The thyroid gland is located at the front of the neck. Thyroxine (T3) and triiodothyronine (T4) are two hormones produced by the thyroid gland. These thyroid hormones are essential for maintaining the metabolic rate. T3 and T4 work together to regulate how the body uses energy.

These hormones also play an important role in controlling weight, body temperature, muscle strength, and the nervous system. The thyroid hormones keep the brain, heart, muscles, and other organs working as they should.

The pituitary gland produces thyroid stimulating hormone, which tells the thyroid how much of its own hormones to make. The pituitary gland is located at the base of the brain under the hypothalamus (a part of the brain), which controls blood pressure, heart rate, body temperature, and digestion. The pituitary gland is connected to the hypothalamus. The hypothalamus secretes TRH (thyrotropin-releasing hormone) to trigger the release of the thyroid-stimulating hormone by the pituitary.

If the thyroid produces low levels of hormones (underactive thyroid), the pituitary increases the secretion of TSH to stimulate the thyroid. Whereas if the thyroid is overactive, the pituitary stops releasing TSH to control the thyroid. Thus, the TSH controls the secretion of thyroid hormones because the right amount of thyroid hormones keeps the organs working smoothly.

An overactive thyroid (hyperthyroidism) speeds up the body's metabolism, causing serious health problems such as irregular heartbeat, stroke, heart failure and other heart conditions. Hyperthyroidism can cause Graves’ ophthalmopathy, an eye disease with symptoms of double vision, light sensitivity, and eye pain. An underactive thyroid (hypothyroidism) slows down the metabolic rate. Hypothyroidism can cause an enlarged heart, heart disease, goitre, infertility, birth defects, etc. Hashimoto's thyroiditis is a common cause of hypothyroidism, which more often develops in women than men.

TSH levels are checked during pregnancy. TSH is temporarily low in the first trimester, which then returns to normal. But some people get thyroid problems during pregnancy, especially if there is a personal history of thyroid disease, which may affect the baby's brain development or cause other problems with pregnancy. Newborns are also routinely given a TSH test to screen for congenital hypothyroidism.

Healthy people over 80 who have no thyroid problems may have high TSH.

A normal TSH almost always means that the thyroid is healthy. However, the TSH test alone may not be enough to confirm the diagnosis. In that case, T3 and T4 tests can be part of the diagnostic process.

Test Necessity

Test Necessity

The TSH test is performed to help diagnose hyperthyroidism and hypothyroidism.

Some symptoms of hypothyroidism are:

  • Weight gain
  • Cold sensitivity
  • Dry skin, thinning hair
  • Irregular menstrual cycles
  • Depression

Hyperthyroidism may exhibit these symptoms:

  • Weight loss
  • Rapid heartbeat
  • Frequent bowel movement
  • Heat sensitivity
  • Muscle weakness
  • Irritability

If you are taking thyroid hormone medicine because of thyroid removal, you should have regular TSH tests to monitor your thyroid hormone levels.

Even after treating hyperthyroidism, TSH tests should be done to monitor thyroid levels.

Test Preparation

Test Preparation

No special preparation is needed for the TSH test, including fasting. Let your doctor know if you are taking any medicines or health supplements.

During the test

The start of the test entails a phlebotomist going to your home with a vein finder and a supply of new needles and syringes. The suitable vein for blood extraction will be located with the vein finder, and a small volume of your blood will be collected and sealed into a tube.
