Overview

The HBsAg test is a blood test that detects the hepatitis B surface antigen (HBsAg), a protein found on the surface of the hepatitis B virus (HBV). The presence of HBsAg in the blood can indicate a current hepatitis B infection, which may be acute or chronic. Hepatitis B is a viral infection that mainly affects the liver and may cause symptoms such as tiredness, nausea, fever, dark urine, abdominal pain, or yellowing of the skin and eyes. However, many people with hepatitis B may not have any symptoms, making testing important for early detection. Chronic hepatitis B infection may also lead to liver scarring, failure, or cancer. Doctors may recommend HBsAg testing when hepatitis B infection is suspected, after possible exposure to the virus, or as part of screening. Testing is particularly important during pregnancy because hepatitis B can pass from a mother to her baby. Centres for Disease Control and Prevention (CDC) recommends screening for HBsAg during every pregnancy, preferably in the first trimester. HBsAg is usually interpreted along with other hepatitis B markers, such as anti-HBs, and total anti-HBc, to determine whether a person has a current or past infection, immunity, or susceptibility to hepatitis B.

  • All adults: At least once in a lifetime, regardless of risk factors. Screening is generally done using the HBsAg, anti-HBs, and total anti-HBc triple panel.
  • Pregnant women: Routine antenatal testing, ideally in the first trimester.
  • Infants: Born to HBsAg-positive mothers.
  • People with symptoms: Yellowish eyes and skin (jaundice), fatigue, fever, dark urine, grey or clay-coloured stools, abdominal pain, diarrhoea, nausea, vomiting, joint pain or loss of appetite.
  • Known/suspected exposure: People with known or suspected exposure to HBV.

People with ongoing risk of exposure, including:

  • History of sexually transmitted infections, multiple partners or practicing unprotected sex
  • Past or current hepatitis C infection
  • Individuals currently or previously held in a correctional facility or prison
  • Birth in HBV-prevalent regions
  • Unvaccinated infants of parents from high-prevalence regions
  • Injection drug use
  • HIV-positive status
  • Men who have sex with men
  • Household or sexual contacts of an HBV-positive person
  • Dialysis patients
  • People with diabetes
  • Elevated liver enzymes
  • Healthcare workers with blood exposure risk
  • People who have travelled to high-prevalence countries

What Conditions Can a HBsAg Test Help Detect?

The HBsAg test detects the hepatitis B virus itself in the body, hence it is used to detect:

  • Active acute hepatitis B: A short-term infection that occurs within the first 6 months after exposure to the virus. Most healthy adults clear the infection on their own and develop lifelong immunity. Symptoms, if present, may include fatigue, jaundice, dark urine, and abdominal discomfort, though many people remain symptom-free.
  • Active chronic hepatitis B: Occurs when the immune system is unable to clear the virus within 6 months, resulting in a long-term infection. Chronic hepatitis B often has no noticeable symptoms for years, but it can gradually damage the liver, increasing the risk of cirrhosis and liver cancer over time. It is more common in people infected during infancy or early childhood compared to those infected as adults.

Note: The HBsAg test detects the actual virus (surface antigen), a reactive or a positive result means the virus is currently present and the person is infectious.

What to Expect Before the HBsAg Test?

  • The HBsAg test requires no special preparation, and fasting is not necessary beforehand.
  • It is helpful to let the doctor know if you are pregnant, recently vaccinated for hepatitis B, or have had exposure to blood or body fluids recently.
  • Also let them know if you are on high-dose biotin (typically used in hair, skin, or nail supplements) or have undergone recent blood transfusion or blood donation.
  • The HBsAg test can be done at any time of the day.
  • Choosing clothing that allows easy access to the arm can also make the blood draw a bit smoother.

What to Expect During the Blood Collection?

  • The arm is first cleaned with an antiseptic swab to minimise infection risk. A tourniquet band is tied above the elbow, making the vein easier to locate and access.
  • Using a small needle, the phlebotomist draws a blood sample from a vein, typically in the arm. The entire process takes just a few minutes and involves brief, mild discomfort comparable to a pinprick.
  • Once the sample is collected, the needle is withdrawn, and gentle pressure is applied to the site. A small bandage is then placed to support clotting and prevent bleeding. Patients are usually asked to keep the arm still for a few seconds during the draw to ensure a smooth, uninterrupted collection.

What to Expect After the HBsAg Test?

  • After the blood sample is collected, you can usually return to your normal activities immediately. You may experience mild soreness, tenderness, or a small bruise at the site where the needle was inserted, which generally resolves on its own.
  • Once the sample is processed, your HBsAg result will be uploaded. If the result is reactive, your doctor may recommend additional hepatitis B tests to confirm the infection and determine whether it is acute or chronic.

  • Less frequently, some patients may experience excessive bleeding, particularly if they are on blood-thinning medication, along with fainting, light-headedness, or infection at the site. Occasionally, a haematoma, a raised bruise caused by blood collecting under the skin, may develop as well.
  • Most of these effects are minor and settle on their own without treatment. Anyone with a history of fainting or a bleeding disorder should mention this to the phlebotomist before the blood draw.
