Know More About The Test
A quick info on RA Factor test
Overview
The rheumatoid factor test is a blood test that measures the level of rheumatoid factor, an autoantibody produced by the immune system. Normally, the immune system protects the body from harmful substances such as bacteria and viruses. In autoimmune conditions, the immune system can mistakenly recognise the body’s own components as targets and produce autoantibodies against them. Rheumatoid factor is one such autoantibody. It reacts with a part of an antibody called immunoglobulin G (IgG), forming complexes that can contribute to inflammation when present in excess. RF is commonly associated with rheumatoid arthritis (RA), a chronic autoimmune condition in which inflammation mainly affects the joints, causing pain, swelling, and stiffness. RF can also be present in other autoimmune, inflammatory, or infectious conditions and, occasionally, in healthy people. An increased RF level does not by itself confirm rheumatoid arthritis. Doctors interpret the results along with your symptoms, medical history, and physical examination and may recommend other blood tests, such as anti-CCP, ESR, or CRP, or imaging such as an X-ray, depending on your clinical findings.
When and Who Should Get a Rheumatoid Factor Test?
The rheumatoid factor test may be recommended for people who have symptoms that could suggest rheumatoid arthritis or another condition associated with rheumatoid factor. A doctor may consider the test when a person has:
- Persistent joint pain, swelling, or tenderness, especially in multiple joints.
- Joint stiffness that is worse in the morning or after periods of inactivity.
- Symptoms affecting both sides of the body, such as both hands, wrists, or feet.
- Hard bumps (rheumatoid nodules) under the skin near the joints.
- Unexplained symptoms such as prolonged fatigue, fever, or weakness along with joint symptoms.
- Signs or symptoms that raise suspicion of another autoimmune, inflammatory, or infectious condition associated with rheumatoid factor.
Note: This test is not generally recommended as a routine screening test for healthy people without symptoms. Your doctor will decide whether it is appropriate based on your symptoms, medical history and physical examination.
What Conditions Can Cause a Positive Rheumatoid Factor Test?
A positive RF result may be seen in:
- Rheumatoid arthritis (RA): The RF test is most commonly used along with other tests when this condition is suspected. A chronic autoimmune condition in which the immune system attacks the joints, causing pain, swelling, and stiffness.
Other conditions where RF may be detected:
- Sjögren’s syndrome: An autoimmune condition that mainly affects the glands that produce tears and saliva, often causing dry eyes and dry mouth.
- Systemic lupus erythematosus (SLE): An autoimmune condition that can cause inflammation in different parts of the body, including the joints, skin, kidneys, and other organs.
- Scleroderma (systemic sclerosis): An autoimmune condition that can cause hardening and tightening of the skin and may affect internal organs.
- Mixed connective tissue disease: A rare autoimmune condition with features that may overlap with conditions such as lupus, scleroderma, and polymyositis.
- Juvenile idiopathic arthritis (JIA): A group of inflammatory arthritis conditions that affect children and can cause persistent joint pain, swelling, and stiffness.
- Chronic infections: Conditions such as hepatitis C, tuberculosis, and infective endocarditis can sometimes cause elevated RF.
- Sarcoidosis: An inflammatory condition in which clusters of immune cells form in organs, commonly affecting the lungs and lymph nodes.
- Certain cancers, including leukaemia: Some cancers, including leukaemia and multiple myeloma, can be associated with elevated RF. However, the RF test cannot be used to diagnose cancer or identify its type.
- Healthy individuals: A positive RF can occasionally be found in people who do not have an underlying disease, particularly with increasing age.
Note: A positive RF result does not confirm any of these conditions. Doctors interpret it along with symptoms, examination findings and, when needed, other tests.
Rheumatoid Factor Test Preparation
What to Expect Before the Rheumatoid Factor Test?
- You may eat and drink normally before the rheumatoid factor test.
- The test can be performed at any time of the day.
- The rheumatoid factor test results may be affected by medications, supplements, and illnesses. Inform your doctor regarding these.
What to Expect During the Blood Collection?
- A trained phlebotomist from PharmEasy will arrive at your home at the scheduled time.
- They will take a blood sample from a vein in your arm using a small needle.
- After the needle is inserted, a small amount of blood will be collected into a test tube or vial.
- You may feel a little sting when the needle goes in or out. This usually takes less than five minutes.
What to Expect After the Rheumatoid Factor Test?
- Mild throbbing may be felt at the blood collection site after the blood is drawn, which will get better in a few minutes.
- Occasionally, you may feel dizzy, in which case you may lie down for a while until the feeling passes.
Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.
What Are the Parameters, Ranges and Interpretation of the Rheumatoid Factor Test?
The rheumatoid factor test measures the level of rheumatoid factor (RF), an autoantibody, in your blood.
|
Parameter |
Biological reference value |
Interpretation |
|---|---|---|
|
Rheumatoid factor (RF) |
Adults: ≤18 IU/mL |
Within the reference range: RF is not elevated. |
|
Above the reference range: RF is elevated and may be seen in rheumatoid arthritis and some other autoimmune or inflammatory conditions. However, an elevated RF alone does not confirm rheumatoid arthritis. |
Note: Rheumatoid factor is an autoantibody that may be produced by the immune system. Elevated RF levels can be associated with rheumatoid arthritis and other autoimmune, inflammatory or infectious conditions. However, RF may also be present in some healthy individuals, so an elevated result alone cannot confirm rheumatoid arthritis. Your doctor will interpret the result along with your symptoms, medical history and other test findings.
Rheumatoid Factor Test Risks and Limitations
Potential Risks and Complications
- The rheumatoid factor test is generally safe and involves minimal risk.
- Mild discomfort, slight bleeding, or bruising at the blood collection site, dizziness, or light-headedness may occasionally occur.
- In rare cases, there may be difficulty locating a vein or a small risk of infection.
Understanding Rheumatoid Factor Test Limitations
- RF is present not just in rheumatoid arthritis; it may also be detected in other autoimmune diseases, infections, certain cancers, or even in some healthy individuals.
- This test cannot rule out RA completely because sometimes people with RA, especially in the early stages, may have undetectable RF levels.
- The RF test results cannot be interpreted alone, it usually is assessed with presenting symptoms, physical examination, and other tests, such as anti-CCP, ESR, or CRP.
- The severity of disease cannot be determined by this test
- Reference ranges can vary between laboratories; therefore, your results should always be interpreted as per the reference ranges mentioned in your report.
Depending on your symptoms and examination, your doctor may recommend other tests to help understand the cause of your joint symptoms. These may include:
- Anti-CCP antibody is often used along with RF because it is more specific for rheumatoid arthritis.
- Complete blood count (CBC)
- Erythrocyte sedimentation rate (ESR)
- C-Reactive protein test (CRP)
- Imaging tests such as an X-ray, ultrasound, or MRI
