The thyroid-stimulating hormone test, also called the thyrotropin test, is a blood test that assesses how well the thyroid gland is working. Located at the front of the neck, this small, butterfly-shaped gland produces thyroxine (T4) and triiodothyronine (T3), hormones that regulate metabolism and support heart function, body temperature, growth, and the healthy functioning of nearly every organ system.

TSH itself comes from the pituitary gland in the brain, acting as a signal that controls how much T3 and T4 the thyroid releases. Because TSH reacts quickly to shifts in thyroid hormone levels, it is typically the first test doctors advise when screening for thyroid conditions. Elevated TSH often points to an underactive thyroid (hypothyroidism), while low TSH may signal an overactive thyroid (hyperthyroidism).

This test may be recommended if you are experiencing symptoms of thyroid disease, are pregnant, have a family history of thyroid disorders, are undergoing thyroid treatment, or have certain autoimmune conditions. Should the result be abnormal, further tests such as free T3 and free T4 may help identify the underlying cause. No fasting is needed beforehand and the test involves a simple blood draw.

What to Expect Before the TSH Test

  • There are no special preparations required for this test. You may eat and drink normally. 
  • The TSH test results may be affected by medications, supplements, and autoimmune disorders:
    • Inform your doctor about any medicines you take (particularly thyroid medicines, metformin, amiodarone, steroids, and PPIs).
    • Let them know about any supplements you use: biotin (vitamin B7) should be stopped at least 72 hours before the test. Supplements that contain iron, calcium, iodine, and soy can also affect TSH levels.
  • You may let your doctor know your pregnancy status if you are pregnant or planning a pregnancy. TSH levels change during pregnancy and must be interpreted accordingly. Moreover, during pregnancy, the risk of thyroid conditions also increases.
  • The test can be done at any time of the day. However, due to the circadian rhythm of TSH, it fluctuates throughout the day. If repeat testing is advised, it is recommended to undergo blood sample collection at similar times of the day at each follow-up test.

What to Expect During the Blood Collection

  • A trained phlebotomist will arrive at your home at the scheduled time.
  • A small blood sample will be collected from a vein in your arm using a sterile needle after the skin is cleaned with an antiseptic. You may feel a slight pricking sensation.
  • The procedure usually takes only a few minutes.

What to Expect After the TSH Test

  • Mild throbbing may be felt at the blood draw site for a few minutes after the blood is drawn. 
  • Occasionally, you may feel dizzy, in which case you may lie down for a while until the feeling passes.
  • The blood sample is sent to the laboratory for analysis, and results are usually made available within a few hours; they can be downloaded from the PharmEasy platform. 
  • Depending on the findings, additional investigations may sometimes be recommended for further evaluation.
