A quick info on Creatinine test

A quick info on Creatinine test

Overview

Overview

The creatinine test, also referred to as a serum creatinine test or blood creatinine test, checks how much creatinine is present in your blood. This helps doctors understand how well your kidneys clear waste from your body. Creatinine is produced during the breakdown of creatine, a substance your body uses to provide energy for muscle activity. Every time you use your muscles, creatinine is released into your bloodstream. Throughout the day, your blood passes through the kidneys repeatedly. Inside the kidneys, tiny filtering units called glomeruli filter your blood, allowing waste products like creatinine to be eliminated in urine. When kidney function is compromised, this filtering process becomes less efficient, causing incomplete filtration of creatinine. This leads to creatinine accumulation in the blood. The creatinine test detects this unfiltered creatinine circulating in your blood. The pace at which your kidneys filter blood is known as the glomerular filtration rate (GFR). Since GFR cannot be measured directly, your doctor may use your creatinine test result to calculate an estimated GFR (eGFR). The serum creatinine test does not require fasting before sample collection, and PharmEasy test reports are usually available within about 10 hours to a day, depending on your location in Kolkata.

When and Who Should Get a Serum Creatinine Test Done?

When and Who Should Get a Serum Creatinine Test Done?

Your doctor may recommend a serum creatinine test in the following situations:

  • During a regular health checkup, usually as part of a metabolic panel.

If you have symptoms of kidney disease such as:

  • Swelling in the hands and feet or puffy eyelids
  • Dry skin, itching, or numbness
  • Fatigue
  • Increased or decreased need to urinate
  • Urine that is bloody or foamy
  • Loss of appetite and weight loss
  • Muscle cramps
  • Nausea and vomiting
  • Shortness of breath
  • Sleep problems
  • Trouble thinking clearly

If you have conditions that can increase your risk for developing kidney disease, such as:

  • Diabetes
  • High blood pressure
  • A family health history of kidney disease, diabetes, or high blood pressure
  • Heart disease
  • Age over 50 years
  • Smoking habit
  • Obesity

If you have kidney problems or suspected kidney problems, such as:

  • Had an abnormal kidney test result in the past
  • Been diagnosed with kidney disease
  • Taken certain medicines that could affect your kidneys
  • Had a kidney transplant
What conditions can Serum Creatinine Test help detect?

What conditions can Serum Creatinine Test help detect?

Blood creatinine levels, along with other test results, are used to calculate estimated glomerular filtration rate and creatinine clearance, which help assess kidney function.

Screens for kidney disease or reduced kidney function before symptoms appear.

Aids in diagnosing kidney problems:

  • Acute kidney injury
  • Chronic kidney disease

Monitoring kidney function in people with known risk factors for kidney damage:

  • Diabetes
  • High blood pressure
  • Heart disease or heart failure
  • Autoimmune conditions affecting the kidneys (e.g., lupus)

Monitoring kidney function before and during certain treatments:

  • Before starting medications that can affect the kidneys
  • Monitoring kidney function while on these medications long-term
  • Before and after kidney-related procedures or surgeries

Monitoring disease progression such as in chronic kidney disease.

Many drugs are cleared by the kidneys, so doctors use creatinine levels to determine or adjust drug dosages.

What to Expect Before the Test?

  • If your serum creatinine test is being done along with other blood tests, you may be asked to avoid eating or drinking for a specific period before sample collection. If not, you do not need to fast for this test.
  • Certain prescription medicines, over-the-counter drugs, and dietary supplements may influence serum creatinine levels. Before the test, inform your doctor if you take any of these. If you take creatine supplements, you may be asked to discontinue them for 48 hours or more before undergoing the test.
  • Also, inform your doctor about any health conditions and your pregnancy status.
  • Intense exercise shortly before the test may slightly influence serum creatinine levels. Avoid vigorous workouts for 24 hours before the test.
  • Avoid a meat-rich diet for 24 hours before the test, as this can also affect serum creatinine levels.

What to Expect During the Sample Collection?

  • The serum creatinine test is a quick procedure that typically takes only a few minutes. During sample collection, a trained phlebotomist cleans the skin and draws a small blood sample from a vein, usually in the arm, using a sterile needle.
  • You may feel slight pain or a stinging sensation when the needle is inserted.

What to Expect After the Test?

  • You may feel mild throbbing at the site after the blood is drawn. Once the blood sample is collected, it is taken to the laboratory for analysis, where your serum creatinine levels are measured.
  • Once the analysis is complete, you can view and download the test report through the PharmEasy app or website.

Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.

What are the Parameters Included in the Serum Creatinine Test?

Serum creatinine results must always be interpreted in the context of your gender, symptoms, medications, and other health conditions.

Serum creatinine result

What it may mean

Normal

  • Creatinine levels depend on muscle mass, diet, age, and physical activity. Normal levels usually mean kidneys are healthy.
  • Sometimes, blood creatinine levels remain in a normal range during the early stages of kidney disease. If kidney disease is suspected, other kidney-related investigations may be advised, even if your creatinine results seem normal.

High

  • Kidney disease or injury, including infection, poor blood flow to the kidneys, a blockage in the urinary system, or kidney failure.
  • A condition that affects the kidneys, such as heart failure or diabetes.
  • Causes not directly related to the kidneys such as dehydration, muscle disorders, and injuries, muscular dystrophy, intense exercise, or a diet high in meat.
  • Certain health problems in pregnancy can also cause increases in serum creatinine.

Low

  • Malnutrition.
  • Conditions that cause muscles to get smaller, such as a long-term illness, nerve disorders, or muscle loss from ageing.
  • Normal pregnancy changes.
  • Serious liver disease.
Serum Creatinine Test Risks and Limitations

Serum Creatinine Test Risks and Limitations

Potential Risks and Complications

  • The serum creatinine test is generally safe with minimal risk.
  • Mild pain, slight bleeding, or bruising may occur at the blood draw site.
  • Occasionally, vein access may be difficult, or infection may occur.
  • Sometimes, you may feel dizzy or light-headed.

Understanding Serum Creatinine Test Limitations

  • People with high muscle mass (like bodybuilders) may have higher creatinine levels without kidney disease, while older adults or those with low muscle mass may have normal creatinine levels even with significant kidney damage.
  • Sometimes serum creatinine is within the normal range until kidney function is already significantly reduced, frequently missing early-stage kidney disease.
  • Eating large amounts of meat or taking creatine supplements can temporarily increase blood creatinine levels. A vegetarian or vegan diet can lead to lower levels.
  • Certain medications block the kidneys from excreting creatinine, causing falsely high blood creatinine levels without actual kidney damage.
  • In sudden kidney failure (acute kidney injury), serum creatinine levels take time to accumulate in the blood, leading to normal test results despite significant damage to the kidneys.
  • Severe dehydration can temporarily concentrate the blood and raise serum creatinine levels.

Persistently high creatinine may require further evaluation to determine whether reduced kidney function or another underlying cause is responsible.

Can a serum creatinine test detect kidney damage early?

Serum creatinine is useful for assessing kidney function, but it may not detect early kidney damage on its own. Other tests may be required.
