The SGPT/ALT test, also called the serum glutamic-pyruvic transaminase (SGPT) or alanine transaminase (ALT) test, is a blood test that measures the level of the ALT enzyme in your bloodstream. ALT is primarily present in the liver and helps the body process proteins and carry out important metabolic functions. Normally, ALT levels in the blood remain low. However, when liver cells are damaged, irritated, or inflamed, they can release ALT into the bloodstream, leading to increased levels. Elevated SGPT/ALT levels may be a sign of liver stress, liver injury, or certain liver-related conditions, sometimes even before symptoms become noticeable. Healthcare providers often recommend an SGPT/ALT test to assess liver health, detect potential liver damage, monitor existing liver conditions, or track the effectiveness of ongoing treatment. It is also commonly included as part of a liver function test (LFT) panel to provide a broader evaluation of liver function.
When and Who Should Get a SGPT/ALT Test Done?
An SGPT/ALT test is used to assess liver health and identify possible signs of liver damage. Measuring ALT levels can help detect liver-related conditions at an early stage, sometimes before any visible symptoms develop. Your doctor may recommend an SGPT/ALT test if you:
- Experience symptoms that could indicate liver problems, such as abdominal discomfort, yellowing of the skin or eyes (jaundice), persistent fatigue, dark urine, pale stools, or unexplained itching.
- Are taking medications or dietary supplements that may impact liver function.
- Have conditions such as obesity, high cholesterol, or type 2 diabetes, which can increase the risk of certain liver diseases.

The SGPT/ALT test may help with the diagnosis of:
- Alcoholic liver disease.
- Inflammation or swelling of the liver.
- Scarring of the liver, called cirrhosis.
- Hepatitis A, B, or C.
- Infectious mononucleosis.
- Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly called nonalcoholic fatty liver disease (NAFLD).
- Metabolic dysfunction-associated steatohepatitis (MASH), formerly called nonalcoholic steatohepatitis (NASH).

SGPT/ALT Test Preparation
- Inform your doctor about any prescription medicines, over-the-counter drugs, and dietary supplements that you take regularly.

- If you regularly consume large amounts of coffee, tea, energy drinks, or other caffeinated beverages, make sure to mention it to your doctor.

- Avoid Intense exercise shortly before the test.

What to Expect Before the Test
- If your SGPT/ALT test is being done along with other blood tests, you may be asked to avoid eating or drinking for a specific period before sample collection.
What to Expect During the Sample Collection
- A phlebotomist will arrive at your home for sample collection.
- An elastic band will be placed around your arm and you may be asked to make a fist. Using a sterile needle, a small blood sample will be drawn from a vein in your arm, and you may be advised to apply mild pressure at the blood draw site using a cotton swab.
- The procedure usually takes only a few minutes.
What to Expect After the Test
- The sample will then be sent to the lab to be analysed. Your test results should be available within a few hours which, you can access from the PharmEasy platform.

Potential Risks and Complications
- The SGPT/ALT test is generally safe.
- Minor risks involved with a blood draw may occasionally occur such as mild pain, slight bleeding, bruising, dizziness, or light-headedness. Sometimes, vein access may be difficult.
Understanding SGPT/ALT Test Limitations
- An SGPT/ALT test alone cannot determine the exact reason for liver damage. Different conditions such as viral hepatitis, fatty liver disease, alcohol-related liver problems, medication-related liver injury, and autoimmune liver disorders may all lead to elevated SGPT levels. Additional diagnostic tests are often needed to identify the underlying cause.
- In cases of advanced liver scarring (cirrhosis), SGPT levels may sometimes remain within the normal range because a large number of liver cells may already be damaged. Therefore, SGPT levels alone do not reflect the severity of liver fibrosis, which may require tests such as FibroScan or a liver biopsy for further evaluation.
- Certain factors including strenuous physical activity, alcohol intake, and some medications can temporarily increase SGPT levels.
- A normal SGPT result does not always exclude the possibility of liver disease, particularly if symptoms, medical history, or other risk factors are present.
