Overview

The SGOT/AST test is a blood test that measures the level of serum glutamic-oxaloacetic transaminase (SGOT), now more commonly known as aspartate aminotransferase (AST). AST is an enzyme found in several tissues, particularly the liver, heart, skeletal muscles, kidneys, and brain. It helps cells process nutrients and produce energy. When these cells are damaged, AST can enter the bloodstream, causing its levels to rise. Doctors may recommend an SGOT/AST test when liver injury is suspected, to evaluate liver health, investigate symptoms, or monitor an existing liver condition and its treatment. Since AST is also present in tissues other than the liver, its levels can be elevated for reasons unrelated to liver disease, including muscle injury, strenuous exercise, heart-related injury, and certain other conditions. Therefore, an abnormal result does not by itself establish a diagnosis of liver disease and is usually interpreted along with other test results, as well as your symptoms and medical history. Liver diseases are an important and growing public health concern in India, with fatty liver disease and alcohol-related liver disease becoming increasingly important alongside viral hepatitis. Hepatitis B remains a major cause of chronic liver disease and liver cancer. Timely testing can help identify liver injury early, allowing the underlying cause to be evaluated and managed before significant liver damage develops.

Your doctor may recommend an SGOT test in the following situations:

  • Routine health check-up: As part of a routine health check-up or to assess overall liver health.
  • Monitoring: To monitor an existing liver condition or response to treatment.
  • Symptoms: If you have symptoms that may suggest liver or other tissue injury, such as nausea, loss of appetite, fatigue, weakness, yellowing of the skin or eyes, abdominal pain or swelling, dark urine, pale stools, or persistent itching.
  • Risk factors: If you have a higher risk of liver disease, such as a family history of liver disease, regular or excessive alcohol use, obesity, diabetes, or hepatitis.
  • Medications: If you take medicines that may affect the liver, your doctor may recommend periodic liver enzyme testing.
  • Hepatitis-related check-ups: If you have a history of hepatitis or possible exposure to hepatitis, testing may be advised as part of your evaluation.

Note: SGOT is not specific to the liver, so your doctor may recommend it along with other liver tests depending on your symptoms and medical history.

  • Heart-related problems: Heart attack or other conditions causing heart muscle injury.
  • Muscle injury: Severe muscle damage, strenuous exercise, or conditions such as rhabdomyolysis.
  • Pancreatitis: Inflammation of the pancreas.
  • Haemochromatosis: A condition in which excess iron builds up in the body and can damage the liver.
  • Mononucleosis: A viral infection that can temporarily raise liver enzymes.
  • Severe burns or seizures: These can cause tissue or muscle injury, leading to increased SGOT.
  • Medicines or substances: Certain medicines and substances that can injure the liver, including excessive alcohol use.
  • Surgery: SGOT may temporarily increase after certain surgical procedures due to tissue injury.

Low levels of SGOT in the blood are usually considered normal. Very rarely, it may be associated with:

  • Vitamin B6 deficiency
  • Certain kidney-related conditions and renal dialysis
  • Pregnancy

What to Expect Before the Test?

  • The sample collection for the SGOT test can be done at any time of day.
  • If your SGOT test is being done along with other blood tests, you may be asked to avoid eating or drinking for a specific period before sample collection. If it is done as a standalone test, fasting is generally not required.
  • Certain medications, over-the-counter drugs, and dietary supplements can influence AST levels. Inform your doctor about all medicines and supplements you are taking, and avoid stopping any prescribed medication unless advised by your healthcare provider. Also, let them know about recent illnesses, previously diagnosed medical conditions, muscle injuries, and pregnancy status.
  • Avoid alcohol consumption for at least 24 hours before the test, as alcohol may temporarily increase AST levels.
  • Intense physical activity or strenuous exercise before testing can sometimes affect AST readings. Inform your doctor if you regularly engage in vigorous workouts and avoid these activities for at least 48 hours before the test. Light exerce is acceptable.

What to Expect During the Sample Collection?

  • A trained phlebotomist will arrive at your home at the scheduled appointment time for sample collection.
  • You will be asked to sit comfortably while the phlebotomist identifies a suitable vein, typically on the inner side of your arm near the elbow.
  • The selected area will be cleaned and disinfected to reduce the risk of infection.
  • A sterile needle will then be gently inserted into the vein to collect the blood sample. You may feel a brief pinching sensation.
  • A small amount of blood will be collected into a sample tube.
  • Once the required sample has been collected, the needle will be removed, and gentle pressure will be applied using cotton or gauze to help stop any bleeding.
  • A small bandage will be placed over the collection site after the procedure is completed.
  • The blood collection process is quick and generally takes only a few minutes.

What to Expect After the Test?

  • The sample will then be taken to the lab to be analysed. Your test results should be available within a few hours.

Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.

Mildly elevated

May occur with mild liver injury, alcohol use, certain medicines, strenuous exercise, or muscle injury. A small increase does not necessarily indicate significant liver disease.

Moderately elevated

May be associated with liver inflammation or injury, such as hepatitis or fatty liver disease. Muscle or heart-related injury can also increase SGOT.

Markedly elevated

Can occur with significant liver or other tissue injury, including severe hepatitis, reduced blood flow to the liver, major muscle injury, or certain heart-related conditions. Further evaluation is needed to determine the cause.

Important: SGOT (AST) is found not only in the liver but also in the heart, skeletal muscles, kidneys, brain, and blood cells. Therefore, an elevated SGOT does not always indicate liver disease. Understanding SGOT Test Limitations

  • SGOT is not specific to the liver and is also present in the heart, muscles, kidneys, and brain. Damage to these tissues can increase SGOT without actual liver injury.
  • A raised SGOT level indicates possible cell or tissue injury but cannot determine the underlying cause, such as hepatitis, alcohol use, medicines, or muscle injury.
  • SGOT may change over time, so a single measurement may not represent the overall pattern of injury.
  • The SGOT level alone cannot determine how severe the liver injury is.
  • Strenuous exercise, physical injury, intramuscular injections, and certain medicines may temporarily raise SGOT levels even without liver disease.
  • SGOT generally has to be assessed along with ALT, ALP, bilirubin, and other clinical information rather than using it as a standalone test.
  • Some people with chronic or mild liver conditions may have SGOT levels within the normal range.
