The thyroid-stimulating hormone (TSH) test, also known as the thyrotropin test, is a blood test used to assess how well your thyroid gland is functioning. TSH is produced by the pituitary gland in the brain and controls the release of the thyroid hormones thyroxine (T4) and triiodothyronine (T3). These hormones regulate many important body functions, including metabolism, energy production, heart function, body temperature, and growth. TSH and thyroid hormones work in a feedback system: when T3 and T4 levels fall, the pituitary releases more TSH to stimulate the thyroid, while higher thyroid hormone levels reduce TSH production. Because even small changes in thyroid hormone levels can cause noticeable changes in TSH, the TSH test is considered one of the most sensitive and reliable ways to screen for thyroid disorders. It is commonly used to detect hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid), as well as to monitor treatment in people with thyroid disease. Although the TSH test is usually the first step in evaluating thyroid function, your doctor may recommend additional tests, such as Free T3, Free T4, or thyroid antibody tests, for a more complete assessment, especially in complex cases.
When and Who Should Get a TSH Test Done?
A TSH test may be recommended if you have symptoms of a thyroid disorder or if you are at increased risk of developing one. Your doctor may advise the test in the following situations:
- Symptoms of thyroid dysfunction: Such as unexplained weight gain or weight loss, ongoing tiredness, hair loss, sensitivity to cold or heat, changes in heart rate, anxiety, depression, or other mood changes.
- During pregnancy: To evaluate thyroid function, as healthy thyroid hormone levels are important for both the mother’s health and the baby’s growth and development.

What Conditions Can a TSH Test Help Detect?

- Hyperthyroidism: An overactive thyroid producing excess hormone, which suppresses TSH production.

TSH Test Preparation
What to Expect Before the TSH Test
- You may eat and drink normally before the TSH test.
- The TSH test can be performed at any time of the day. However, because TSH levels naturally fluctuate throughout the day (circadian rhythm), every time you take the test, it is best to have the sample collection performed at approximately the same time of day to ensure more consistent and reliable comparison of results.
- The TSH test results may be affected by medications, supplements, autoimmune disorders, and pregnancy. Inform your doctor regarding these.
- Thyroid medicines, metformin, amiodarone, steroids, proton pump inhibitors (PPIs), biotin, iron, calcium, iodine, and soy supplements can all affect TSH levels.

What to Expect During the Blood Collection
- A trained phlebotomist from PharmEasy will arrive at your home at the scheduled time.

- After the needle is inserted, a small amount of blood will be collected into a test tube or vial.
- You may feel a little sting when the needle goes in or out. This usually takes less than five minutes.
What to Expect After the TSH Test
- Mild throbbing may be felt at the blood collection site after the blood is drawn, which will get better in a few minutes.
- Occasionally, you may feel dizzy, in which case you may lie down for a while until the feeling passes.
