Overview
The rheumatoid factor test is a blood test that measures rheumatoid factor (RF), an autoantibody produced by the immune system and commonly associated with rheumatoid arthritis (RA). Normally, the immune system defends the body by identifying and attacking harmful invaders such as bacteria and viruses. However, in certain conditions, it can mistakenly attack the body’s own healthy tissues, a process known as autoimmunity. The antibodies involved in this misdirected response are called autoantibodies. RF is one such autoantibody most commonly linked to rheumatoid arthritis, a chronic autoimmune disorder that primarily affects the joints, leading to symptoms such as pain, swelling, and stiffness. While RA can develop at any age, it is more frequently diagnosed in adulthood, especially during the middle years of life. Elevated RF can also appear in other autoimmune, inflammatory, or infectious conditions, and occasionally in individuals with no underlying disease at all. Doctors typically order this test when RA or a related condition is suspected. They will interpret the result alongside the patient’s medical history, symptoms, and physical examination findings. Depending on these findings, additional blood work or imaging, such as X-rays, may be recommended to further assess joint health.
When and Who Should Get a Rheumatoid Factor Test?
The rheumatoid factor test may be recommended for people who have symptoms that could suggest rheumatoid arthritis or another condition associated with rheumatoid factor. A doctor may consider the test when a person has:
- Persistent joint pain, swelling, or tenderness, especially in multiple joints.
- Joint stiffness that is worse in the morning or after periods of inactivity.
- Symptoms affecting both sides of the body, such as both hands, wrists, or feet.
- Hard bumps (rheumatoid nodules) under the skin near the joints.
- Unexplained symptoms such as prolonged fatigue, fever, or weakness along with joint symptoms.
- Signs or symptoms that raise suspicion of another autoimmune, inflammatory, or infectious condition associated with rheumatoid factor.
Note: This test is not generally recommended as a routine screening test for healthy people without symptoms. Your doctor will decide whether it is appropriate based on your symptoms, medical history and physical examination.
What Conditions Can Cause a Positive Rheumatoid Factor Test?
A positive RF result may be seen in:
- Rheumatoid arthritis (RA): The RF test is most commonly used along with other tests when this condition is suspected. A chronic autoimmune condition in which the immune system attacks the joints, causing pain, swelling, and stiffness.
Other conditions where RF may be detected:
- Sjögren’s syndrome: An autoimmune condition that mainly affects the glands that produce tears and saliva, often causing dry eyes and dry mouth.
- Systemic lupus erythematosus (SLE): An autoimmune condition that can cause inflammation in different parts of the body, including the joints, skin, kidneys, and other organs.
- Scleroderma (systemic sclerosis): An autoimmune condition that can cause hardening and tightening of the skin and may affect internal organs.
- Mixed connective tissue disease: A rare autoimmune condition with features that may overlap with conditions such as lupus, scleroderma, and polymyositis.
- Juvenile idiopathic arthritis (JIA): A group of inflammatory arthritis conditions that affect children and can cause persistent joint pain, swelling, and stiffness.
- Chronic infections: Conditions such as hepatitis C, tuberculosis, and infective endocarditis can sometimes cause elevated RF.
- Sarcoidosis: An inflammatory condition in which clusters of immune cells form in organs, commonly affecting the lungs and lymph nodes.
- Certain cancers, including leukaemia: Some cancers, including leukaemia, and multiple myeloma, can be associated with elevated RF. However, the RF test cannot be used to diagnose cancer or identify its type.
- Healthy individuals: A positive RF can occasionally be found in people who do not have an underlying disease, particularly with increasing age.
Note: A positive RF result does not confirm any of these conditions. Doctors interpret it along with symptoms, examination findings and, when needed, other tests.

What to Expect Before the Rheumatoid Factor Test?

- The blood sample for this test can be taken at any time of the day.

What to Expect During the Blood Collection?
- This test involves a blood draw for a sample.
- A phlebotomist will insert a sterile (IV) needle into a vein (usually a vein in your arm) and fill up one or more test tubes with blood.
- A slight sting may be felt when the needle is inserted and removed.
What to Expect After the Rheumatoid Factor Test?
- Mild throbbing at the site may be felt after the blood is drawn, which will pass in a few moments.

Understanding Rheumatoid Factor Test Limitations
- RF is not specific to rheumatoid arthritis; it may also be detected in other autoimmune diseases, infections, certain cancers, or even in some healthy individuals.
- Even if the RF test results are negative, it still does not rule out RA completely because sometimes people with RA, especially in the early stages, may have normal or undetectable RF levels.
- The RF test results usually have to be assessed together with presenting symptoms, physical examination, and other tests, such as anti-CCP, ESR, or CRP.
- The level of RF does not determine the severity of disease.
- Reference ranges can vary between laboratories; therefore, your results should always be interpreted as per the reference ranges in your report provided by the laboratory that performed the test.
