A quick info on ANA test

A quick info on ANA test

Overview

Overview

The antinuclear antibody (ANA-Elisa) test is a blood test that identifies antinuclear antibodies, markers that can appear in a range of autoimmune conditions. Normally, the immune system defends the body against infections and harmful substances while recognising and sparing healthy cells and tissues. In autoimmunity, however, the immune system mistakenly targets the body’s own cells as though they were foreign. Autoimmune conditions can involve multiple organs and present with a wide range of symptoms depending on which system is affected. Joint pain and stiffness are common in rheumatoid arthritis and lupus, while lupus may also cause skin rashes and heightened sun sensitivity. Persistent dryness of the eyes and mouth can point toward Sjögren syndrome, while thickened or tightened skin may indicate systemic sclerosis. Doctors often recommend this test when patients present with ongoing or unexplained symptoms, such as joint or muscle pain, fatigue, fever, skin changes, or swelling, that raise concern for an underlying autoimmune disorder, as well as when unexplained organ dysfunction is present. Results are typically evaluated alongside clinical history, and additional investigations, including complete blood count, urinalysis, kidney or liver function tests, and disease-specific antibody panels. On its own, a positive ANA result does not confirm any particular autoimmune diagnosis.

When and Who Should Get an ANA-Elisa Test Done?

When and Who Should Get an ANA-Elisa Test Done?

The ANA-Elisa test may be recommended when your symptoms do not have another clear explanation or when your medical history and examination suggest a possible autoimmune condition. Your doctor may recommend this test when there are signs or symptoms such as:

  • Persistent or unexplained fatigue
  • Joint pain, stiffness, or swelling
  • Muscle pain or weakness
  • Recurrent fever
  • Unexplained skin rashes, blisters, or changes in skin colour
  • Swelling of the salivary glands, with dry mouth or dry eyes
  • Symptoms affecting multiple parts of the body
  • Unexplained abdominal pain
  • Unexplained hair loss
  • Increased sensitivity to sunlight
  • Fingers or toes that turn unusually pale, blue, or red, especially in cold weather

Note: ANA testing is not usually recommended as a routine screening test for people without symptoms suggestive of an autoimmune disorder. This is because antinuclear antibodies can also be found in some healthy people, so a positive result does not necessarily mean that you have an autoimmune disease.

What Conditions Can an ANA-Elisa Test Help Detect?

What Conditions Can an ANA-Elisa Test Help Detect?

The ANA-Elisa test can help doctors evaluate certain autoimmune and connective tissue diseases. ANA is commonly associated with conditions such as:

  • Systemic lupus erythematosus (SLE): A long-term autoimmune disease that can affect several parts of the body, including the joints, skin, kidneys, heart, lungs, blood vessels, and nervous system.
  • Sjögren syndrome: An autoimmune condition that commonly affects the glands that produce tears and saliva, causing dry eyes and dry mouth.
  • Scleroderma (systemic sclerosis): An autoimmune condition that can cause changes in the skin and may affect blood vessels and internal organs.
  • Inflammatory muscle diseases (myositis): A group of conditions that can cause inflammation and weakness of the muscles.
  • Rheumatoid arthritis: An autoimmune condition that mainly affects the joints, causing pain, stiffness, and swelling.
  • Psoriatic arthritis: An inflammatory condition that can cause joint pain, stiffness, and swelling in people with psoriasis.
  • Autoimmune hepatitis: A condition in which the immune system attacks the liver, causing inflammation.
  • Autoimmune thyroid diseases: Conditions in which the immune system attacks the thyroid gland and may affect thyroid hormone production.

Note: ANA is not specific to any one disease and cannot diagnose these conditions on its own. A positive ANA result may also occur in some healthy people and in certain other conditions. Doctors interpret the result along with your symptoms, medical history, examination, and other test results.

ANA-Elisa Test Preparation

ANA-Elisa Test Preparation

What to Expect Before the ANA-Elisa Test?

