A quick info on HBsAg test

A quick info on HBsAg test

Overview

Overview

The Hepatitis B Surface Antigen (HBsAg) test detects a protein on the outer coat of the hepatitis B virus (HBV), making it the primary marker used to identify ongoing hepatitis B infection. hepatitis B is a liver infection caused by the HBV, which can lead to short-term (acute) or long-term (chronic) illness, with chronic infection raising the risk of liver damage, cirrhosis, and liver cancer over time. The virus spreads through contact with infected blood or body fluids, commonly via unprotected sexual contact, sharing needles, exposure through cuts or wounds, and from an infected mother to her baby during childbirth. It does not spread through casual contact. A positive HBsAg test result indicates that a person is infectious, though it may appear temporarily positive within 30 days of a hepatitis B vaccine dose. More than half of people living with hepatitis B remain unaware of their infection, and many acute cases show no symptoms. Screening guidelines recommend HBsAg testing at least once for all adults, along with targeted testing for pregnant women, infants born to HBsAg-positive parents, and those with recognised risk factors.

When and Who Should Get a HBsAg Test Done?

When and Who Should Get a HBsAg Test Done?

  • All adults: At least once in a lifetime, regardless of risk factors. Screening is generally done using the HBsAg, anti-HBs, and total anti-HBc triple panel.
  • Pregnant women: Routine antenatal testing, ideally in the first trimester.
  • Infants: Born to HBsAg-positive mothers.
  • People with symptoms: Yellowish eyes and skin (jaundice), fatigue, fever, dark urine, grey or clay-coloured stools, abdominal pain, diarrhoea, nausea, vomiting, joint pain or loss of appetite.
  • Known/suspected exposure: People with known or suspected exposure to HBV.

People with ongoing risk of exposure, including:

  • History of sexually transmitted infections, multiple partners or practising unprotected sex
  • Past or current hepatitis C infection
  • Individuals currently or previously held in a correctional facility or prison
  • Birth in HBV-prevalent regions
  • Unvaccinated infants of parents from high-prevalence regions
  • Injection drug use
  • HIV-positive status
  • Men who have sex with men
  • Household or sexual contacts of an HBV-positive person
  • Dialysis patients
  • People with diabetes
  • Elevated liver enzymes
  • Healthcare workers with blood exposure risk
  • People who have travelled to high-prevalence countries
What Conditions Can a HBsAg Test Help Detect?

What Conditions Can a HBsAg Test Help Detect?

The HBsAg test detects the hepatitis B virus itself in the body, hence it is used to detect:

  • Active acute hepatitis B: A short-term infection that occurs within the first 6 months after exposure to the virus. Most healthy adults clear the infection on their own and develop lifelong immunity. Symptoms, if present, may include fatigue, jaundice, dark urine, and abdominal discomfort, though many people remain symptom-free.
  • Active chronic hepatitis B: Occurs when the immune system is unable to clear the virus within 6 months, resulting in a long-term infection. Chronic hepatitis B often has no noticeable symptoms for years, but it can gradually damage the liver, increasing the risk of cirrhosis and liver cancer over time. It is more common in people infected during infancy or early childhood compared to those infected as adults.

Note: The HBsAg test detects the actual virus (surface antigen), a reactive or a positive result means the virus is currently present and the person is infectious.

HBsAg Test Preparation

HBsAg Test Preparation

What to Expect Before the HBsAg Test?

  • No special preparation, such as fasting, is required for the HBsAg test.
  • Wearing clothing with easy arm access can make the blood draw more convenient.
  • The HBsAg test can be done at any time of the day.
  • Inform your doctor if you are pregnant, recently vaccinated against hepatitis B, or have had recent exposure to blood or body fluids, as this can help with result interpretation. Also inform them if you are on high-dose biotin (typically used in hair, skin, or nail supplements) or have undergone recent blood transfusion or blood donation.

What to Expect During the Blood Collection?

  • A trained phlebotomist cleans your arm with an antiseptic swab and ties a band above the elbow to make the vein more visible.
  • A small needle is then used to draw a blood sample from a vein, usually in the arm.
  • The process takes only a few minutes and involves mild, brief discomfort similar to a pinprick.
  • A small bandage is applied afterwards to prevent bleeding.

What to Expect After the HBsAg Test?

  • Results are typically available within a few hours via digital report.
  • If the result is positive, your doctor will guide you on further tests and next steps for evaluation and care.

Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.

What Are the Parameters, Ranges and their Interpretation Included in the HBsAg Test?

What Are the Parameters, Ranges and their Interpretation Included in the HBsAg Test?

The HBsAg test detects the hepatitis B surface antigen, a protein found on the outer coat of the hepatitis B virus itself. Because it detects a piece of the actual virus, a reactive or positive result means the virus is currently present and the person is infectious.

Result

What it may indicate

Interpretation

Non-reactive

Indicates the absence of hepatitis B viral surface antigen

No current hepatitis B infection detected

Reactive

Indicates the presence of hepatitis B viral surface antigen

Active acute or chronic hepatitis B infection

Note: Unlike anti-HBs or anti-HBc, which detect antibodies produced by the body in response to the virus, the HBsAg test detects a protein from the virus itself, making it a direct marker of active infection. A reactive result should always be confirmed with additional testing (HBV DNA PCR or other tests) and reviewed by a doctor, as it does not by itself distinguish between acute and chronic infection.

HBsAg Test Risks and Limitations

HBsAg Test Risks and Limitations

Potential Risks and Complications

  • The HBsAg test is a simple blood test with minimal risk.
  • Common, minor effects at the puncture site may include brief stinging during the needle prick, slight bruising, or mild tenderness lasting a day or two.
  • Less common risks include excessive bleeding, particularly in those on blood-thinning medication, fainting or light-headedness during the draw, and infection at the puncture site.
  • A haematoma, a raised bruise from blood pooling under the skin, may also occur occasionally.
  • These effects are typically minor and resolve without treatment. Patients with a history of fainting or bleeding disorders should inform the phlebotomist beforehand.

Understanding HBsAg Test Limitations

  • Does not detect past infection or immunity: a negative result does not rule out prior exposure to hepatitis B.
  • Does not differentiate between acute and chronic infection: a reactive result confirms active infection but requires additional testing (such as IgM anti-HBc) to determine the stage.
  • May temporarily stay positive after vaccination: HBsAg can show a false positive for up to 30 days following a hepatitis B vaccine dose.
  • Does not indicate disease severity.
  • False-positive: A test result that incorrectly shows a reactive result when the person is not actually infected. This may occur due to certain autoimmune conditions or interfering antibodies or lab or sample-handling errors.
  • False-negative: A test result that incorrectly shows a non-reactive result when the person is actually infected. This may occur due to testing too early after exposure, before HBsAg reaches detectable levels, very low viral load, mutant HBV strains not recognised by the test assay, or occult hepatitis B infection, where the virus is present at levels below detection.

Does the HBsAg test detect past hepatitis B infection?

No, HBsAg only detects current infection. To check for past infection or immunity, tests such as anti-HBc and anti-HBs are needed.

Can the HBsAg test tell if my infection is acute or chronic infection?

No, a reactive HBsAg result confirms active infection but cannot distinguish between acute and chronic on its own. Additional testing, such as IgM anti-HBc, is needed for this.

Is fasting required before the HBsAg test?

No, fasting is not required. You can eat and drink normally before the test.
