What is Mounjaro?

Mounjaro is the brand name for tirzepatide. It is a once-weekly injectable medicine given under the skin. It belongs to a newer class of medicines that works by activating two natural gut hormone receptors: GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1). Together, these help improve blood sugar control and support long-term weight management (see what Mounjaro is approved for in India).

In India, Mounjaro is marketed by Eli Lilly and is approved for the management of type 2 diabetes and chronic weight management in eligible adults alongside lifestyle changes.1,4 Your doctor will determine whether it is suitable for your condition.

Tirzepatide is also available in India under the brand name Yurpeak, which is the same Eli Lily Tirzepatide marketed by Cipla in India and manufactured by Eli Lilly and distributed by Cipla. Both brands contain the same active ingredient, and your doctor may prescribe either based on availability and your treatment plan. You can learn more about the medicine on our tirzepatide molecule page.

Note: Mounjaro is intended to be used alongside a healthy diet, regular physical activity, and other treatments recommended by your doctor. It is designed for long-term management (not as a shortcut) and should always be used under medical supervision.

How Mounjaro works

Unlike most GLP-1 medicines, which act only on the GLP-1 receptor, tirzepatide works on both the GIP and GLP-1 receptors.4 This helps improve blood sugar control and supports weight management through several mechanisms by activating these two receptors. It helps your body:5

1

release more insulin when blood sugar levels are high

2

reduce the amount of sugar released by the liver

3

slow stomach emptying (helps you feel full for longer)

4

reduce appetite and food cravings

Because tirzepatide targets two hormone pathways instead of one, clinical trials have generally shown greater average weight loss with tirzepatide than with semaglutide (which acts only on the GLP-1 receptor). This has been evaluated in the SURMOUNT and STEP clinical trial programmes, including head-to-head studies.2 However, individual results vary and depend on factors such as your health condition, dose, treatment adherence, and lifestyle. Your doctor will monitor your progress and adjust the treatment plan as needed.

What Mounjaro is approved for in India

In India, Mounjaro is approved for two indications:1,4

Type 2 diabetes

To improve blood sugar control in adults, along with a healthy diet, regular physical activity, and, when needed, other diabetes medicines (if needed).

Chronic weight management

As an adjunct to a reduced-calorie diet and increased physical activity for adults with obesity (BMI ≥30 kg/m²) or overweight (BMI ≥27 kg/m²) with at least one weight-related medical condition.

Obesity is now recognised as a chronic disease that often requires long-term management. In December 2025, the World Health Organization (WHO) published its first guideline recommending the use of GLP-1 receptor agonists as part of obesity care for eligible adults (see above).3 This reflects the growing recognition that obesity is a chronic medical condition rather than simply a lifestyle issue.

Although Mounjaro is approved for these uses, it may not be suitable for everyone. In general, GLP-1-based medicines are considered for adults with a BMI of 30 kg/m² or higher, or 27 kg/m² or higher with at least one weight-related medical condition, but the decision to prescribe Mounjaro depends on your overall health, medical history, and your doctor's clinical assessment. If you are unsure whether you're eligible, consult a doctor to find out.

Mounjaro dosage and titration

Mounjaro is injected once a week (preferably on the same day each week) with or without food. Treatment usually starts with a low dose, which is gradually increased to help your body adjust and reduce the risk of side effects. Here's what you need to know:

Starting dose

Treatment typically begins with 2.5 mg once weekly. This is an initiation dose to help improve tolerability and is not intended for long-term blood sugar control or weight management.

Dose escalation

If needed, the dose may be increased gradually depending on the response and side effects (strictly under your doctor's supervision) until the most suitable maintenance dose is reached.

Individualised treatment

The dose that is right for you depends on your goals, response, and also on how well you are tolerating the medicine. Your doctor will decide if and when your dose should be adjusted.

What's the Difference Between Vial vs KwikPen?

Mounjaro is available in two formats, both containing the same active ingredient.

Single-dose vial: Contains one weekly dose that is drawn up using a syringe before injection. It may be preferred at lower doses or when cost is an important consideration.

Your doctor or pharmacist will show you how to use the device correctly and advise you on rotating injection sites (like the abdomen, thigh, or upper arm).

Side effects of Mounjaro

Like all medicines, Mounjaro may also cause side effects, although not everyone might experience them. The most common side effects affect the digestive system and are usually more noticeable when you first start treatment or after your dose is increased. They often improve as your body adjusts to the medicine.

Common side effects

These are usually mild to moderate and often improve over time:4

Nausea
Diarrhoea or constipation
Vomiting
Indigestion, burping, or bloating
Reduced appetite
Tiredness

Note: Eating smaller meals, eating slowly, and staying well hydrated may help reduce these side effects. You can also read our guide on managing GLP-1 side effects.

Serious side effects

Although less common, some side effects require prompt medical attention. Contact your doctor immediately if you experience:

Severe or persistent stomach pain, which may be a sign of pancreatitis
Gallbladder problems, including gallstones
Low blood sugar (hypoglycaemia), especially if Mounjaro is used with insulin or a sulfonylurea
Dehydration or kidney problems caused by prolonged vomiting or diarrhoea
Signs of a serious allergic reaction, such as swelling of the face, lips, tongue, or throat, difficulty breathing, or a severe rash
When to stop and call your doctor

Seek medical help right away if you have severe or persistent stomach pain (a sign of possible pancreatitis), signs of an allergic reaction, or symptoms of very low blood sugar (shakiness, sweating, confusion).

Suspected adverse reactions can also be reported through the Pharmacovigilance Programme of India (PvPI).

Who should not take Mounjaro

Mounjaro may not be suitable for everyone. So, before starting treatment, tell your doctor about your medical history and any medicines you are taking currently. It is generally not recommended if you:4

×Have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)
×Have had a serious allergic reaction to tirzepatide or any of the ingredients in Mounjaro
×Have a history of pancreatitis
×Have severe stomach or digestive disorders, such as gastroparesis

You should also tell your doctor if you are pregnant, planning to become pregnant, or breastfeeding.

Note: Inform them if you have kidney disease, gallbladder disease, diabetic eye disease, or if you take other diabetes medicines such as insulin or sulfonylureas, as your treatment may need to be adjusted.
