Cytomegalovirus Antibody (CMV) Test - IgM

The cytomegalovirus (CMV) antibody - IgM test is a blood test that checks for immunoglobulin M (IgM) antibodies (proteins made by the immune system in response to an infection). These antibodies may indicate a recent cytomegalovirus (CMV) infection. The test is also called the CMV antibody test.

Human CMV belongs to the Herpesviridae family. Once a person gets infected with this virus, it stays in their body for life. Most of the time, the infection is mild and shows no symptoms. However, in serious cases, it can lead to cytomegalovirus gastroenteritis/colitis (inflammation of the stomach and intestines), cytomegalovirus pneumonia, cytomegalovirus retinitis (inflammation of the retina), and congenital cytomegalovirus infection (a birth defect).

Soon after a viral infection begins, the immune system starts making IgM antibodies to fight the virus; therefore, their presence indicates a recent or current infection. Doctors use this test to quickly find these CMV-related conditions and provide the right treatment.

You might be told to take the CMV antibody test if you are experiencing the following symptoms of CMV infection:

• Fever
• Tiredness
• Feeling unwell or lacking energy
• Rash
• Throat pain
• Swollen lymph nodes (small glands that help the body fight infection)

The following categories of people are also highly advised to take the test:

• Individuals with weakened immune systems – This includes those with HIV/AIDS, or who have undergone bone marrow transplantation, chemotherapy, and an organ transplant.
• Pregnant women – If a woman is pregnant and has a CMV infection, it may lead to a miscarriage or stillbirth.
• Newborns with very low birth weight – They are at high risk of CMV infection during breastfeeding.

The CMV antibody test may be used along with other tests to diagnose certain conditions that are caused by the virus, such as:

• CMV infection – This is a type of infection caused by the herpesvirus. It can be passed from one person to another through body fluids, including blood, saliva, urine, semen, and vaginal fluids.
• CMV gastroenteritis/colitis – This is a condition in which the stomach and intestines are inflamed because of the virus.
• CMV pneumonia – This infection occurs in people who have a weakened immune system.
• CMV retinitis – This is a viral infection characterised by inflammation of the retina of the eye.
• Congenital CMV – This condition occurs when an infant gets infected with the virus before birth.

What to Expect Before the Cytomegalovirus Antibody-IgM Test

No fasting is required for a standalone cytomegalovirus antibody-IgM test. You can eat and drink normally. However, if it is part of a broader package that includes fasting tests, your doctor may advise fasting for 8-12 hours. Avoid strenuous exercise for 24 hours before the test, as intense physical activity can temporarily raise cytomegalovirus antibody-IgM levels and affect interpretation. Always inform your doctor about any ongoing medications.

What to Expect During the Blood Collection

A phlebotomist cleans the skin with an antiseptic, then places an elastic band around your upper arm to make the veins easier to access. A small needle is inserted into the vein; you may feel a brief pinch. Blood is collected into a labelled tube in under a minute.

What to Expect After the Cytomegalovirus Antibody-IgM Test

A small cotton swab and bandage are placed over the site to stop any minor bleeding. You can return to normal activities immediately. Minor bruising is normal and disappears quickly. If dizzy, sit and rest for a few minutes.

The CMV antibody test is a simple blood test. However, it has some risks and limitations that you should be aware of:

Risks:

The blood collection process can cause bleeding, bruising, or minor soreness as side effects. However, most of these resolve quickly.

Limitations:

• The CMV antibody test alone is not enough to confirm a diagnosis; additional tests may be needed.
• Sometimes, CMV shows no symptoms or stays hidden (latent) in people with weakened immune systems. Therefore, it is difficult to diagnose it correctly.

• Cerebrospinal fluid (CSF) analysis
• Sputum test
• Tissue biopsy
• Amniocentesis (for pregnant women)
