AMH plays different roles in the body of both men and women, and levels of this hormone vary with age and sex. An anti-Müllerian hormone is produced by the reproductive tissues of the foetus before birth while in the womb. The sex of an unborn child depends on whether it inherits chromosomes from both parents or only from the mother. But hormones play a role in the development and formation of the child's reproductive organs, among which AMH is one.

AMH plays a major role in sex differentiation in foetuses. Very early in pregnancy, the foetus has a set of Müllerian ducts. High levels of anti-Müllerian hormone are produced in the testicular tissues of male babies. These high levels of AMH prevent female organs from forming. High AMH makes the Mullerian ducts shrink and assists in the growth of male reproductive organs. This level remains high until puberty then starts to decline.

Low levels of anti-Müllerian hormone are released during the prenatal period in order to develop female reproductive structures. In girls, levels of this hormone increase after puberty when the ovaries begin to produce AMH. AMH levels decline with egg release during their reproductive years and cease at menopause. AMH levels can show the size of the ovarian reserve. AMH testing can therefore be useful to understand a woman's chances of conceiving, and it may predict the onset of menopause. The AMH test cannot predict fertility as it depends on other factors like sperm count, health conditions etc.

Polycystic ovarian syndrome is a condition in which the ovarian follicles release too much AMH. PCOS can cause irregular menstrual periods. The AMH test can be useful in diagnosing PCOS and monitoring the treatment of this disease.
