A quick info on Prolactin test
Overview
A prolactin (PRL) test is a blood test that measures the level of prolactin, a hormone produced by the pituitary gland. Prolactin levels are naturally higher during pregnancy and breastfeeding, as prolactin is primarily responsible for breast development during pregnancy and milk production after childbirth.
In other individuals, elevated prolactin levels, known as hyperprolactinemia, may indicate an underlying medical condition. High prolactin levels can affect reproductive and sexual health, leading to symptoms such as irregular menstrual cycles, infertility, reduced libido, erectile dysfunction, or unexpected breast milk production (galactorrhoea)
Common causes of high prolactin levels include prolactinoma, a non-cancerous tumour of the pituitary gland, as well as certain medications, hypothyroidism, kidney disease, and other pituitary gland disorders.
A prolactin test helps evaluate abnormal prolactin levels and supports the diagnosis and monitoring of hormone-related conditions.
When and Who Should Get a Prolactin Test?
- A prolactin test may be recommended if symptoms suggest elevated prolactin levels. In women, these may include irregular or absent menstrual periods, infertility, nipple discharge, or breast milk production unrelated to pregnancy or breastfeeding. In men, symptoms can include reduced sex drive, erectile dysfunction, enlarged breast tissue, nipple discharge, or infertility.
- When high prolactin levels are caused by a prolactinoma, additional symptoms such as headaches, vision changes, or signs of other hormone deficiencies may occur due to pressure from the tumor on nearby structures in the brain.
- The test may also be used to monitor treatment response in individuals diagnosed with a prolactinoma and to check for recurrence after treatment.
How Frequently Should You Take the Prolactin Test?
- A prolactin test is not usually performed as part of routine health screening. The frequency of testing depends on the underlying condition, symptoms, treatment plan, and your doctor’s recommendations.
- In case of mild prolactin elevation, repeating a serum prolactin level is recommended, as levels can often normalise, in which case further testing (especially in an asymptomatic individual) is unnecessary.
- Repeat testing may also be advised to monitor treatment response or assess recurrence of conditions such as prolactinoma.
What Conditions Can a Prolactin Test Help Detect?
A prolactin test is most often used with other tests to help diagnose the following conditions:
In women who are not pregnant or breastfeeding and in men, higher-than-normal prolactin levels may be a sign of many types of conditions, such as:
- Prolactinoma or another pituitary disorder
- Hypothalamus disease (it is an area of the brain that controls the pituitary gland and other body functions)
- Hypothyroidism (underactive thyroid gland)
- Cirrhosis (scarring of the liver)
- Kidney disease
- Polyendocrine metabolic ovarian syndrome (PMOS) in women
- Medication-Induced hyperprolactinemia: Certain medications, including some antipsychotics, antidepressants, and anti-nausea medicines, can increase prolactin levels
- Lower-than-normal prolactin levels are rare but may be a sign of a pituitary disorder.
Disclaimer: The above list is for educational purposes only and does not confirm a diagnosis or the presence of any condition. Always consult your doctor for your test results and diagnosis.
Prolactin Test Preparation
You may inform your doctor regarding regular medications that you take, as they may affect prolactin levels. However, never stop taking any medicine without talking to your doctor first. Your doctor may advise avoiding vigorous exercise, sexual activity, or breast / nipple stimulation for 24 hours before testing, as these can temporarily increase prolactin levels.
What to Expect Before the Test
- Your doctor may ask you not to eat or drink for several hours before your test.
- As prolactin levels fluctuate throughout the day, your blood sample will usually be scheduled three to four hours after you wake up.
What to Expect During the Sample Collection
- A doctor or a phlebotomist will take a blood sample from a vein in your arm, using a small needle. After the needle is inserted, a small amount of blood will be collected into a test tube or vial. You may feel a little sting when the needle goes in or out. This usually takes less than five minutes.
What to Expect After the Test
- The sample will then be sent to a lab to be analysed. Your test results should be available within a few hours.
Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.
What Are the Parameters Included in the Prolactin Test?
The prolactin test measures a single key parameter:
- Serum prolactin level - Measured in ng/mL (nanograms per millilitre) or mIU/L (milli-international units per litre).
Because prolactin is a pulsatile hormone and is released in bursts, a single reading can be misleading. Stress, physical activity, food, breast stimulation, sleep, and even the act of blood collection itself can temporarily raise prolactin.
Normal Range of Prolactin Test Parameters
The table below shows standard reference ranges of prolactin levels.

Prolactin Test Result Interpretation
The prolactin result must always be interpreted in the context of your gender, pregnancy status, symptoms, and current medications.
|
Prolactin level |
Classification |
Potential interpretation |
|---|---|---|
|
Prolactin within the laboratory reference range |
Normal |
Prolactin is within the expected range. No evidence of hyperprolactinemia. |
|
Mildly elevated |
Hyperprolactinaemia |
May indicate stress-induced elevation, medication effect, hypothyroidism, pmos or a small prolactinoma. Repeat tests and clinical review may be recommended. |
|
Moderately elevated |
Hyperprolactinaemia |
May be associated with a prolactinoma, medication-induced prolactin elevation, hypothyroidism, kidney disease, or other pituitary disorders. Further evaluation is usually required. |
|
Markedly elevated |
Hyperprolactinaemia |
Strongly suggests a macroprolactinoma (larger tumour). particularly when accompanied by relevant symptoms and imaging findings. |
|
Very high levels |
Severe hyperprolactinemia |
Consistent with a large macroprolactinoma or invasive pituitary tumour. However, additional investigations are required to determine the underlying cause. |
|
Prolactin below the laboratory reference range. |
Hypoprolactinaemia |
Suggests pituitary gland dysfunction (hypopituitarism). |
Disclaimer: This table is for educational purposes only. Prolactin interpretation requires the expertise of a qualified endocrinologist or gynaecologist. A single elevated result must be repeated before clinical decisions are made.
Prolactin Test Risks and Limitations
Potential Risks and Complications
- There is very little risk to having a blood test. You may have slight pain or bruising at the spot where the needle was put in, but most symptoms resolve quickly.
Understanding Prolactin Test Limitations
- Prolactin is highly sensitive to physical and emotional stressors. A single elevated result is therefore insufficient for diagnosis and must be confirmed with a repeat morning fasting sample.
- Pregnancy and breastfeeding naturally raise prolactin levels and must be considered when evaluating results.
- Medications including antipsychotics, metoclopramide, and domperidone are common causes of elevated prolactin that can be missed without a thorough medication review.
- Low prolactin requires evaluation of the full pituitary hormone panel.
- In some individuals, elevated prolactin levels may be caused by macroprolactin, a biologically inactive form of prolactin that can produce high laboratory results without causing symptoms. Your doctor may recommend macroprolactin testing if prolactin is elevated but symptoms are absent or inconsistent with the result.
