It is common for some people to experience discomfort after eating specific foods. Problems such as bloating, stomach pain, gas, headaches, or nausea may appear after consuming items like milk, wheat, spicy foods, or processed products. In many cases, these reactions are linked to food intolerance. 

While usually less serious than a food allergy (that involves the immune system), food intolerance could still greatly affect daily comfort and quality of life1

What Is Food Intolerance?

Food intolerance is a non-allergic reaction to a food or ingredient that can cause symptoms after eating amounts that are usually well tolerated by others. It is one of the most common types of adverse food reactions and is estimated to affect up to 20% of the population.  

Food intolerance differs from food allergy because it does not involve the immune system. Instead, it usually occurs because the body has difficulty digesting or processing certain foods or ingredients1

Types of Food Intolerance

Food intolerance can occur in response to a wide range of food items, ingredients, or naturally occurring food chemicals. Some of the most common types of food intolerance include1,2

  • Lactose Intolerance: This is the most common form of food intolerance. It occurs when the body lacks enough lactase enzyme to properly digest lactose, the natural sugar found in milk and dairy products such as cheese, yogurt, and ice cream. 
  • Gluten Intolerance (Non-Celiac): Some individuals experience bloating and discomfort after consuming gluten (a protein present in wheat, rye, and barley) despite testing negative for celiac disease (an autoimmune condition triggered by gluten). Foods such as bread, pasta, and baked products are common triggers. 
  • Histamine Intolerance: Histamine is a naturally occurring compound found in fermented and aged foods. People with histamine sensitivity may experience food intolerance symptoms after consuming foods and beverages such as wine, cheese, and processed meats. 
  • Caffeine Intolerance: Certain individuals are highly sensitive to caffeine, which is commonly found in coffee, tea, energy drinks, chocolate, and some soft drinks. Even small amounts may cause food intolerance symptoms or discomfort. 
  • Alcohol Intolerance: Alcohol intolerance occurs when the body cannot efficiently break down alcohol, leading to food intolerance symptoms after drinking alcoholic beverages (such as wines, beers, champagne, whisky, etc). 
  • Sulphite Sensitivity: Sulphites are preservatives commonly used in foods and beverages like wine, beer, dried fruits, and packaged foods. In sensitive individuals, they may trigger food intolerance symptoms. 
  • Salicylate Intolerance: Salicylates are naturally found in certain fruits, vegetables, herbs, spices, and processed foods. Some people may react to these compounds. 
  • Monosodium Glutamate (MSG) Sensitivity: MSG is a flavour enhancer often added to savoury and processed foods. Some individuals may experience symptoms after consuming foods containing MSG. 

Causes of Food Intolerance

Food intolerance causes can vary from person to person and may include digestive, chemical, or gastrointestinal factors. Common causes of food intolerance include: 

  • Enzyme deficiency: Some people lack specific digestive enzymes needed to break down certain foods. For example, lactose intolerance occurs due to low levels of the lactase enzyme needed to digest lactose in dairy products1
  • Sensitivity to natural food chemicals: Certain naturally occurring substances in foods, such as caffeine, histamine, salicylates, or amines, can trigger symptoms in sensitive individuals1,2
  • Reaction to food additives: Preservatives, artificial colours, flavour enhancers, and sulphites present in processed foods may cause intolerance symptoms in some people1,2
  • Digestive disorders: Conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or other gastrointestinal problems can make people more sensitive to certain foods1
  • Difficulty absorbing nutrients: Some individuals may have problems absorbing specific carbohydrates or sugars, such as fructose, leading to bloating, gas, and diarrhoea1
  • Gut microbiome imbalance: Changes in gut bacteria and increased intestinal sensitivity may contribute to food intolerance symptoms and digestive discomfort1.

