Alkaline Phosphatase (ALP) Test

The alkaline phosphatase (ALP) test measures the alkaline phosphatase level in your blood. The test is also known as the ALP test, ALP, ALKP, and Alk Phos.

Alkaline phosphatase is found in various tissues, with the highest amounts produced in the liver, bones, intestines, kidneys, and placenta (during pregnancy). It plays key roles in cellular processes, including cell growth, communication, and mineralisation of bones (depositing calcium and phosphorus to maintain bone strength). The alkaline phosphatase normal range varies with age, sex, blood type, and medication. Alkaline phosphatase high or alkaline phosphatase low levels may indicate underlying health conditions, mainly related to the liver, bones, and kidneys.

Doctors usually prescribe this test as part of a routine check-up or if you are at risk of developing liver or bone diseases. For an accurate diagnosis and to identify the source of ALP (since different organs have their own ALP), additional tests may be needed.

The alkaline phosphatase test is typically suggested if you have symptoms of liver or bone disorders, such as:

• Nausea
• Unexplained weakness and fatigue
• Vomiting
• Jaundice
• Abdominal pain
• Ankle or leg swelling
• Large or unusually shaped bones
• Bone pain
• Joint inflammation or arthritis

The ALP test may be recommended for individuals with:

• Suspected liver disease: Increased ALP may indicate liver damage, caused by conditions like a blocked bile duct (a duct that carries bile from the liver and gallbladder to the intestine).
• Wilson’s disease: In this condition, ALP levels may be reduced as a result of copper build-up in the liver.
• Bone diseases: Conditions such as Paget’s disease or calcium or vitamin D deficiency may lead to increased ALP levels in the blood.

An alkaline phosphatase test may be recommended as a regular check-up or if you are at risk of liver or bone conditions.

Changes in ALP levels may occur in several conditions. The ALP test may help support the diagnosis or monitoring of such conditions, which include:

• Cholestasis: Reduced or blocked bile flow through the bile ducts, leading to accumulation of ALP in the liver and liver damage.
• Cirrhosis: Impaired liver function due to scarring of liver tissue.
• Hepatitis: Liver inflammation due to viral infection, autoimmune conditions, alcohol use disorder, or medications.
• Coeliac disease: A condition in which gluten triggers an immune reaction that damages the lining of the small intestine.
• Paget’s disease of bone: A condition in which there is abnormal bone breakdown and rebuilding, leading to weak or misshapen bones.
• Anaemia: A reduced number of healthy haemoglobin-carrying red blood cells, which leads to low oxygen levels in the blood.
• Leukaemia: Certain cancers, such as leukaemia, may be associated with abnormal ALP levels.
• Wilson’s disease: A genetic condition in which copper builds up in organs like the liver and brain.
• Hypothyroidism: A reduced production of thyroid hormones by the thyroid gland.

What to Expect Before the Alkaline Phosphatase (ALP) Test

No fasting is required for a standalone Alkaline Phosphatase (ALP) test. You can eat and drink normally. However, if it is part of a broader package that includes fasting tests, your doctor may advise fasting for 8-12 hours. Avoid strenuous exercise for 24 hours before the test, as intense physical activity can temporarily raise Alkaline Phosphatase (ALP) levels and affect interpretation. Always inform your doctor about any ongoing medications.

What to Expect During the Blood Collection

A phlebotomist cleans the skin with an antiseptic, then places an elastic band around your upper arm to make the veins easier to access. A small needle is inserted into the vein; you may feel a brief pinch. Blood is collected into a labelled tube in under a minute.

What to Expect After the Alkaline Phosphatase (ALP) Test

A small cotton swab and bandage are placed over the site to stop any minor bleeding. You can return to normal activities immediately. Minor bruising is normal and disappears quickly. If dizzy, sit and rest for a few minutes.

Note:

• In growing children, ALP levels may be naturally higher due to increased activity of bone-forming cells called osteoblasts.
• Pregnant women may also have high ALP levels during the third trimester due to the production of ALP in the placenta, normal pregnancy-related changes, and foetal growth.
• Smoking, obesity, and a high-fat diet may also influence ALP levels in the blood.
• The test results should be interpreted along with a person’s symptoms and medical history.

Risks (Rare):

The ALP test is usually safe and unlikely to cause any severe complications.

• There may be some pain, minor bleeding, or swelling at the site of needle insertion; however, this usually resolves shortly.

Limitations

The alkaline phosphatase test has some limitations, like any other medical test.

• The alkaline phosphatase (ALP) test is not specific; it cannot identify the exact cause of abnormal ALP levels. Therefore, additional tests may be needed to determine the cause of abnormal levels.
• ALP levels may vary depending on factors such as diet, age, pregnancy, and certain medicines.
• ALP test results should be interpreted along with a person’s symptoms, medical history, other health conditions (especially liver and bone disorders), and additional test results.
