Syphilis is a sexually transmitted infection caused by the bacteria Treponema pallidum. When the bacteria enter the body, the immune system produces antibodies to prevent the infection. A VDRL test measures the amount of the specific antibodies in the blood. This test not only shows whether a person has syphilis but can also detect the severity of the infection.

A negative VDRL test result means the person does not have antibodies to syphilis in their blood. People with negative results do not need additional testing.

A positive result means that the VDRL test has confirmed the presence of syphilis antibodies. Those with a positive VDRL test result require an additional treponemal test to confirm the diagnosis, which is specific for syphilis infection.

Symptoms of syphilis vary according to the stage of the disease. The four stages are as follows:

• Primary stage: A single chancre (there may be multiple sores) appears at the location where the syphilis bacteria enters the body. The chancre is usually firm, round, and painless. If primary syphilis is not treated, secondary syphilis develops.
• Secondary stage: Skin rashes appear in the mouth, vagina, anus, palms and soles of the feet.
• Latent stage: There are no symptoms at this stage.
• Tertiary syphilis: Tertiary syphilis is fatal and occurs 10 to 30 years after the infection first started. In this stage, syphilis damages the internal organs.

Without treatment, syphilis can damage the heart and blood vessels, brain, and nervous system. It can be life-threatening. However, syphilis can be cured with penicillin if diagnosed early. So, it is very important to see a doctor as soon as symptoms appear or if there are risk factors. The risk factors are as follows:

• Engaging in sexual activities without barrier protection
• Homosexual activities between men
• Diagnosis of HIV
• Having more than one sexual partner

Syphilis infection can pass from a pregnant woman to foetus and also to an infant during delivery.
