Overview
The thyroid-stimulating hormone (TSH) test, also called the thyrotropin test, is a blood test that helps evaluate how well your thyroid gland is functioning. TSH is made by the pituitary gland, a small gland located at the base of the brain. It controls the thyroid by signalling it to produce the hormones T3 and T4, which play an essential role in regulating metabolism, body temperature, heart rate, energy levels, digestion, and many other body functions. When the body needs more thyroid hormones, the pituitary releases more TSH. Conversely, when thyroid hormone levels are high, TSH production decreases. By measuring the amount of TSH in the blood, doctors can assess whether the thyroid is overactive or underactive. The TSH test is commonly used to screen for and monitor hypothyroidism and hyperthyroidism, as well as to evaluate the effectiveness of thyroid hormone replacement therapy or treatment for an overactive thyroid. While TSH is usually the first test performed to assess thyroid function, an abnormal result often needs to be followed by Free T3, Free T4, or thyroid antibody tests to identify the underlying cause of the thyroid disorder.
When and Who Should Get a TSH Test Done?
Your doctor may advise a TSH test if you:
- Have symptoms of an underactive thyroid (hypothyroidism), such as tiredness, weight gain, constipation, dry skin, feeling unusually cold, low mood, heavy or irregular menstrual periods or a slow heartbeat.
- Have symptoms of an overactive thyroid (hyperthyroidism), including unexplained weight loss, a fast or irregular heartbeat, anxiety, trembling hands, excessive sweating, heat intolerance, trouble sleeping, or light, infrequent, or missed menstrual periods.
- Notice swelling in the front of your neck or have been diagnosed with a goitre or another suspected thyroid condition.
- Have a personal or family history of thyroid disease or autoimmune disorders, which can increase the risk of thyroid problems.
- Are pregnant or planning to become pregnant, especially if you have risk factors for thyroid disease, as normal thyroid function is important for a healthy pregnancy.
- Are receiving treatment for a thyroid disorder, to monitor your condition and determine whether your medication needs adjustment.
- Take medicines that may affect thyroid function, if your doctor recommends monitoring.
- Need thyroid screening because you are at increased risk, based on your age, medical history, or other underlying health conditions.
What Conditions Can a TSH Test Help Detect?
The TSH test helps doctors assess thyroid function and may aid in detecting or monitoring:
- Hypothyroidism (underactive thyroid): The thyroid does not produce enough hormones.
- Hyperthyroidism (overactive thyroid): The thyroid produces too much hormone.
- Subclinical hypothyroidism or hyperthyroidism: Early thyroid dysfunction where TSH is abnormal but thyroid hormone levels may still be within the normal range.
- Goitre (enlarged thyroid gland): To evaluate whether thyroid dysfunction is contributing to thyroid enlargement.
- Thyroid disorders during pregnancy: To monitor thyroid function, as abnormal thyroid hormone levels can affect both the mother and baby.

TSH Test Preparation
What to Expect Before the TSH Test
- No special preparation is usually needed before a TSH test. You can eat and drink normally, as fasting or dietary changes are generally not required.
- The TSH test can be performed at any time of the day. If you need repeat testing, try to have your blood sample collected at approximately the same time each visit to help compare results accurately.
- It is important to let your doctor know about medicines and supplements you take. Certain medicines (such as metformin, amiodarone, steroids, and proton pump inhibitors) and supplements containing biotin, iodine, calcium, iron, or soy can affect TSH levels or interfere with thyroid hormone medicines, which may affect your test results.
- If advised, stop taking biotin (vitamin B7; found in many skin, hair, and nail supplements) at least 72 hours before the test, as it can interfere with thyroid test results.
- Inform your doctor if you take iron or calcium supplements, as they may affect the absorption of thyroid hormone medicines.
- Let your doctor know if you have recently been seriously ill or hospitalised, since certain illnesses can temporarily alter TSH levels and affect the interpretation of your test results.
What to Expect During the Blood Collection
- A trained phlebotomist will visit your home at the scheduled time for sample collection.
- You will be seated comfortably while a suitable vein, usually near the inner elbow, is identified and cleaned with an antiseptic.
- A sterile needle will be used to collect a small blood sample, and you may feel a brief pinching sensation.
- Once the sample is collected, the needle will be removed, gentle pressure will be applied, and a small bandage will be placed over the site.
- The entire procedure is quick and typically takes only a few minutes.
What to Expect After the TSH Test
- You may feel a mild throbbing pain, which will pass in a few minutes. The sample will then be sent to a lab to be analysed. Your test results should be available for download within a few hours on the PharmEasy platform.
