The SGPT/ALT test, also known as the alanine aminotransferase test (ALT) or alanine transaminase test, measures the level of the ALT enzyme in your blood. The liver performs many essential functions in the body, such as helping in digestion by producing bile, filtering toxins in the blood, aiding in blood clotting, storing certain vitamins and minerals, and regulating blood sugar levels. The liver also produces ALT, an enzyme that helps in the metabolism of protein. In healthy individuals, ALT is present predominantly inside the liver cells, and hence the level of ALT in the blood is low. However, in cases where liver injury or disease may have occurred, such as fatty liver disease, hepatitis, heavy alcohol use, infections, long-term use of medications, or other liver-related conditions, ALT is released into the bloodstream from the damaged liver cells. Since the liver plays a crucial role in maintaining your health, monitoring its function is important. An SGPT/ALT blood test can help detect liver injury early, sometimes even before symptoms appear, allowing timely diagnosis and management of liver conditions.
When and Who Should Get a SGPT/ALT Test Done?
An SGPT/ALT blood test may be recommended to assess liver health, identify possible liver-related concerns, and monitor liver function over time. An SGPT/ALT test can support the early detection and monitoring of liver health concerns, even before symptoms become noticeable. Your doctor may advise this test in the following situations:
- As part of a routine health check-up or preventive screening to evaluate overall health and monitor liver function.
- To screen for liver conditions, often through a liver function test (LFT) panel, particularly if you have risk factors such as regular alcohol consumption, a family history of liver disease, diabetes, obesity, or prolonged use of medications that may affect the liver.
- If you are experiencing symptoms that could be linked to liver problems, including nausea, vomiting, abdominal pain, unexplained tiredness, reduced appetite, itchy skin, jaundice (yellowing of the skin or eyes), dark urine, or pale stools.
- If you have factors that may increase the risk of viral hepatitis, such as exposure to infected blood or other situations associated with a greater risk of infection.
- To monitor an existing liver condition and track whether liver health is improving, stable, or changing over time during or after treatment.
How Frequently Should You Take the SGPT/ALT Test?
The recommended frequency for an SGPT/ALT test can vary depending on your health condition, medical history, and your doctor’s advice.
- Healthy adults: once a year as part of routine preventive health check-up to monitor overall health and liver function.
- Individuals taking medications: those taking medications that may affect the liver, such as cholesterol-lowering medicines, anti-tubercular drugs, long-term painkillers, or anti-seizure medications, may require testing every 1 to 3 months or as advised by their doctor.
- People with active liver conditions: once a month or according to doctor’s recommendations.
- Those with heavy alcohol consumption: every 3 to 6 months to track liver enzyme changes and monitor liver health over time.
- After completing treatment for a liver-related condition: follow-up test within 4 to 6 weeks to assess recovery and treatment response.
- Before surgical procedure: an SGPT/ALT test may be performed, usually within 48 to 72 hours before surgery.
What Conditions Can a SGPT/ALT Test Help Detect?
The SGPT/ALT test may help with diagnosing:
- Alcohol-induced liver injury.
- Inflammation or swelling of the liver.
- Scarring of the liver, called cirrhosis.
- Hepatitis A, B, or C.
- Infectious mononucleosis.
- Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly called nonalcoholic fatty liver disease (NAFLD).
- Metabolic dysfunction-associated steatohepatitis (MASH), formerly called nonalcoholic steatohepatitis (NASH).
- As part of a complete work-up if liver cancer is suspected.
- Liver ischemia (not enough blood flow to your liver which may lead to death of liver tissue.
- Hemochromatosis (too much iron in the blood).

SGPT/ALT Test Preparation
Most people do not require any special preparation before an SGPT/ALT blood test. However, following your doctor’s instructions and keeping a few important points in mind may help ensure more accurate test results:
- Inform your doctor about all prescription medications, over-the-counter medicines, and dietary supplements you regularly take, as some of these may influence ALT levels. Do not stop any medication unless specifically advised by your healthcare provider.
- Try to avoid heavy or high-fat meals, as well as sugary and caffeinated beverages before the test, since these may temporarily affect ALT readings.
- Avoid alcohol consumption for at least 24 hours before your test, as alcohol can temporarily raise ALT levels.
- Intense physical activity or strenuous exercise may influence ALT results. Inform your doctor if you regularly participate in demanding workouts or exercise routines.
What to Expect Before the Test
- Fasting is not necessary for the SGPT/ALT test, but if it is done along with other blood tests, your doctor may ask you to avoid eating or drinking for a specific period before sample collection.
What to Expect During the Sample Collection
- This test involves blood sample collection. A phlebotomist or other trained professional will insert an intravenous (IV) needle into a vein (usually a vein in your arm) and fill up one or more test tubes with blood. A slight sting may be felt when the needle is inserted and removed.
What to Expect After the Test
- Mild throbbing at the site may be felt after the blood is drawn, which will pass in a few moments. Once the sample is analysed in the laboratory, the test reports will be uploaded to the PharmEasy platform after review by a medical practitioner. SGPT/ALT Test Risks and Limitations
Potential Risks and Complications
- Minor risks involved with a blood draw may occasionally occur, such as mild pain, slight bleeding, bruising, dizziness, or light-headedness. Rarely, vein access may be difficult.
- Medical attention should be sought if persistent bleeding, swelling, redness, or severe pain develops.
Understanding SGPT/ALT Test Limitations
- Mildly elevated SGPT/ALT levels do not always indicate liver damage. ALT levels can temporarily increase due to factors such as heavy meals, strenuous exercise, alcohol intake, or the long-term use of certain medications.
- An SGPT/ALT test alone cannot identify the exact cause of liver injury or abnormal liver function. Additional tests and clinical evaluation may be required to determine the underlying reason.
- In advanced liver scarring (cirrhosis), SGPT/ALT levels may sometimes remain lower than expected because significant liver fibrosis and liver cell damage may already be present. Further tests, such as FibroScan or a liver biopsy, may be recommended for a more detailed assessment.
- A normal SGPT/ALT result does not completely exclude the possibility of liver disease, particularly if symptoms, medical history, or risk factors suggest an underlying concern.
- Although ALT is found mainly in the liver, smaller amounts are present in other tissues. In rare cases, elevated ALT levels may also occur due to injury or conditions affecting other parts of the body.
