Mounjaro is approved for both type 2 diabetes and chronic weight management, while Wegovy is approved specifically for chronic weight management. It is very important to keep in mind that they are not interchangeable. So, the right choice depends on your medical condition, treatment goals, and your doctor's recommendation.

How Mounjaro and Wegovy work differently
Mounjaro and Wegovy are incretin-based medicines that mimic the effects of natural gut hormones released after you eat.2,6,7 This helps regulate blood sugar and appetite. The main difference is that they act on different hormone receptors.
It is a GLP-1 receptor agonist. It stimulates insulin release when blood sugar is high, reduces glucagon secretion, slows stomach emptying, and increases feelings of fullness, helping reduce calorie intake by activating the GLP-1 receptor.7
It is a dual GIP and GLP-1 receptor agonist. In addition to activating the GLP-1 receptor, it also activates the GIP receptor.6 This dual mechanism distinguishes tirzepatide from semaglutide and is thought to contribute to its effects on blood sugar control and weight reduction observed in clinical trials.
Mounjaro and Wegovy are different medicines, not different brands of the same drug. They contain different active ingredients and have different approved uses, dosing schedules, and available strengths. They should not be used interchangeably or switched without your doctor's advice.
What the trials show
Semaglutide and tirzepatide have each been evaluated in large clinical trial programmes.
Semaglutide for chronic weight management was studied in the STEP programme,5 while tirzepatide was evaluated in the SURMOUNT programme.4 In their respective trials, both medicines produced clinically meaningful average weight reduction compared with placebo, forming the basis for their regulatory approvals.
The most relevant evidence for this comparison comes from the SURMOUNT-5 trial, which directly compared tirzepatide with semaglutide in adults with overweight or obesity. In this head-to-head study, tirzepatide produced greater average weight reduction than semaglutide.4

Single-dose vials start at an MRP of approximately ₹3,281 for the 2.5 mg/0.5 ml strength, while KwikPen MRPs range from ₹13,125 (2.5 mg) to ₹25,781 (15 mg), depending on the strength. And because each KwikPen contains four once-weekly doses, one pen typically provides about one month's treatment.6
It is available as a FlexTouch prefilled pen in strengths from 0.25 mg to 2.4 mg. Following price revisions in 2025 and 2026, MRPs start at approximately ₹5,660 for the 0.25 mg starter pen and rise to around ₹16,400 for the 2.4 mg maintenance pen.7
Although Wegovy has a lower entry price than the Mounjaro KwikPen, Mounjaro is also available as lower-cost single-dose vials. Price by strength
Side effects of Mounjaro and Wegovy
Gastrointestinal side effects are common with both Mounjaro and Wegovy, particularly during dose increases. These include nausea, diarrhea, vomiting, constipation, indigestion, and stomach discomfort.6,7 For many people, these effects improve as the body adjusts to treatment.
Whether one medicine causes more side effects than the other depends on several factors, including the dose, how quickly it is increased, and individual response. Your doctor will consider both effectiveness and tolerability when recommending a treatment.
Seek medical help right away if you have severe or persistent stomach pain (a sign of possible pancreatitis), signs of an allergic reaction, or symptoms of very low blood sugar (shakiness, sweating, confusion).
Note: Neither Mounjaro nor Wegovy is suitable for everyone. Neither medicine should be used by people with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia syndrome type 2 (MEN 2). They should be used only under medical supervision, which is why a valid prescription and regular follow-up are required.
The most common side effects are gastrointestinal and are usually most noticeable during the first few weeks of treatment or after a dose increase. Your doctor or pharmacist can advise you on ways to manage these side effects if they occur.6,7
Can you switch between Mounjaro and Wegovy?
People may switch between Mounjaro and Wegovy because of factors such as tolerability, availability, approved indication, or cost. However, the two medicines are not directly interchangeable.6,7 Therefore, this should only be done under medical supervision.
Both of them contain different active ingredients and use different dose-escalation schedules. So, there is no equivalent dose conversion between Mounjaro and Wegovy.6,7
If your doctor recommends switching, treatment is usually restarted at an appropriate lower dose of the new medicine and increased gradually while monitoring your response and side effects. Consult a doctor and do not stop, start, or switch either medicine without first consulting your doctor.
When might Mounjaro or Wegovy be considered?

Wegovy is approved specifically for chronic weight management,1,7 while Mounjaro is also approved for this indication.1,6 GLP-1 therapy is generally considered for adults with a BMI of 30 kg/m² or higher, or 27 kg/m² or higher with a weight-related condition such as high blood pressure or obstructive sleep apnoea.3 But keep in mind that these are general eligibility criteria rather than a self-assessment tool. Only your doctor will determine whether treatment is appropriate.
The SURMOUNT-5 trial directly compared tirzepatide with semaglutide and reported greater average weight reduction with tirzepatide. However, treatment decisions should not be based on one trial alone.
