At the end of their lifespan, they are naturally broken down and recycled by the body through a process called\u00a0hemolysis<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33790808\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a><\/sup>.\u00a0<\/p>\n\n\n\n<p>However, if\u00a0RBCs\u00a0are destroyed much earlier than expected, or in larger numbers than usual, it can cause problems. This may happen because of inherited blood disorders or other medical conditions, and it can sometimes lead to anaemia or serious complications<sup><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6309361\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a><\/sup>.\u00a0Though\u00a0many complications can be prevented or managed well when the cause is found and treated early.\u00a0<\/p>\n\n\n\n<p>In this blog, we will explain\u00a0the\u00a0meaning of\u00a0hemolysis, its causes and types, common symptoms, how\u00a0doctors diagnose\u00a0it, and the treatment options\u00a0that may help.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"What_Is_Hemolysis\"><\/span><strong>What Is\u00a0Hemolysis?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Hemolysis\u00a0means the breakdown of\u00a0RBCs. This can be a normal part of how the body renews blood cells. RBCs\u00a0usually circulate for about 120 days before old or damaged cells are removed<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33790808\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a><\/sup>.\u00a0<\/p>\n\n\n\n<div class=\"wp-block-group fact-box\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<p>Did You Know?<\/p>\n\n\n\n<ul>\n<li>Around 5 million\u00a0old and damaged\u00a0RBCs\u00a0are cleared every second by macrophages\u00a0(immune cells).\u00a0These cells break down haemoglobin so its components can be recycled or removed from the body<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33790808\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li>The iron released from old\u00a0RBCs\u00a0is recycled and used by the bone marrow to make new\u00a0RBCs<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33790808\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li>Hemolysis\u00a0can occur inside blood vessels (intravascular\u00a0hemolysis) or in organs such as the spleen and liver (extravascular\u00a0hemolysis), depending on the underlying cause<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33790808\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a>,<a href=\"https:\/\/medlineplus.gov\/ency\/article\/002372.htm\" target=\"_blank\" rel=\"noreferrer noopener\">3<\/a><\/sup>.\u00a0<\/li>\n<\/ul>\n<\/div><\/div>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Causes_of_Hemolysis\"><\/span><strong>Causes of\u00a0Hemolysis<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Problems\u00a0can occur\u00a0when\u00a0RBCs\u00a0are destroyed faster than the bone marrow can replace them. This\u00a0usually\u00a0happens\u00a0for\u00a0two broad\u00a0reasons: intrinsic\u00a0causes, where the\u00a0issue is\u00a0within the\u00a0RBCs\u00a0themselves, and extrinsic\u00a0causes, where\u00a0something outside the\u00a0RBCs\u00a0damages them.\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Intrinsic\u00a0(intracorpuscular)\u00a0Causes<\/strong><\/h3>\n\n\n\n<p>The defect is within the RBC itself, such as an abnormality in the RBC membrane, enzymes, or haemoglobin. While most intrinsic causes are inherited (present from birth), some may be\u00a0acquired\u00a0later in life.\u00a0<\/p>\n\n\n\n<ul>\n<li><strong>G6PD deficiency:<\/strong>\u00a0A genetic enzyme deficiency that makes\u00a0RBCs\u00a0vulnerable to certain medicines, infections, or foods such as fava beans<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33790808\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK470315\/\" target=\"_blank\" rel=\"noreferrer noopener\">4<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Sickle cell disease:<\/strong>\u00a0An inherited disorder in which RBCs can become rigid and sickle-shaped under certain conditions, such as low oxygen levels, dehydration, or acidosis. These abnormal cells break down more easily than normal RBCs, leading to\u00a0hemolysis<sup><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6309361\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Thalassaemia:<\/strong>\u00a0A group of inherited blood disorders that reduce haemoglobin production, leading to fragile\u00a0RBCs<sup><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6309361\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a><\/sup>.<\/li>\n\n\n\n<li><strong>Hereditary spherocytosis:<\/strong>\u00a0A condition in which\u00a0RBCs\u00a0become sphere-shaped instead of their normal disc shape, making them more likely to be destroyed by the spleen<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/33790808\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Pyruvate kinase deficiency:<\/strong>\u00a0A rare inherited enzyme disorder that\u00a0causes\u00a0RBCs\u00a0to break down faster than usual<sup><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10430506\/\" target=\"_blank\" rel=\"noreferrer noopener\">5<\/a><\/sup>.