  • This medication should not be used in patients with a known history of psychotic symptoms.
  • It is contraindicated in patients with liver failure.
  • It should not be used in patients with Myasthenia Gravis, a condition affecting muscles and respiration.
  • It is contraindicated in patients with rare hereditary problems of galactose intolerance, total lactase deficiency, or glucose-galactose malabsorption.

Pregnancy

This medication is considered unsafe in pregnancy. It is a Category C medication. This means that studies have shown that it can affect the growing foetus. Inform your doctor about your pregnancy, or intention to conceive at the time of prescription.

Driving

Driving and operation of other heavy machinery while on this drug is not recommended as it causes light-headedness, sedation and sleepiness. There are also reports of sleep-driving and sleepwalking associated with Zolpidem.

Alcohol

mixing alcohol with Zolpirest can cause life-threatening side effects on the user, as it can lead to respiratory depression, coma, and even death. It is strictly contraindicated.

Talk to your doctor if
  • If symptoms do not improve within 7-10 days of usage, inform your doctor immediately.
  • You should talk to your doctor if you have a history of addiction or substance misuse.
  • You should inform your doctor in case you are taking any other medications. The ingredients of this tablet may react with several other tablets that use the same pathway of metabolism in the body.
  • Inform your doctor if you experience any mood changes.

Directions for Use

  • This medication should be taken as prescribed by the treating physician.
  • It is recommended to take this medication on a regular schedule, i.e., at the same time every day, for optimal pharmacological effects of this medication.
  • Swallow this medication as a whole. Do not crush, chew or break this tablet.
  • If symptoms do not improve within 7-10 days of usage, inform your doctor immediately.
  • Take this medication before food. The absorption of Zolpidem is decreased by taking it with or immediately after food.
  • Abrupt stoppage of Zolpirest can cause withdrawal side effects. Consult your doctor before stopping this medication.
  • Driving/ use of heavy machinery is not advised while using this medication.
  • Do not mix alcohol with this medication, as it can be life-threatening.
  • This medication has habit-forming tendencies. Hence it is recommended to follow the directions of your treating physician precisely and have regular follow-ups.

Storage and disposal

Store in a cool and dry place away from direct light and moisture. Do not use the medicine after the date of expiration. Keep the tablets away from children and pets. Do not flush the medication down the toilet. Dispose of it with biomedical waste only.

Dosage

Overdose

Symptoms may include excessive sleepiness, double vision, slurred speech and impaired movements. This may proceed to respiratory failure, fall in BP and heart rate, coma, cardiac arrest and even death. In case of a suspected overdose, rush the patient to a tertiary care centre with ICU facilities for further evaluation and treatment.

Missed a Dose

If you miss a dose of this tablet, it is advised to take it as soon as possible. If the duration missed is too long, and it is almost time for the next dose, it is recommended to skip the missed dose and go back to the regular schedule. Do not double the dose of the tablet.

Mode of Action

How Does It Work?

Zolpidem is a non-benzodiazepine sedative and hypnotic agent. It produces sedation by affecting certain chemical messengers or neurotransmitters. The most significant neurotransmitter affected by Zolpidem is GABA (gamma-aminobutyric acid). It affects this neurotransmitter in specific parts of the brain that aid in the maintenance of deep sleep. It is important to note that Zolpidem has addictive potential and can be a substance of misuse/ abuse. Careful monitoring is essential while on this medication.

Interactions

Interactions with other medicines

  • Combined use with central nervous system depressants like alcohol causes increased psycho-motor side effects.
  • The use of zolpidem with Imipramine, a tricyclic antidepressant, decreases one's alertness.
  • Impaired alertness is observed in patients who use zolpidem and chlorpromazine.
  • The concomitant use of Rifampin and Zolpidem decreases the efficacy of the latter.
  • Concomitant use of Ketoconazole and Zolpidem can decrease the efficacy of zolpidem.
  • The concomitant use of this medication with opioids (like morphine, tramadol etc.) is contraindicated, as it may cause respiratory depression, coma and death.
  • Inform your doctor regarding your current medications at the time of prescription of Zolpirest.
