A widal test is an agglutination test. When antibodies bind (react) to antigens they clump red blood cells together. This clumping of cells is referred to as agglutination.
In the Widal test, typhoid bacteria (Salmonella typhi) are mixed with the blood sample in a lab. If the person has typhoid, the antibodies in the blood will react with the antigen, resulting in agglutination. This will conclude that the person has typhoid fever.
Typhoid fever is a bacterial infection caused by the bacteria salmonella typhi which is related to the bacteria that cause salmonella food poisoning. This bacteria inhibits in animal and human intestines and passes through urine and faeces. The two main ways typhoid can spread are:
- Through contact with an infected person
- By consuming food and water contaminated by salmonella typhi bacteria
Typhoid fever can last 7 to 10 days if treated soon after the symptoms appear. If untreated it may last a month or more, and if complications or a relapse occur it takes a long time to fully recover.
Typhoid infection can spread throughout the body and harm several organs and systems. Typhoid bacteria can travel to the lungs, (pneumonia), the linings of the brain (meningitis), the bone marrow (osteomyelitis), the heart valves (endocarditis), the kidneys (glomerulonephritis), the urinary tracts, and the muscles. Hepatitis (inflammation of the liver) can also occur.

If a person shows symptoms of typhoid fever, the doctor may suggest a widal test to be done for confirmation. The symptoms are:
- High fever with body aches
- Abdominal pain
- Constipation in the first stage, diarrhoea in the later
- Weakness and appetite loss
- Red spots on the body
- Agitation, confusion and in the worst case hallucinations.
