SGPT, also referred to as ALT (alanine aminotransferase), is an enzyme produced primarily by liver cells, though trace amounts are also present in tissues, such as the heart, kidneys, and muscles. The SGPT/ALT blood test measures SGPT/ALT levels and is routinely used to evaluate liver health and screen for potential liver-related concerns. The liver is a vital organ that carries out a wide range of functions essential to overall health. It produces bile to support fat digestion, filters harmful substances and metabolic waste from the blood, stores key vitamins and nutrients, aids in blood clotting, and helps regulate blood sugar. It is also responsible for processing dietary proteins, fats, and carbohydrates, and plays a central role in metabolising medications so they can be safely eliminated from the body. Under normal circumstances, ALT circulates in the blood at low concentrations. However, when liver cells are stressed, inflamed, or damaged, this enzyme can leak into the bloodstream in larger quantities, causing levels to rise above the normal range. Elevated SGPT/ALT readings may be linked to a range of conditions, including non-alcoholic fatty liver disease, viral hepatitis, alcohol-induced liver injury, drug or medication-related liver toxicity, or other underlying liver disorders. Because a raised ALT value alone is not sufficient to determine the root cause of liver injury, your doctor may suggest further diagnostic tests to guide a more complete evaluation.
When and Who Should Get a SGPT/ALT Test Done?
- Routine health screening: The test may be included as part of a standard preventive health check-up, even in the absence of symptoms.
- Presence of liver disease risk factors: Testing may be recommended if you have known risk factors such as regular or heavy alcohol consumption, a family history of liver conditions, obesity, type 2 diabetes, or prolonged use of medications known to affect liver function.

SGPT/ALT Test Preparation
Before an SGPT/ALT Test:
- Inform your doctor about all medications and supplements as certain substances can influence ALT readings. However, do not stop or adjust any medication without medical guidance.
- In the period leading up to the test, it is advisable to avoid large meals, high-fat foods, sugary drinks, and caffeinated beverages, as these may temporarily affect test results.

What to Expect Before the Test
- The SGPT/ALT test does not require fasting on its own. However, if it is being conducted alongside other blood investigations or as part of a comprehensive health check-up package, your doctor may instruct you to avoid food and beverages for a specified period before the test.
What to Expect During the Sample Collection
- A trained phlebotomist from PharmEasy will arrive at your home at the scheduled time. They will take a blood sample from a vein in your arm, using a small needle. After the needle is inserted, a small amount of blood will be collected into a test tube or vial. You may feel a little sting when the needle goes in or out. This usually takes less than five minutes.
What to Expect After the Test
- Mild throbbing may be felt at the blood collection site after the blood is drawn, which will get better in a few minutes. Occasionally, you may feel dizzy, in which case you may lie down for a while until the feeling passes.
