The SGPT/ALT test, also known as the ALT test (alanine aminotransferase test or alanine transaminase test), is a blood test used to measure the level of the ALT enzyme in your bloodstream. ALT is found mainly in the liver, although small amounts are also present in the muscles, kidneys, and other tissues. This test is commonly performed to evaluate liver health and identify possible signs of liver damage or liver disease. The liver is one of the body’s most important organs and performs several essential functions. It helps digest fats by producing bile, filters toxins and waste products from the blood, supports blood clotting, stores important vitamins and minerals, and helps regulate blood sugar levels. Since the liver plays a key role in maintaining overall health, monitoring its function is important. Normally, ALT levels in the blood remain low. However, when liver cells become damaged due to conditions such as fatty liver disease, hepatitis, alcohol use, infections, medications, or other liver-related conditions, ALT can leak into the bloodstream and cause levels to rise. An SGPT/ALT blood test can help detect liver injury early, sometimes before symptoms appear, allowing timely diagnosis and management of liver health concerns.
When and Who Should Get a SGPT/ALT Test Done?
Your doctor may recommend an SGPT/ALT blood test for several reasons, including:
- As part of a routine health check-up or preventive health screening to monitor your overall health and assess liver function.
- To screen for liver-related conditions, often as part of a liver function test (LFT) panel, especially if you have risk factors such as alcohol consumption, a family history of liver disease, diabetes, obesity, or long-term use of medications that may affect the liver.
- If you have symptoms that may suggest liver problems, including nausea, vomiting, abdominal discomfort, unexplained fatigue, loss of appetite, itchy skin, yellowing of the skin or eyes (jaundice), dark urine, or pale-coloured stools.

The recommended frequency for an SGPT/ALT test can vary depending on your health condition, medical history, and your doctor’s advice.
- Healthy adults may consider getting an SGPT/ALT test once a year as part of their routine preventive health check-up to monitor overall health and liver function.
- Individuals taking medications that may affect liver health, such as cholesterol-lowering medicines, anti-tubercular drugs, long-term painkillers, or anti-seizure medications, may require testing every 1 to 3 months or as advised by their doctor.
- People with active liver conditions, including hepatitis or fatty liver disease, may need more frequent monitoring, often on a monthly basis or according to medical recommendations.
- Those with heavy alcohol consumption may be advised to undergo SGPT/ALT testing every 3 to 6 months to track liver enzyme changes and monitor liver health over time.
- After completing treatment for a liver-related condition, doctors may recommend a follow-up SGPT/ALT test within 4 to 6 weeks to assess recovery and treatment response.

SGPT/ALT Test Preparation
To help ensure accurate SGPT/ALT test results, it is important to follow any instructions provided by your doctor. You may also keep the following points in mind before your test:
- Inform your doctor about all regular medications and dietary supplements you are taking, as many of these cause changes to the ALT levels in the blood. However, avoid stopping any medication without advice from your doctor.
- Heavy or high-fat meals and sugary or caffeinated beverages may temporarily affect the ALT levels, so try to avoid these before the test.
- Avoid alcohol consumption for at least 24 hours before the test, as alcohol may temporarily increase ALT levels.
- Vigorous exercise can sometimes affect ALT readings. Inform your doctor if you do strenuous workouts.
What to Expect Before the Test
- Fasting is not necessary for the SGPT/ALT test, but if it is done along with other blood tests, your doctor may ask you to avoid eating or drinking for a specific period before sample collection.
What to Expect During the Sample Collection
- A phlebotomist will collect a small blood sample from a vein, usually in the arm, using a sterile needle. The blood is then transferred into a collection tube for laboratory analysis. Some individuals may experience a brief pinching or stinging sensation during needle insertion or removal. The entire sample collection process is typically completed within a few minutes.
What to Expect After the Test
- You may feel a mild throbbing pain which will pass in a few minutes. The sample will then be sent to a lab to be analysed. Your test results should be available for download within a few hours on the PharmEasy platform.

SGPT/ALT Test Risks and Limitations
Potential Risks and Complications
There is minimal risk involved with having blood taken for the SGPT/ALT test. Blood vessels may vary in size from one person to another and from one side of the body to the other. Taking blood from some people may be more difficult than from others.
Other risks associated with having blood drawn are slight, but may include:
- Fainting or feeling light-headed
- Multiple punctures to locate veins
- Bleeding
- Infection (a slight risk any time the skin is broken)
Understanding SGPT/ALT Test Limitations
- Mildly elevated SGPT/ALT levels are not specific to liver damage alone, they can temporarily be elevated even in situations such as having heavy meals, vigorous exercise, or consuming alcohol before the test and in cases of long-term use of certain medications.
- Additional investigations may be needed to determine the underlying reason for liver damage as this test does not determine its cause.
- In advanced liver scarring (cirrhosis), sometimes SGPT/ALT levels may not be elevated as expected, as extensive fibrosis might have already occurred. A FibroScan or a liver biopsy may be recommended to assess liver fibrosis.
- A normal SGPT/ALT level does not completely rule out liver disease, especially if symptoms are present.
- Rarely, the test may show elevated levels when there is damage in other parts of the body.