Symptoms of Food Intolerance

Symptoms of food intolerance can range widely depending on the individual. Common food intolerance symptoms include3

  • Bloating, gas, or diarrhoea 
  • Headaches or migraines 
  • Tremors or shakiness 
  • Excessive sweating 
  • Nervousness or irritability 
  • Burning or tingling sensations on the skin

Understanding the difference between food allergy and food intolerance is important because food allergies can sometimes become life-threatening, while food intolerance is usually less severe3,4

A doctor may refer you to a dietitian or food and nutrition specialist to help identify the possible trigger foods. Common methods used to diagnose food intolerance include: 

  • Elimination Diet: This is one of the most common approaches. The suspected food is removed from the diet for a few weeks to check whether symptoms improve. The food is then gradually reintroduced to see if symptoms return. 
  • Food and Symptom Diary: Keeping a record of foods eaten and symptoms experienced can help identify patterns and possible trigger foods. 
  • Blood Tests: Validated blood tests may be performed to rule out true IgE-mediated food allergies, nutrient deficiencies, or autoimmune conditions like celiac diseases that may be causing symptoms.  
  • Breath Tests: Breath tests are commonly used to diagnose certain intolerances, especially lactose intolerance.

The ideal management option for food intolerance is to avoid or reduce the intake of foods that trigger symptoms. Common approaches include: 

  • Eliminating Trigger Foods: Avoiding the food or ingredient responsible for symptoms is often the most effective way to manage food intolerance2,3. However, it’s important to replace the triggering food with an alternate source of similar nutritional content to avoid deficiencies. 
  • Reducing Portion Size: Some people may tolerate small amounts of the trigger food without developing symptoms, especially in cases of food intolerance (rather than food allergy)3
  • Enzyme Supplementation: For certain food intolerances caused by enzyme deficiencies, such as lactose intolerance, enzyme supplements (such as lactase supplementation) may help improve digestion and reduce symptoms when the trigger food is consumed5
  • Using Probiotics: Probiotics may help support gut health and improve the balance of intestinal microorganisms. They may also play a role in managing some food-related adverse reactions, although their benefits can vary depending on the probiotic strain used and individual patient factors5,6
  • Gradual Reintroduction: In certain cases, foods may be reintroduced slowly and in small quantities after a period of avoidance to assess tolerance levels3
  • Reading Food Labels Carefully: Checking ingredient labels can help avoid hidden sources of trigger foods or additives3
  • Dietary Guidance from a Specialist: Before removing foods from the diet, it is important to consult a doctor, allergy specialist, or dietitian. Unnecessary food restrictions may lead to nutritional deficiencies, particularly in children who require adequate vitamins and minerals for proper growth and development2

While these strategies are mostly preventive, during an adverse episode of intolerance of a food item, managing the associated symptoms is important. Medications such as antacids, antidiarrheals, or other symptom-relieving medications may be used to relieve discomfort7,8. However, these medicines do not address the underlying cause and should be used under medical guidance.  

Note: The effectiveness of enzyme supplements, probiotics, and other symptom relieving medications may differ from person to person. So, it is advisable to consult a doctor before starting any new treatment for food intolerance. 

When to See a Doctor?

You should consult a doctor if you or your child experiences symptoms of food intolerance that occur repeatedly or do not improve. Persistent digestive discomfort, bloating, diarrhoea, headaches, or reactions after eating certain foods may require medical evaluation to identify the trigger and rule out other conditions2

In addition to this, seek immediate emergency medical help if you notice2

  • Swollen, itchy, or raised skin rash (hives) 
  • Fainting or loss of consciousness 
  • Sudden swelling of the lips, tongue, mouth, or throat 
  • Severe dizziness, confusion, or drowsiness 
  • A child becoming limp, unusually sleepy, or unresponsive 
  • Difficulty breathing, wheezing, choking, or gasping for air 
  • Tightness in the throat or trouble swallowing 
  • Skin, lips, or tongue turning blue, grey, or unusually pale 

These symptoms may indicate anaphylaxis, a serious and potentially life-threatening allergic reaction that requires urgent medical attention.

Can menopause cause food intolerance? 

Hormonal changes during menopause may affect digestion and gut sensitivity11. This could make some women more sensitive to certain foods. So, although menopause does not directly cause food intolerance, symptoms may become more noticeable during this stage.
