Know More About The Test
A quick info on PSA test
Overview
Prostate-specific antigen, or PSA, is a protein produced by both normal and cancerous cells of the prostate gland. The prostate gland is a walnut-sized gland below the bladder in men, and it aids in the production of semen. PSA levels in the blood may rise due to prostate cancer as well as non-cancerous conditions such as benign prostatic hyperplasia and prostatitis.
The PSA test measures the level of PSA in the blood. It is used in many situations, such as:
- For routine screening for prostate cancer in men who do not have symptoms of the disease.
- To monitor the progression of prostate cancer in men who have already been diagnosed with the disease.
- To monitor and follow up on prostate symptoms, such as painful or frequent urination, blood in urine or semen, and pelvic and/or back pain.
- To evaluate treatment effectiveness in men being treated for prostate cancer.
When and Who Should Get a PSA Test Done?
PSA testing may be considered for men in the following situations:
- Age: typically from the age of 50 years.
- High risk: start testing at 40 if your father or brother had prostate cancer or you carry a BRCA1/BRCA2 mutation.
- Urinary problems: difficulty urinating, increased frequency, or sudden urgency.
- Hematuria: visible blood in the urine.
- Pain: pelvic or lower-back pain.
- Sexual dysfunction: trouble achieving or maintaining an erection.
- Known prostate cancer: to monitor disease courses.
- During treatment: to assess treatment response.
How Frequently Should You Take the PSA Test?
PSA testing frequency: general guidance
- Decide on the testing schedule with your doctor based on individual risk.
- Average risk: consider screening from 55 to 69 years, every 2 to 3 years.
- High risk: begin annual screening between 40 to 54 years.
- Elevated or rapidly rising PSA: repeat testing every 6 months or more often as needed.
- Stop routine screening if life expectancy is under 10 years or age is over 75.
What Conditions Can a PSA Test Help Detect?
The PSA test aids in screening for prostate cancer, but raised PSA can occur with other non‑cancerous conditions:
- Benign prostate enlargement
- Prostatitis (prostate inflammation or pain)
- Urinary tract infection
- Certain medications
A raised PSA often requires further testing to confirm prostate cancer or identify the cause.
Disclaimer: This information is educational only and not a diagnosis. Consult your physician for interpretation of test results.
PSA Test Preparation
Fasting is not required for this test, and it can be done at any time of the day.
Refrain from the following for 48 hours before the test:
- Sexual activity, including ejaculation, can temporarily increase PSA levels because releasing semen can increase PSA levels.
- Any vigorous exercise, especially cycling.
What to Expect Before the Test
Book your slot on the Pharmeasy app or website according to your convenient time.
Inform your healthcare provider if you:
- Take medications for prostate enlargement or hair loss, such as finasteride or dutasteride.
- Have recently undergone prostate surgery, a prostate biopsy, urinary catheterisation, or other urinary tract procedures.
- Had a urinary tract infection within the past three months.
- Have a liver condition such as cirrhosis, hepatitis, or cystic fibrosis.
What to Expect During the Sample Collection
- This test involves a blood draw for a sample. A phlebotomist or other trained professional will insert an intravenous (IV) needle into a vein (usually a vein in your arm) and fill up one or more test tubes with blood. If you already have an IV line inserted into a vein, a provider may use that line to collect the blood sample.
What to Expect After the Test
- Once the sample is analysed in the laboratory, the test reports will be uploaded to the PharmEasy platform after review by a medical practitioner. The report can be downloaded to your device from the comfort of your home.
Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.
What are the Parameters Included in a PSA Test?
The PSA test measures a single key parameter:
- Total PSA (prostate specific antigen) - measured in ng/mL (nanograms per millilitre).
Normal Range of PSA Test Parameters
PSA normal values are age-dependent:
|
Age group |
Normal PSA range |
Clinical interpretation |
|---|---|---|
|
40 - 49 years |
0 - 2.5 ng/mL |
PSA within this range is reassuring for this age group. |
|
50 - 59 years |
0 - 3.5 ng/mL |
PSA within this range is reassuring. Above 3.5 warrants further evaluation. |
|
60 - 69 years |
0 - 4.5 ng/mL |
PSA within this range is acceptable. Above 4.5 requires urological evaluation. |
|
Above 70 years |
0 - 6.5 ng/mL |
Slightly higher PSA is expected with age. Above 6.5 warrants clinical evaluation. |
Important note: Normal PSA ranges may vary between laboratories and between Indian and Western reference populations. Studies show PSA ranges tend to be lower in Indians.
PSA Test Result Interpretation
The PSA result must be interpreted in the context of your age, symptoms, digital rectal examination findings, and the free/total PSA ratio. A single elevated PSA reading is not sufficient for diagnosis.
|
PSA level |
Risk category |
Potential interpretation and action |
|---|---|---|
|
Below 4 ng/mL |
Low risk |
Generally reassuring. Routine monitoring as per age-specific schedule. |
|
4 - 10 ng/mL |
Borderline/moderate risk |
Grey zone. Prostate cancer is possible, but so are BPH and prostatitis. Free PSA ratio and urological evaluation recommended. |
|
Above 10 ng/mL |
High risk |
Significantly elevated. High suspicion of prostate cancer. Urological referral and prostate biopsy are typically recommended. |
|
Rapidly rising PSA (>0.75 ng/mL/year) |
PSA velocity concern |
Even if the absolute value is below 4, a rapid annual rise warrants urgent evaluation. |
|
Low free PSA/Total PSA Ratio (<15%) |
Higher cancer risk |
A lower free-to-total PSA ratio suggests a higher likelihood of prostate cancer over benign conditions. |
|
Post-treatment - rising PSA |
Biochemical recurrence |
A rising PSA after prostatectomy (above 0.2 ng/mL) or after radiation suggests cancer recurrence. |
Disclaimer: This table is for educational purposes only. PSA interpretation requires the expertise of a qualified urologist. A high PSA does not confirm prostate cancer; false positives are common. Never make treatment decisions based on PSA alone.
PSA Test Risks and Limitations
Potential Risks and Complications
- Minor risks associated with a blood draw may occasionally occur, such as mild pain, slight bleeding, bruising, dizziness, or light-headedness. Rarely, vein access may be difficult.
- Medical attention should be sought if persistent bleeding, swelling, redness, or severe pain develops.
Understanding Prostate Specific Antigen Test Limitations
- Some cancers detected through PSA screening grow so slowly that they would never cause symptoms or become life-threatening. However, treatment may sometimes lead to side effects, including urinary problems, bowel dysfunction, and difficulties with sexual function.
- The PSA test may give false-positive results. This occurs when the PSA level is elevated, but no cancer is present. This may create anxiety and lead to additional medical procedures, such as a prostate biopsy or digital rectal examination.
- Not all prostate cancers produce high PSA levels, which means some cancers may not be detected through PSA testing alone.
PSA Test Price in Delhi
In Delhi, the standard cost for a PSA test at PharmEasy typically starts from INR 489. However, the price may vary due to different factors such as location and the diagnostic provider. Feel free to compare PSA test prices across major cities in India for better clarity.
