Overview

Doctors use the thyroid-stimulating hormone (TSH) test, also called the thyrotropin test, to check how well your thyroid gland is working. The thyroid is a small, butterfly-shaped gland in the front of your neck that makes two hormones: thyroxine (T4) and triiodothyronine (T3). Thyroid hormones control how the body uses energy, so they affect nearly every organ in your body, even your heart. TSH is a hormone made in the pituitary gland that tells the thyroid how much T4 and T3 to make. Because TSH levels change even with small changes in thyroid hormone levels, it is usually the first test used to assess thyroid function. A high TSH level most often means you have hypothyroidism, or an underactive thyroid. This means that your thyroid is not making enough hormones. As a result, the pituitary keeps making and releasing TSH into your blood. A low TSH level usually means you have hyperthyroidism, or an overactive thyroid. This means that your thyroid is making too much hormone, so the pituitary stops making and releasing TSH into your blood. Your doctor may recommend a TSH test if you have symptoms of a thyroid disorder, are pregnant, have a family history of thyroid disease, are taking thyroid medication, or have certain autoimmune conditions. If the TSH result is abnormal, additional tests such as free T3 and free T4 may be needed.

  • Symptoms of thyroid dysfunction: unexplained increase or loss of weight, prolonged fatigue, hair loss, intolerance to cold or heat, changes in heart rate, anxiety, depression, or other mood changes.
  • During pregnancy: healthy thyroid hormone levels are important for both the mother’s health and the baby’s growth and development.
  • Fertility or menstrual concerns: thyroid disorders can affect reproductive health.
  • Family history of thyroid disease: regular screening may be advised.
  • Monitoring thyroid treatment: to check how well thyroid medicines are working and whether the dose needs adjustment.
  • Routine newborn screening: to detect congenital hypothyroidism early and allow timely treatment.
  • Routine screening in certain medical conditions: autoimmune diseases, such as type 1 diabetes or other autoimmune disorders, may increase the risk of thyroid disease.

What to Expect Before the TSH Test

  • No fasting or dietary restrictions are required for this test, so you can continue eating and drinking as usual before sample collection. 
  • However, inform your doctor about the following factors as they may sometimes influence the TSH results and interpretation.
    • Any medicines you take (especially thyroid medicines, metformin, amiodarone, steroids, and PPIs).
    • Any supplements you use: biotin (vitamin B7) should be stopped at least 72 hours before the test. Also mention iron, calcium, iodine, and soy supplements.
    • If you are pregnant or planning a pregnancy. TSH is interpreted differently during pregnancy, and regular monitoring is required.
  • The TSH test can be performed at any time of the day. However, because TSH levels naturally fluctuate throughout the day, it is recommended that repeat tests be performed at approximately the same time to allow more consistent comparison of results.

What to Expect During the Blood Collection

  • A trained phlebotomist will arrive at your home at the scheduled appointment time for sample collection.
  • You will be asked to sit comfortably while the phlebotomist identifies a suitable vein, typically on the inner side of your arm near the elbow.
  • The selected area will be cleaned and disinfected.
  • A sterile needle will then be gently inserted into the vein to collect the blood sample. You may feel a brief pinching sensation.
  • A small amount of blood will be collected into a sample tube.
  • Once the required sample has been collected, the needle will be removed, and gentle pressure will be applied using cotton or gauze to help stop any bleeding.
  • A small bandage will be placed over the collection site after the procedure is completed.
  • The blood collection process is quick and generally takes only a few minutes.

  • The sample will then be sent to a lab to be analysed. Your test results should be available within a few hours.

Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.
