Overview

The rheumatoid factor test checks the amount of rheumatoid factor (RF) in your blood. RF is an autoantibody made by the immune system and is commonly associated with rheumatoid arthritis (RA), a long-term autoimmune condition that primarily affects the joints and may lead to pain, swelling, and stiffness. Antibodies normally help defend the body against foreign substances such as bacteria and viruses. In autoimmune conditions, the immune system can mistakenly react against the body’s own components and produce autoantibodies. RF is one such autoantibody that attaches to a portion of another antibody called immunoglobulin G (IgG), and this interaction may contribute to inflammation, including inflammation affecting the joints. RA can occur at any age, although it most often develops during middle age. In India, rheumatoid arthritis is estimated to affect around 0.28%–0.7% of the population and is considerably more common among women, with prevalence estimated to be nearly three times higher in women than in men. RF may also be present in people with other autoimmune or inflammatory conditions, certain infections, or cancers, and occasionally in healthy individuals. Doctors consider the result together with your symptoms, medical history, and physical examination and may recommend additional tests, such as anti-CCP, ESR, or CRP, or imaging such as an X-ray, based on your clinical findings. This test does not require fasting and can be done at any time of day.

The rheumatoid factor test may be recommended for people who have symptoms that could suggest rheumatoid arthritis or another condition associated with rheumatoid factor. A doctor may consider the test when a person has:

  • Persistent joint pain, swelling, or tenderness, especially in multiple joints.
  • Joint stiffness that is worse in the morning or after periods of inactivity.
  • Symptoms affecting both sides of the body, such as both hands, wrists, or feet.
  • Hard bumps (rheumatoid nodules) under the skin near the joints.
  • Unexplained symptoms such as prolonged fatigue, fever, or weakness along with joint symptoms.
  • Signs or symptoms that raise suspicion of another autoimmune, inflammatory, or infectious condition associated with rheumatoid factor.

Note: This test is not generally recommended as a routine screening test for healthy people without symptoms. Your doctor will decide whether it is appropriate based on your symptoms, medical history and physical examination.

  • Rheumatoid arthritis (RA): The RF test is most commonly used along with other tests when this condition is suspected. A chronic autoimmune condition in which the immune system attacks the joints, causing pain, swelling, and stiffness.

Other conditions where RF may be detected:

  • Sjögren’s syndrome: An autoimmune condition that mainly affects the glands that produce tears and saliva, often causing dry eyes and dry mouth.
  • Systemic lupus erythematosus (SLE): An autoimmune condition that can cause inflammation in different parts of the body, including the joints, skin, kidneys, and other organs.
  • Scleroderma (systemic sclerosis): An autoimmune condition that can cause hardening and tightening of the skin and may affect internal organs.
  • Mixed connective tissue disease: A rare autoimmune condition with features that may overlap with conditions such as lupus, scleroderma, and polymyositis.
  • Juvenile idiopathic arthritis (JIA): A group of inflammatory arthritis conditions that affect children and can cause persistent joint pain, swelling, and stiffness.
  • Chronic infections: Conditions such as hepatitis C, tuberculosis, and infective endocarditis can sometimes cause elevated RF.
  • Sarcoidosis: An inflammatory condition in which clusters of immune cells form in organs, commonly affecting the lungs and lymph nodes.
  • Certain cancers, including leukaemia: Some cancers, including leukaemia, and multiple myeloma, can be associated with elevated RF. However, the RF test cannot be used to diagnose cancer or identify its type.
  • Healthy individuals: A positive RF can occasionally be found in people who do not have an underlying disease, particularly with increasing age.

Note: A positive RF result does not confirm any of these conditions. Doctors interpret it along with symptoms, examination findings and when needed, other tests.

  • The RF test requires no special preparation, and fasting is not necessary beforehand.
  • It is helpful to let the doctor know about any current medications, supplements, medical conditions, and previously diagnosed autoimmune disorders, as these details can influence how results are interpreted.
  • The RF test can be done at any time of day.
  • Choosing clothing that allows easy access to the arm can also make the blood draw more convenient.

What to Expect During the Blood Collection

  • The arm is first cleaned with an antiseptic swab to minimise infection risk. A tourniquet band is tied above the elbow, making the vein easier to locate and access.
  • Using a small needle, the phlebotomist draws a blood sample from a vein, typically in the arm. The entire process takes just a few minutes and involves brief, mild discomfort comparable to a pinprick.
  • Once the sample is collected, the needle is withdrawn, and gentle pressure is applied to the site. A small bandage is then placed to support clotting and prevent bleeding. Patients are usually asked to keep the arm still for a few seconds during the draw to ensure a smooth, uninterrupted collection.

What to Expect After the Rheumatoid Factor Test

  • There are no restrictions following the procedure, so normal activities can be resumed right away. Mild soreness or slight bruising at the puncture site is common but generally fades within a day.

  • As a simple blood test, the RF test carries minimal risk overall.
  • At the puncture site, mild effects such as brief stinging during the needle prick, slight bruising, or short-lived tenderness are fairly common and usually fade within a day or two.
  • Less frequently, some patients may experience excessive bleeding, particularly if they are on blood-thinning medication, along with fainting, light-headedness, or infection at the site. Occasionally, a haematoma, a raised bruise caused by blood collecting under the skin, may develop as well.
  • Most of these effects are minor and settle on their own without treatment. Anyone with a history of fainting or a bleeding disorder should mention this to the phlebotomist before the blood draw.

Understanding Rheumatoid Factor Test Limitations

  • RF can be positive in other autoimmune conditions, chronic infections, certain cancers, people who use tobacco, and occasionally in healthy individuals, particularly older adults. Hence, it is not specific to RA and cannot be used alone for confirmation of diagnosis.
  • Some people with RA have persistently normal RF levels, while RF may also be undetectable during the early stages of the disease.
  • RF levels may remain increased even when RA is well controlled, so the RF test is not reliable for measuring treatment response or day-to-day inflammation.
  • The RF level cannot be used alone to assess how advanced or active RA is.
  • Doctors may consider other tests such as anti-CCP, CRP, and ESR, along with physical examination and, when needed, imaging such as ultrasound or MRI in conjunction with the RF test.
