Current scientific evidence does not show that using sunscreen causes cancer1. In fact, sunscreen helps protect the skin from ultraviolet (UV) radiation which is a known risk factor for skin cancer2

Concerns about sunscreen often arise from questions about specific ingredients and their absorption into the body3. However, finding that an ingredient is absorbed does not mean that it causes cancer. The available evidence has not established sunscreen use as a cause of cancer1,3.

UV radiation can damage skin cells and increase the risk of skin cancer. Sunscreen use can help reduce the risk of skin cancer by protecting the skin from this harmful radiation7. Therefore, regular sunscreen use is an important part of a broader sun-protection strategy. 

Let us understand the basics of sunscreen for skin cancer prevention7:

  • Blocks harmful UV radiation: A broad-spectrum sunscreen protects against both UVA and UVB rays. Both of these rays can damage the skin and may contribute to skin cancer. 
  • Reduces UV-related DNA damage: By limiting the amount of UV radiation reaching the skin, sunscreen helps reduce damage at the cellular level. 
  • Helps avoid sunburn: Sunburn is a sign of UV-related skin injury. So, by protecting against repeated sunburns, sunscreens can help reduce cumulative skin damage7.
  • May reduce the risk of skin cancer with regular use: Evidence supports sunscreen as part of skin cancer prevention, with particularly strong evidence for reducing squamous cell carcinoma (cancer in cells that form the middle and outer layers of the skin)9

Sunscreens contain certain active UV filters that protect the skin from harmful UV radiation10. Commonly recommended sunscreen ingredients include: 

  • Zinc oxide and titanium dioxide: These are inorganic UV filters (mineral-based compounds that absorb and scatter UV radiation). They are considered safe and effective for use in sunscreens at specified concentrations5. There is no evidence that their topical use in sunscreen causes cancer. 
  • Avobenzone, cinoxate, dioxybenzone, ensulizole, homosalate, meradimate, octinoxate, octisalate, octocrylene, oxybenzone, padimate O, sulisobenzone, and bemotrizinol: These are organic UV filters (chemical/carbon-based compounds that absorb UV radiation). They are permitted for use by regulatory authorities in most countries. Some ingredients continue to be reviewed for safety, but this does not mean they have been shown to cause cancer or other serious health problems5,10.

Ingredients like aminobenzoic acid (PABA) and trolamine salicylate are generally not recommended. This is because: 

  • PABA has been associated with allergic skin reactions in some individuals5,11. Laboratory studies have also raised concerns about its potential to increase the effects of UV radiation on cells. However, these findings have not been established in human skin12.  
  • Trolamine salicylate, on the other hand, has safety concerns related to salicylate toxicity5.  

It is important to understand that these are specific safety considerations for these ingredients and do not mean that sunscreens are harmful or unsafe when used as directed.  

Important: The classification and number of approved UV filters can vary by regulatory authority and may change as new ingredients are evaluated. Therefore, always check the latest information from the relevant regulatory authority for the most up-to-date list of approved UV filters. 

How to Choose a Safe Sunscreen?

When choosing a sunscreen for cancer prevention, look for the following features: 

  • Broad-spectrum protection: Choose a sunscreen that protects against both UVA and UVB rays. This provides more complete protection from UV-related skin damage. 
  • SPF 30 or higher: An SPF of 30 or higher is recommended for adequate protection against UVB radiation when the sunscreen is applied correctly. 
  • Water resistance: Water-resistant sunscreen is useful during swimming, sweating, or other activities involving water. Remember that no sunscreen is completely waterproof, so it needs to be reapplied as directed13
  • Suitable for your skin: If you have sensitive or easily irritated skin, consider products formulated for sensitive skin and avoid ingredients that you know may cause irritation or allergic reactions. 
  • Check the label: Choose a sunscreen that is properly labelled and approved or authorised by the relevant regulatory authority in your country.

Follow these simple steps to get the most protection from your sunscreen6

  • Apply before sun exposure: Apply sunscreen about 15 minutes before going outdoors so it has enough time to form a protective layer on the skin. 
  • Cover all exposed skin: Apply sunscreen evenly to all exposed areas, including the face, neck, arms, and legs. Remember to avoid contact with the eyes and mouth. 
  • Use enough sunscreen: Apply a generous, even layer to all exposed areas. An average-sized adult needs about 1 ounce (roughly a shot-glassful) to cover the entire body. 
  • Reapply regularly: Reapply at least every 2 hours and more often after swimming, sweating, or towel-drying. 
  • Do not forget commonly missed areas: Make sure to apply sunscreen to ears, nose, hands, lips (use an SPF lip balm), back of the neck, tops of the feet, along the hairline, exposed scalp, especially if you have thinning or no hair. 

          Note: Sunscreen should be used as one part of overall sun protection, along with seeking shade, wearing protective clothing, and avoiding excessive UV exposure7.

          When to Consult a Doctor?

          You should consider consulting a doctor if you have concerns about your skin or notice any unusual changes, such as14,15:

          • A new or changing mole or skin growth 
          • A sore or wound that does not heal 
          • A spot that bleeds or crusts 
          • A skin lesion that changes in size, shape, or colour 
          • A reaction to sunscreen, such as severe irritation, swelling, rash, or difficulty breathing
