Overview
The thyroid-stimulating hormone (TSH) test, also known as the thyrotropin test, is a blood test used to assess the health and function of the thyroid gland. TSH is produced by the pituitary gland in the brain and regulates the production of the thyroid hormones T3 and T4. These hormones influence several vital body functions, including metabolism, body temperature, heart rate, digestion, energy levels, growth, and mood. When thyroid hormone levels fall, the pituitary releases more TSH to stimulate the thyroid gland. When thyroid hormone levels are high, TSH production decreases. Measuring TSH helps doctors determine whether the thyroid is producing too little or too much thyroid hormone. The test is widely used to detect and monitor hypothyroidism (underactive thyroid) and hyperthyroidism (overactive thyroid). It is also commonly performed to monitor people receiving thyroid hormone replacement therapy or treatment for an overactive thyroid. Although TSH is the preferred first-line test for evaluating thyroid function, it does not identify the exact cause of a thyroid disorder. If the result is abnormal, your doctor may recommend further investigations, such as Free T4, Free T3, or thyroid antibody tests, to confirm the diagnosis and guide treatment.
When and Who Should Get a TSH Test Done?
Your doctor may advise a TSH test in the following situations:
- Individuals with symptoms of an underactive thyroid (hypothyroidism), such as tiredness, weight gain, constipation, dry skin, feeling unusually cold, low mood, heavy or irregular menstrual periods or a slow heartbeat.
- Individuals with symptoms of an overactive thyroid (hyperthyroidism), including unexplained weight loss, a fast or irregular heartbeat, anxiety, trembling hands, excessive sweating, heat intolerance, trouble sleeping, or light, infrequent, or missed menstrual periods.
- Individuals with swelling in the front of the neck.
- A patient who has been diagnosed with a goitre or another suspected thyroid condition.
- A person with a personal or family history of thyroid disease or autoimmune disorders, which can increase the risk of thyroid problems.
- Pregnant women or women planning pregnancy, as normal thyroid function is crucial for a healthy pregnancy.
- Monitoring a patient receiving treatment for a thyroid disorder to monitor the condition and assess whether medication needs adjustment.
- Monitoring thyroid function in patients taking medicines that may affect the thyroid.
- Thyroid screening in people who are at increased risk, based on age, medical history, or other underlying health conditions.
What Conditions Can a TSH Test Help Detect?
The TSH test helps doctors assess thyroid function and may aid in detecting or monitoring:
- Hypothyroidism (underactive thyroid): The thyroid does not produce enough hormones.
- Hyperthyroidism (overactive thyroid): The thyroid produces too much hormone.
- Subclinical hypothyroidism or hyperthyroidism: Early thyroid dysfunction where TSH is abnormal but thyroid hormone levels may still be within the normal range.

TSH Test Preparation
What to Expect Before the TSH Test
- You can eat and drink normally before the test.
- The blood sample for this test can be taken at any time of the day. However, try to schedule further TSH tests at approximately the same time at each visit, as TSH levels can vary slightly through the day.
- Let your physician know about all the medicines and supplements you take and if you have recently been ill.
- Stop taking biotin (vitamin B7; found in many skin, hair, and nail supplements) at least 72 hours before the test, as it can interfere with thyroid test results.
- Iron and calcium supplements may affect the absorption of thyroid medicines.
- Certain medicines (such as metformin, amiodarone, steroids, and proton pump inhibitors), iodine, and soy can affect TSH levels or interfere with thyroid hormone medicines, which may affect your test results.
- Serious illnesses and hospitalisation can temporarily alter TSH levels and affect the interpretation of your test results.
What to Expect During the Blood Collection
- This test involves a blood draw for a sample.
- A phlebotomist will insert a sterile (IV) needle into a vein (usually a vein in your arm) and fill up one or more test tubes with blood.
- A slight sting may be felt when the needle is inserted and removed.
What to Expect After the TSH Test
- Mild throbbing at the site may be felt after the blood is drawn, which will pass in a few moments.

Understanding TSH Test Limitations
- The TSH test is not enough to diagnose every thyroid disorder. You may have to undergo additional tests, such as Free T3, Free T4, or thyroid antibody tests, for a clearer diagnosis.
- Several factors, including age, pregnancy, smoking, iodine intake, the time of sample collection, certain medicines and supplements (especially biotin), and some medical conditions affect TSH levels.
- In disorders of the pituitary or the hypothalamus, central hypothyroidism, or severe illnesses, TSH levels may not accurately reflect thyroid function.
- A normal TSH result does mean there is no thyroid disease. Similarly, an abnormal result does not always mean that there is a thyroid disorder. Your doctor will interpret the result along with your symptoms, medical history, and other thyroid tests.
- Repeat testing with TSH test, to check efficiency of treatment should be done only after 6 to 8 weeks of starting thyroid medication. This is because changes in TSH levels are gradual and take a few weeks to stabilise.