\u00a0<\/li>\n<\/ul>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>Extrinsic\u00a0(extracorpuscular) Causes<\/strong><\/h3>\n\n\n\n<p>The RBCs are structurally normal, but they are destroyed by factors outside the RBC, such as immune reactions, infections, certain medications or toxins, mechanical injury, or an enlarged spleen. These causes usually develop later in life, although not all extrinsic causes are\u00a0acquired.\u00a0<\/p>\n\n\n\n<ul>\n<li><strong>Autoimmune haemolytic anaemia (AIHA):<\/strong>\u00a0The immune system mistakenly attacks and destroys healthy\u00a0RBCs<sup><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000571.htm\" target=\"_blank\" rel=\"noreferrer noopener\">6<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Infections:<\/strong>\u00a0Certain bacterial, viral, or parasitic infections, such as malaria, can trigger\u00a0hemolysis<sup><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000571.htm\" target=\"_blank\" rel=\"noreferrer noopener\">6<\/a>,<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6208064\/\" target=\"_blank\" rel=\"noreferrer noopener\">7<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Blood transfusion reactions:\u00a0<\/strong>Receiving incompatible blood can cause rapid destruction of transfused\u00a0RBCs<sup><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000571.htm\" target=\"_blank\" rel=\"noreferrer noopener\">6<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Mechanical damage:<\/strong>\u00a0Artificial heart valves, ventricular assist devices, or prolonged strenuous exercise (such as marathon running) can physically damage\u00a0RBCs<sup><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000571.htm\" target=\"_blank\" rel=\"noreferrer noopener\">6<\/a>,<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC6179786\/\" target=\"_blank\" rel=\"noreferrer noopener\">8<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Microangiopathic haemolytic anaemia (MAHA):<\/strong>\u00a0RBCs\u00a0are damaged as they pass through narrowed or injured small blood vessels, as seen in conditions such as thrombotic thrombocytopenic purpura and haemolytic uraemic syndrome<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/39835047\/\" target=\"_blank\" rel=\"noreferrer noopener\">9<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Certain cancers:\u00a0<\/strong>Blood cancers such as lymphoma or chronic lymphocytic leukaemia may increase the risk of\u00a0hemolysis<sup><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC8616265\/\" target=\"_blank\" rel=\"noreferrer noopener\">10<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Medications, toxins,\u00a0and chemicals:\u00a0<\/strong>Exposure to\u00a0certain medications, heavy metals, or certain industrial chemicals can damage\u00a0RBCs<sup><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000571.htm\" target=\"_blank\" rel=\"noreferrer noopener\">6<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li><strong>Hypersplenism (hyperactive\u00a0spleen):<\/strong>\u00a0An enlarged spleen may\u00a0become hyperactive and\u00a0trap and destroy\u00a0RBCs\u00a0more rapidly than normal<sup><a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/27703501\/\" target=\"_blank\" rel=\"noreferrer noopener\">11<\/a><\/sup>.\u00a0<\/li>\n<\/ul>\n\n\n\n<p><strong>Important:<\/strong>\u00a0Although intrinsic haemolytic anaemias are usually inherited and extrinsic haemolytic anaemias are often\u00a0acquired, this is not always the case. Some intrinsic causes may be\u00a0acquired, so the terms intrinsic and extrinsic do not strictly mean inherited and\u00a0acquired, respectively.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Symptoms_of_Hemolysis\"><\/span><strong>Symptoms of\u00a0Hemolysis<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Symptoms can vary from person to person. They may depend on how quickly\u00a0RBCs\u00a0are being destroyed, how severe the condition is, and what is causing it. Common\u00a0hemolysis\u00a0symptoms\u00a0and signs\u00a0include<sup><a href=\"https:\/\/medlineplus.gov\/ency\/article\/000571.htm\" target=\"_blank\" rel=\"noreferrer noopener\">6<\/a>,<a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/35584830\/\" target=\"_blank\" rel=\"noreferrer noopener\">12<\/a><\/sup>:\u00a0<\/p>\n\n\n\n<ul>\n<li>Fatigue or weakness\u00a0<\/li>\n\n\n\n<li>Pale skin (pallor)\u00a0<\/li>\n\n\n\n<li>Jaundice\u00a0(which causes yellowing of the skin\u00a0and\u00a0eyes)\u00a0<\/li>\n\n\n\n<li>Dark or reddish-brown urine\u00a0<\/li>\n\n\n\n<li>Shortness of breath\u00a0<\/li>\n\n\n\n<li>Rapid heartbeat (tachycardia)\u00a0<\/li>\n\n\n\n<li>Dizziness or light-headedness\u00a0<\/li>\n<\/ul>\n\n\n\n<p><em><strong>Note:<\/strong>\u00a0Dark or reddish-brown urine is more commonly seen in intravascular\u00a0hemolysis\u00a0due to the presence of haemoglobin in the urine (haemoglobinuria). In extravascular\u00a0hemolysis, urine usually does not become dark because\u00a0RBCs\u00a0are broken down\u00a0mainly in\u00a0the spleen and liver.
