Overview
A blood clot is a mass of blood that forms when platelets, proteins, and cells in the blood stick together. This is a natural process that usually occurs at a site of injury to help prevent too much blood loss.
Normally, the body dissolves the clot once the injury has healed. D-dimer is a protein fragment that is made when a blood clot dissolves in the body. D-dimer is usually found in undetectable quantities in blood unless the body is making and breaking up blood clots.
With a blood clotting disorder, clots can form in the body even without an injury, or the clots do not dissolve when they should. These disorders can be serious and even life-threatening. A D-dimer test detects increased levels of D-dimer in the blood and helps to determine whether abnormal blood clotting is occurring in the body. It is also used to monitor already diagnosed clotting disorders and their treatment progress.
When and Who Should Get a D-Dimer Test?
The D-dimer test may be recommended in the following situations:
- To help evaluate abnormal blood clotting disorders such as deep vein thrombosis (DVT), pulmonary embolism (PE), and disseminated intravascular coagulation (DIC). Symptoms of DVT may include leg pain, swelling, redness, or warmth. PE can cause shortness of breath, chest pain, cough, rapid breathing, or a fast heartbeat, although some people may have no symptoms.
- To monitor clotting abnormalities following a snake bite.
- To help doctors determine the duration of anticoagulant (blood thinner) therapy after a blood clot has been diagnosed.

How Frequently Should You Take the D-Dimer Test?
- The D-dimer test is not typically performed on a regular schedule. It is usually ordered when there is a clinical suspicion of a blood clotting disorder or to monitor treatment and disease progression in patients with known clotting conditions.
What Conditions Can a D-Dimer Test Help Detect?
A D-dimer test may be used to help evaluate conditions associated with abnormal blood clotting, including:
- Deep vein thrombosis (DVT)
- Pulmonary embolism (PE)
- Disseminated intravascular coagulation (DIC)
- Stroke
However, elevated D-dimer levels do not always indicate a clotting disorder. Higher levels may also be seen in older adults, during pregnancy, after surgery, in smokers, and in individuals with infections, inflammation, liver disease, cancer, or certain autoimmune conditions such as rheumatoid arthritis. Therefore, D-dimer results should always be interpreted alongside clinical findings and other diagnostic tests.

- An elastic band is tied over the upper arm.
- A needle is inserted into the vein of the inner arm to collect the blood.
- A bandage is placed over the blood collection site to stop minor bleeding.
What to Expect After the Test
- A slight pain or a sting may be felt when the needle is inserted. Mild throbbing at the site may also be felt after the blood is drawn.

- D-dimer level: measured in μg/mL FEU or ng/mL (nanograms per millilitre).
- D-dimer is a protein fragment produced when the body breaks down blood clots. Elevated D-dimer levels may indicate that clot formation and clot breakdown are taking place within the body. However, the test only measures the amount of D-dimer in the blood and cannot determine the location, size, or cause of a clot.

Potential Risks and Complications
- There is minimal risk involved with having blood taken for the D-dimer test. Blood vessels may vary in size from one person to another and from one side of the body to the other. Taking blood from some people may be more difficult than from others.
Other risks associated with having blood drawn are slight but may include:
- Fainting or feeling lightheaded
- Multiple punctures to locate veins
- Bleeding
- Infection (a slight risk any time the skin is broken)
Understanding D-Dimer Test Limitations
- Elevated D-dimer levels may occur due to several conditions other than blood clots and even in advancing age. Therefore, a positive result does not confirm a clotting disorder and often requires imaging tests for diagnosis. While a normal D-dimer level may help rule out abnormal clotting in low-risk individuals, it is less reliable in high-risk cases.
- Additionally, the test cannot determine the location or size of a blood clot.
