Overview

The rheumatoid factor test is a blood test that checks the amount of rheumatoid factor (RF), an autoantibody, in the bloodstream. It is detected in about 70-80% of people with rheumatoid arthritis (RA), although it can also occur in other conditions. Unlike antibodies that protect the body by responding to foreign substances such as bacteria, autoantibodies can react with the body’s own proteins and may contribute to inflammation. RF does not directly damage the joints in RA. It binds to part of an antibody called IgG and may contribute to the inflammation in the joints, and sometimes, other parts of the body. Doctors may advise the rheumatoid factor test when RA is suspected, particularly when symptoms such as persistent fatigue, mild fever, unexplained weight loss, muscle aches, tingling in the hands, or prolonged morning stiffness are present. RA commonly affects joints in the hands and feet and may also involve the knees and shoulders. Because these symptoms can occur with other inflammatory conditions, RF testing is considered along with the clinical examination, and other investigations to support diagnosis and guide treatment. If RA remains untreated, ongoing inflammation can damage the cartilage and bones, leading to reduced joint function and in some cases, deformities. RA is more common in women than men, while factors such as smoking may worsen the condition. RF testing may also be useful when evaluating certain other autoimmune or chronic inflammatory conditions, including lupus and Sjögren syndrome.

  • Sjögren’s syndrome: An autoimmune condition that mainly affects the glands that produce tears and saliva, often causing dry eyes and dry mouth.
  • Systemic lupus erythematosus (SLE): An autoimmune condition that can cause inflammation in different parts of the body, including the joints, skin, kidneys, and other organs.
  • Scleroderma (systemic sclerosis): An autoimmune condition that can cause hardening and tightening of the skin and may affect internal organs.

  • Healthy individuals: A positive RF can occasionally be found in people who do not have an underlying disease, particularly with increasing age.

Note: A positive RF result does not confirm any of these conditions. Doctors interpret it along with symptoms, examination findings and when needed, other tests.
