A SGPT / ALT Test Reference Ranges

A SGPT / ALT Test Reference Ranges

Overview

Overview

The SGPT/ALT test, also called the serum glutamic-pyruvic transaminase (SGPT) or alanine transaminase (ALT) test, is a blood test that measures the level of the ALT enzyme in your bloodstream. ALT is an enzyme mainly found in the liver, though it may be found in other parts of your body. This enzyme helps process proteins and aids important metabolic functions in your body. Inhealthy adults, ALT levels in your blood remain low. However, when your liver cells are damaged or inflamed, they can release ALT into your bloodstream, leading to increased levels of ALT in your blood. Your doctor may recommend an SGPT/ALT test to assess your liver health, check for potential liver damage, or track the effectiveness of ongoing treatment. This test is also usually included in the liver function test (LFT) panel or comprehensive metabolic panel to provide a broader evaluation of liver health. Other common names for an ALT blood test include alanine aminotransferase and GPT.

When and Who Should Get a SGPT/ALT Test Done?

When and Who Should Get a SGPT/ALT Test Done?

SGPT/ALT tests are often included in routine blood panel tests that assess your general overall health, such as a comprehensive metabolic panel.

Your doctor may recommend screening with a liver panel blood test that includes an ALT test if you have risk factors for liver disease, such as:

  • Heavy alcohol use.
  • Family history of liver disease.
  • Diabetes.
  • Obesity.
  • Risk factors for viral hepatitis, such as exposure to infected blood, sharing needles, unprotected sex, or other situations that may increase the risk of infection.
  • Medications that can affect liver health.

If you have a liver condition, your doctor may advise an SGPT/ALT test to monitor your condition to see if it is improving, worsening, or staying the same with or without treatment.

Your doctor may also advise the ALT test if you have symptoms of liver disease such as:

  • Nausea or vomiting.
  • Abdominal pain.
  • Itchy skin.
  • Jaundice (a yellowing of your skin and the whites of your eyes).
  • Tiredness (fatigue).
  • Loss appetite.
How Frequently Should You Take the SGPT/ALT Test?

4 to 6 weeks after completing treatment

To confirm liver fitness before surgery

Once, within 48 to 72 hours before the surgery

What Conditions Can a SGPT/ALT Test Help Detect?

What Conditions Can a SGPT/ALT Test Help Detect?

The SGPT/ALT test may help with diagnosing:

  • Alcohol-induced liver injury.
  • Inflammation or swelling of the liver.
  • Scarring of the liver, called cirrhosis.
  • Hepatitis A, B, or C.
  • Infectious mononucleosis.
  • Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly called nonalcoholic fatty liver disease (NAFLD).
  • Metabolic dysfunction-associated steatohepatitis (MASH), formerly called nonalcoholic steatohepatitis (NASH).
  • As part of a complete blood work-up if liver cancer is suspected.
  • Liver ischemia (not enough blood flow to your liver, which may lead to death of liver tissue
  • Hemochromatosis (too much iron in the blood)

Disclaimer: The above list is for educational purposes only and does not confirm diagnosis or the presence of any condition. Always consult your doctor for your test results and diagnosis.

SGPT/ALT Test Preparation

SGPT/ALT Test Preparation

  • A lot of medications and dietary supplements can affect your ALT levels. Your doctor should be informed about the same. Only stop medications upon advice from your doctor.
  • Avoid heavy meals and sugary drinks before the test.
  • Avoid coffee, tea, energy drinks, and other caffeinated beverages before the test.
  • Avoid alcohol consumption for at least 24 hours before your test. Alcohol can temporarily raise ALT levels.
  • Inform your doctor if you frequently do demanding physical workouts as these can alter your ALT levels.

For the most accurate results, follow any specific instructions provided by your doctor or diagnostic lab.

What to Expect Before the Test

  • If your ALT test is being done along with other blood tests, you may be asked to avoid eating or drinking for a specific period before sample collection.

