Know More About The Test
A quick info on RA Factor test
Overview
The rheumatoid factor test is a blood test used to measure the amount of rheumatoid factor (RF), an autoantibody made by the immune system. Normally, the immune system protects the body by recognising and fighting harmful substances such as bacteria and viruses. In some situations, it may mistakenly target the body’s own healthy cells or tissues. This abnormal immune response is called autoimmunity, and the antibodies produced against the body’s own tissues are known as autoantibodies. RF is one such autoantibody and is commonly associated with rheumatoid arthritis (RA), a long-term autoimmune condition that mainly affects the joints and may cause pain, swelling, and stiffness. RA can occur at any age but commonly develops during adulthood, particularly in middle age. An increased RF level may also occur with certain autoimmune, inflammatory, or infectious conditions and, sometimes, in people without an underlying disease. Doctors may recommend the rheumatoid factor test when RA or another condition associated with rheumatoid factor is suspected. The result is assessed together with your symptoms, medical history, and physical examination. Based on your clinical findings, your doctor may also advise other blood tests or imaging tests, such as an X-ray, to evaluate your joints.
When and Who Should Get a Rheumatoid Factor Test?
The rheumatoid factor test may be recommended for people who have symptoms that could suggest rheumatoid arthritis or another condition associated with rheumatoid factor. A doctor may consider the test when a person has:
- Persistent joint pain, swelling, or tenderness, especially in multiple joints.
- Joint stiffness that is worse in the morning or after periods of inactivity.
- Symptoms affecting both sides of the body, such as both hands, wrists, or feet.
- Hard bumps (rheumatoid nodules) under the skin near the joints.
- Unexplained symptoms such as prolonged fatigue, fever, or weakness along with joint symptoms.
- Signs or symptoms that raise suspicion of another autoimmune, inflammatory, or infectious condition associated with rheumatoid factor.
Note: This test is not generally recommended as a routine screening test for healthy people without symptoms. Your doctor will decide whether it is appropriate based on your symptoms, medical history and physical examination.
What Conditions Can Cause a Positive Rheumatoid Factor Test?
A positive RF result may be seen in:
- Rheumatoid arthritis (RA): The RF test is most commonly used along with other tests when this condition is suspected. A chronic autoimmune condition in which the immune system attacks the joints, causing pain, swelling, and stiffness.
Other conditions where RF may be detected:
- Sjögren’s syndrome: An autoimmune condition that mainly affects the glands that produce tears and saliva, often causing dry eyes and dry mouth.
- Systemic lupus erythematosus (SLE): An autoimmune condition that can cause inflammation in different parts of the body, including the joints, skin, kidneys, and other organs.
- Scleroderma (systemic sclerosis): An autoimmune condition that can cause hardening and tightening of the skin and may affect internal organs.
- Mixed connective tissue disease: A rare autoimmune condition with features that may overlap with conditions such as lupus, scleroderma, and polymyositis.
- Juvenile idiopathic arthritis (JIA): A group of inflammatory arthritis conditions that affect children and can cause persistent joint pain, swelling, and stiffness.
- Chronic infections: Conditions such as hepatitis C, tuberculosis, and infective endocarditis can sometimes cause elevated RF.
- Sarcoidosis: An inflammatory condition in which clusters of immune cells form in organs, commonly affecting the lungs and lymph nodes.
- Certain cancers, including leukaemia: Some cancers, including leukaemia, and multiple myeloma, can be associated with elevated RF. However, the RF test cannot be used to diagnose cancer or identify its type.
- Healthy individuals: A positive RF can occasionally be found in people who do not have an underlying disease, particularly with increasing age.
Note: A positive RF result does not confirm any of these conditions. Doctors interpret it along with symptoms, examination findings and, when needed, other tests.
Rheumatoid Factor Test Preparation
What to Expect Before the Rheumatoid Factor Test?
- Fasting is not necessary for this test, and it may be done at any time of the day.
- Your doctor should be informed about all the medicines and supplements you take, and whether you have an already diagnosed autoimmune disorder or other conditions, as these may affect the rheumatoid factor test results.
What to Expect During the Blood Collection?
- A phlebotomist will arrive at your home for the blood sample collection.
- An elastic band will be placed around your arm, and you may be asked to make a fist. Using a sterile needle, a small blood sample will be drawn from a vein in your arm. You may be advised to apply mild pressure at the blood draw site using a cotton swab.
- The procedure usually takes only a few minutes.
What to Expect After the Rheumatoid Factor Test?
- Mild throbbing at the site may be felt after the blood is drawn.
- If you feel faint, you may lie down for a while.
- The sample will be taken to the lab to be analysed. Your test results should be available within a few hours, which you can access from the PharmEasy online platform.
Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.
What Are the Parameters, Ranges and Interpretation of the Rheumatoid Factor Test?
The rheumatoid factor test measures the level of rheumatoid factor (RF), an autoantibody, in your blood.
|
Parameter |
Biological reference value |
Interpretation |
|---|---|---|
|
Rheumatoid factor (RF) |
Adults: ≤18 IU/mL |
Within the reference range: RF is not elevated. |
|
Above the reference range: RF is elevated and may be seen in rheumatoid arthritis and some other autoimmune or inflammatory conditions. However, an elevated RF alone does not confirm rheumatoid arthritis. |
Note: Rheumatoid factor is an autoantibody that may be produced by the immune system. Elevated RF levels can be associated with rheumatoid arthritis and other autoimmune, inflammatory or infectious conditions. However, RF may also be present in some healthy individuals, so an elevated result alone cannot confirm rheumatoid arthritis. Your doctor will interpret the result along with your symptoms, medical history and other test findings.
Rheumatoid Factor Test Risks and Limitations
Potential Risks and Complications
- The rheumatoid factor test is generally safe.
- Minor risks involved with a blood draw may occasionally occur, such as mild pain, slight bleeding, bruising, dizziness, or light-headedness. Sometimes, vein access may be difficult.
Understanding Rheumatoid Factor Test Limitations
- A raised RF level does not necessarily mean rheumatoid arthritis: RF may also be present with other autoimmune diseases, infections, certain cancers, or even in some healthy individuals.
- A normal RF result cannot completely exclude rheumatoid arthritis: Some people with RA, especially in the early stages, may have normal or undetectable RF levels.
- RF is not sufficient to diagnose RA: Your doctor will assess the result together with your symptoms, physical examination, and other investigations, such as anti-CCP, ESR, or CRP.
- RF levels do not directly indicate disease severity: Although higher levels may be associated with RA and, in some cases, more severe disease, the RF value alone cannot show how active or advanced the condition is.
- Reference ranges can differ between laboratories: Your RF result should be interpreted according to the reference range provided by the laboratory that performed the test.
Depending on your symptoms and examination, your doctor may recommend other tests to help understand the cause of your joint symptoms. These may include:
- Anti-CCP antibody is often used along with RF because it is more specific for rheumatoid arthritis.
- Complete blood count (CBC)
- Erythrocyte sedimentation rate (ESR)
- C-Reactive protein test (CRP)
- Imaging tests such as an X-ray, ultrasound, or MRI
