Overview

Prostate-specific antigen (PSA) is a glycoprotein enzyme secreted by the cells of the prostate gland, with a small amount normally released into the bloodstream. The levels of PSA present in the blood can be measured using the PSA test. It is the primary test when there is a suspicion of prostate cancer. Its use as a serum marker has revolutionised prostate cancer screening. 

However, PSA is specific to the prostate gland but is not specific to cancer. This means PSA levels may also be elevated in non-cancerous prostatic conditions such as prostate enlargement, prostatitis, and other non-malignant conditions. 

If your PSA results are abnormal, your doctor may advise a repeat PSA test, digital rectal exam, prostate MRI, or biopsy to support the diagnosis. For guidance on whether you should undergo PSA testing, consult your healthcare provider before considering testing.

It is also important to note that, in rare cases, some men may have prostate cancer even with low serum PSA levels.

  • For men carrying BRCA2 mutations, the corresponding age is 40 years
  • Men with symptoms such as difficulty and increased frequency of urination, erectile dysfunction, urine retention, pelvic pain, blood in sperm or urine, and bony pain.

The frequency of undergoing the PSA test depends on the individual’s age and risk factors and must be decided after a discussion with your doctor.

  • Age 40-45 years: A single PSA test to establish a baseline PSA level.
  • High-risk men aged 45 years and older: Annual PSA testing.
  • Men aged 50 years and older: PSA testing every 2-3 years, depending on individual risk factors and previous results.
  • Men aged 75 years and older or those with a life expectancy of less than 10 years: Routine PSA screening is generally not recommended.
  • Abnormal PSA results: Repeat testing and follow-up monitoring, typically every 6-12 months, may be advised.

Avoid activities that may temporarily increase PSA levels for at least 48 hours before the blood sample is collected:

  • Sexual activity, including ejaculation.
  • Vigorous physical activity, especially cycling or motorcycle riding, can place pressure on the prostate gland.
  • Avoid prostate manipulation, such as prostate massage or a digital rectal examination (DRE), immediately before the PSA test.

What to Expect Before the Test

  • The PSA test can be performed at any time of the day and does not require fasting.
  • Inform your healthcare provider if you are taking any medications.
  • Also inform them regarding recent urinary tract procedures, including catheterisation, cystoscopy, or a prostate biopsy within the previous six weeks.
  • In case of recent or ongoing urinary tract infections, your healthcare provider may recommend postponing the test.

What to Expect During the Sample Collection

  • A trained phlebotomist will arrive at your home at the scheduled time.
  • A small blood sample will be collected from a vein in your arm using a sterile needle after the skin is cleaned.
  • The procedure usually takes only a few minutes and may cause mild discomfort or a slight pinching sensation.

What to Expect After the Test

  • The blood sample is sent to the laboratory for analysis, and results are usually made available within a few hours, and can be downloaded from the PharmEasy platform. 
  • Depending on the findings, additional tests may be recommended for further evaluation.