  • No fasting or dietary restrictions are required for this test, so you can continue eating and drinking as usual before sample collection.
  • The test can be scheduled at any time of day.
  • However, inform your doctor about current medications, supplements, and recent or current illnesses, especially bacterial or viral illnesses, as they may sometimes influence ANA-Elisa results.

What to Expect During the Blood Collection?

  • A trained phlebotomist will arrive at your home at the scheduled appointment time for sample collection.
  • You will be asked to sit comfortably while the phlebotomist identifies a suitable vein, typically on the inner side of your arm near the elbow.
  • The selected area will be cleaned and disinfected.
  • A sterile needle will then be gently inserted into the vein to collect the blood sample. You may feel a brief pinching sensation.
  • A small amount of blood will be collected into a sample tube.
  • Once the required sample has been collected, the needle will be removed, and gentle pressure will be applied using cotton or gauze to help stop any bleeding.
  • A small bandage will be placed over the collection site after the procedure is completed.
  • The blood collection process is quick and generally takes only a few minutes.

What to Expect After the ANA-Elisa Test?

  • The sample will then be taken to the lab to be analysed. Your test results should be available within a day or two.

Note: If you notice anything unusual at the puncture site, such as persistent bleeding, a rash, or pain, consult your healthcare provider.

What Are the Parameters and their Ranges Included in the ANA-Elisa Test?

What Are the Parameters and their Ranges Included in the ANA-Elisa Test?

The ANA-Elisa test measures antinuclear antibodies (ANA) in your blood using the enzyme-linked immunosorbent assay (ELISA) method. The result is reported as an OD (optical density) ratio.

ANA result

OD ratio

Negative

< 0.90

Equivocal

0.90–1.10

Positive

> 1.11

The reference range and cut-off values may vary depending on the laboratory and the testing method used. Your ANA-Elisa report will include the applicable reference range for interpreting your result.

ANA-Elisa Test Interpretation

ANA-Elisa Test Interpretation

ANA Result

Interpretation

Negative

ANA is not detected at a significant level. However, a negative result does not completely rule out an autoimmune disease, particularly in the early stages or when other specific autoantibodies are involved.

Equivocal

The result is borderline. Your doctor may recommend repeat testing or additional tests for specific antibodies, depending on your symptoms and medical history.

Positive

ANA is detected above the laboratory’s cut-off. ANA positivity is commonly associated with conditions such as systemic lupus erythematosus (SLE), systemic sclerosis, Sjögren’s syndrome, and rheumatoid arthritis. Your doctor may recommend tests for specific autoantibodies, such as anti-Sm, anti-RNP, anti-Ro, or anti-La, to further evaluate a suspected autoimmune condition.

A positive ANA result is not specific to any one condition. ANA may also be found in some healthy individuals, particularly older adults, as well as in certain infections, malignancies and drug-induced autoimmune conditions. The final diagnosis is made by a doctor based on the overall clinical picture, and other relevant investigations.

ANA-Elisa Test Risks and Limitations

ANA-Elisa Test Risks and Limitations

Potential Risks and Complications

There is little risk involved with having your blood taken. Veins and arteries vary in size from one person to another and from one side of the body to the other. Taking blood from some people may be more difficult than from others.

Other risks associated with having blood drawn are slight, but may include:

  • Excessive bleeding.
  • Fainting or feeling light-headed.
  • Multiple punctures to locate veins.
  • Haematoma (blood accumulating under the skin).
  • Infection (a slight risk any time the skin is broken).

Understanding ANA-Elisa Test Limitations

  • Common in healthy people: ANA can be detected in as many as 30%-50% of certain healthy populations at low levels, so a positive result does not necessarily mean disease is present.
  • Low specificity: Despite high sensitivity for conditions such as lupus and systemic sclerosis, ANA testing has low specificity for actually confirming an autoimmune disease.
  • Titre-dependent accuracy: A weak positive result is more likely to be a false alarm, while a stronger positive is more likely to reflect an actual underlying condition.
  • Requires confirmation: A positive result cannot identify a specific disease and typically needs follow-up testing to clarify the diagnosis.
  • Does not rule out every autoimmune condition: A negative result does not fully exclude certain autoimmune conditions, which may need alternative testing methods.