What to Expect During the Sample Collection

  • The phlebotomist will visit your home at the scheduled time.
  • Once you are seated, the phlebotomist will check your arms for an easily accessible vein. This is usually in the inner part of your arm on the other side of your elbow.
  • Once the vein is located, they will clean and disinfect the area.
  • They will then insert a small needle into your vein to take a blood sample. This may feel like a small pinch.
  • After they insert the needle, a small amount of blood will be collected in a test tube.
  • Once they have enough blood to test, they will remove the needle and hold a cotton ball or gauze on the site to stop any bleeding.
  • They will place a bandage over the blood draw site. The entire process usually takes less than five minutes.

What to Expect After the Test

  • Mild throbbing at the site may be felt after the blood is drawn. The sample will be sent to the lab to be analysed. Your test results should be available within a few hours, which you can access from the PharmEasy platform.

Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.

What are the Parameters Included in a SGPT/ALT Test?

What are the Parameters Included in a SGPT/ALT Test?

The SGPT/ALT test measures a single key parameter:

  • SGPT/ALT (alanine aminotransferase), reported in units per litre (U/L).
Normal Range of SGPT/ALT Test Parameters

Normal Range of SGPT/ALT Test Parameters

The table below shows standard reference ranges of SGPT/ALT levels.

Parameter

Normal range

SGPT/ALT (alanine aminotransferase)

Less than 45 U/L (units per litre)

Important note: SGPT values can be slightly higher in men than women, and in individuals with higher muscle mass. Levels are also marginally higher in children, especially infants. Normal ranges may vary slightly between laboratories. Always refer to the reference range printed on your specific lab report and consult your doctor for a personalised interpretation.

SGPT/ALT Test Result Interpretation

SGPT/ALT Test Result Interpretation

SGPT results must always be interpreted in the context of your gender, symptoms, alcohol use, medications, and the degree of elevation.

SGPT level

Category

Potential interpretation

Below 45 U/L

Normal

Liver cells are not significantly damaged. No evidence of active liver injury at this time.

45 - 120 U/L

Mildly elevated

May indicate early fatty liver, mild hepatitis, medication side effects, or moderate alcohol use.

120 - 400 U/L

Moderately elevated

Suggests significant liver inflammation or injury. Commonly seen in active viral hepatitis, alcoholic hepatitis, or moderate fatty liver.

Above 400 U/L

Severely elevated

Indicates serious liver cell damage. Seen in acute viral hepatitis, paracetamol toxicity, ischaemic hepatitis, or severe autoimmune hepatitis.

Above 1000 U/L

Very severely elevated

Suggests massive liver cell destruction, such as acute liver failure, severe drug toxicity, or toxic shock. This is a medical emergency.

Disclaimer: This table is for educational purposes only. SGPT/ALT interpretation requires the expertise of a healthcare professional.

 SGPT/ALT Test Risks and Limitations

SGPT/ALT Test Risks and Limitations

Potential Risks and Complications

  • The SGPT/ALT test is generally safe.
  • Minor risks involved with a blood draw may occasionally occur such as mild pain, slight bleeding, bruising, dizziness, or light-headedness. Sometimes, vein access may be difficult.

Understanding SGPT/ALT Test Limitations

  • Having a high SGPT/ALT test result does not necessarily mean you have a medical condition. Very few people with elevated ALT levels have severe liver conditions.
  • An SGPT/ALT test alone cannot determine the exact reason for liver damage. Additional diagnostic tests are often needed to identify the underlying cause.
  • In cases of advanced liver scarring (cirrhosis), SGPT levels may not rise sometimes because a large number of liver cells may already be damaged. FibroScan or a liver biopsy are advised for further evaluation.
  • Factors such as excessive physical activity, alcohol consumption, and some medications can temporarily increase SGPT levels.
  • A normal SGPT result does not always exclude the possibility of liver disease, particularly if symptoms, medical history, or other risk factors are present.
  • Rarely, elevated ALT levels can also indicate injury to cells in other parts of your body, since ALT is not found only in the liver.
