Fructosamine Test

The fructosamine test, also called the glycated serum protein (GSP) test, measures the level of fructosamine, which is formed when glucose attaches to blood proteins, primarily albumin. This test helps doctors assess how well blood sugar has been controlled over the past 2–3 weeks.

Although the HbA1c test is commonly used for assessing longer-term blood glucose control, the fructosamine blood test may be more useful when a shorter-term assessment is required or when HbA1c may be unreliable because of conditions affecting red blood cells or haemoglobin. These include conditions such as haemolytic anaemia, sickle cell anaemia, and anaemia caused by iron, folate, or vitamin B12 deficiencies. Your doctor may recommend a fructosamine blood test to assess recent blood sugar control or when HbA1c test results may be unreliable.

The doctor might advise the fructosamine blood test if you have:

• Gestational diabetes (diabetes during pregnancy)
• Blood loss or a condition that affects your RBCs, such as haemolytic anaemia
• Recently started or changed diabetes treatment and your doctor wants to check whether it is working properly

The fructosamine blood test helps monitor short-term blood glucose control over the previous 2–3 weeks. It is commonly used in people with gestational diabetes, conditions affecting RBCs, or after changes in diabetes treatment.

What to Expect Before the Fructosamine Test

No fasting is required for a standalone fructosamine test. You can eat and drink normally. However, if it is part of a broader package that includes fasting tests, your doctor may advise fasting for 8-12 hours. Avoid strenuous exercise for 24 hours before the test, as intense physical activity can temporarily raise fructosamine levels and affect interpretation. Always inform your doctor about any ongoing medications.

What to Expect During the Blood Collection

A phlebotomist cleans the skin with an antiseptic, then places an elastic band around your upper arm to make the veins easier to access. A small needle is inserted into the vein; you may feel a brief pinch. Blood is collected into a labelled tube in under a minute.

What to Expect After the Fructosamine Test

A small cotton swab and bandage are placed over the site to stop any minor bleeding. You can return to normal activities immediately. Minor bruising is normal and disappears quickly. If dizzy, sit and rest for a few minutes.

The fructosamine test is generally safe and has very low risk. Mild pain, bruising, or slight bleeding may occur at the needle site. This usually resolves on its own soon.

Limitations:

Similar to any other test, the fructosamine blood test may also have some limitations:

• It cannot be used alone to diagnose diabetes; it only helps monitor sugar levels over a short period.
• Other follow-up tests must be done to confirm the diagnosis of diabetes.
• Vitamin C, bilirubin, and temperature variations can affect fructosamine results.
• Conditions that affect blood protein levels, especially albumin, such as liver cirrhosis, multiple myeloma, or nephrotic syndrome, could affect the fructosamine test results.
