The infection is caused by the Salmonella typhi bacteria that enters and spreads throughout your body when you consume contaminated food or water<sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC10867790\/\" target=\"_blank\" rel=\"noreferrer noopener\">1<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a><\/sup>. Some symptoms of typhoid, like fever, tiredness, and stomach pain, can resemble other diseases like malaria. After the bacteria pass through the intestinal walls, they reach the blood within 24 hours. The infection then spreads to other organs like the liver, spleen, bone marrow, gall bladder, and, sometimes, into the brain. Within 7 to 14 days of exposure, symptoms like high fever, body pain, abdominal pain, and weakness start to show. If not tested and treated early, typhoid can become more severe with complications like bleeding in the stomach, perforation of the intestines, and brain-related issues like delirium, confusion, and memory loss<sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a>,<a href=\"https:\/\/immunisationhandbook.health.gov.au\/contents\/vaccine-preventable-diseases\/typhoid-fever\" target=\"_blank\" rel=\"noreferrer noopener\">5<\/a><\/sup>.\u00a0<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Why_Typhoid_Testing_is_Important\"><\/span><strong>Why Typhoid Testing is Important<\/strong>?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>The typhoid test is important to confirm that you have a typhoid infection. In severe cases, confusion or drowsiness may be seen<sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a>,<a href=\"https:\/\/www.mdpi.com\/2036-7449\/15\/4\/38\" target=\"_blank\" rel=\"noreferrer noopener\">11<\/a>,<a href=\"https:\/\/www.merckmanuals.com\/professional\/infectious-diseases\/gram-negative-bacilli\/typhoid-fever\" target=\"_blank\" rel=\"noreferrer noopener\">12<\/a><\/sup>.\u00a0<\/li>\n\n\n\n<li>Recent travel to typhoid-affected area or close contact with a typhoid-affected individual in the last 28 days<sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a>,<a href=\"https:\/\/www.psmid.org\/diagnosis-treatment-and-prevention-of-typhoid-fever-in-adults-2017\/\" target=\"_blank\" rel=\"noreferrer noopener\">10<\/a><\/sup>\u00a0<\/li>\n<\/ul>\n\n\n\n<p>In such cases, a doctor may look into your symptoms, medical history and accordingly recommend an appropriate blood test for typhoid.\u00a0\u00a0<\/p>\n\n\n\n<blockquote class=\"wp-block-quote doc-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>In my experience, I have noticed that unlike other fevers where pulse usually rises by 10 with every 1 degree increase in temperature, in typhoid fever, the heart beat may remain normal or slow down to less than 60 beats per minute. This relative bradycardia is classical sign seen in typhoid.<\/p>\n<cite><a href=\"https:\/\/pharmeasy.in\/legal\/editorial-policy\/dr-kathak-shah-96\" target=\"_blank\" rel=\"noreferrer noopener\"><strong><em>Dr. Kathak Shah, MBBS, DDV, Fellowship in Lasers &amp; Aesthetics, Fellowship in Dermatosurgery<\/em><\/strong><\/a><\/cite><\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\"><span class=\"ez-toc-section\" id=\"Who_is_at_Higher_Risk\"><\/span><strong>Who is at Higher Risk?<\/strong><span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>The risks of typhoid depend on an individual\u2019s age, environment, and location and is higher in the following individuals<sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a>,<a href=\"https:\/\/www.who.int\/news-room\/fact-sheets\/detail\/typhoid\" target=\"_blank\" rel=\"noreferrer noopener\">3<\/a>,<a href=\"http:\/\/cdc.gov\/typhoid-fever\/about\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\">13<\/a><\/sup>: \u00a0<\/p>\n\n\n\n<ul>\n<li>People living in areas with unclean water and poor sanitation\u00a0<\/li>\n\n\n\n<li>Travellers to places where typhoid is common (areas of Africa, Asia, or Latin America)\u00a0<\/li>\n\n\n\n<li>Those with a weak immunity, including those with HIV\/AIDS, cancer, and those taking medicines that lower their immunity (steroids or cancer medicines) 2\u00a0<\/li>\n\n\n\n<li>Children under 15 years of age, especially between 5 and 15 years, and those in areas where typhoid is common <sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a>,<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC2647431\/\" target=\"_blank\" rel=\"noreferrer noopener\">14<\/a><\/sup>\u00a0<\/li>\n<\/ul>\n\n\n\n<p>If these individuals experience symptoms of typhoid fever, it is best to <strong><a href=\"https:\/\/pharmeasy.in\/online-doctor-consultation\/\" target=\"_blank\" rel=\"noreferrer noopener\">consult a doctor<\/a><\/strong> who may advise a blood test for typhoid.\u00a0<\/p>\n\n\n\n<blockquote class=\"wp-block-quote doc-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>A with a fever lasting more than two weeks with decrease in white blood cell counts should raise the suspicion of typhoid fever. A blood culture test is considered the gold standard test for typhoid; however, different typhoid tests may help identify typhoid infection and provide more accurate results when done early. Let us see how to test for typhoid using different tests:\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>1. Culture Tests<\/strong><\/h3>\n\n\n\n<p>This is considered the standard test for typhoid. In this test, a sample of your blood or bone marrow is checked to find the presence of bacteria causing typhoid. It may give more reliable results compared to other tests when done in the first week of illness. It has a sensitivity of 50-80%, which means it can correctly detect the infection 5 to 8 times out of 10. Stool and urine samples can also be helpful, but they may be less reliable early on.\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>2. Antibody Tests<\/strong><\/h3>\n\n\n\n<p>The <a href=\"https:\/\/pharmeasy.in\/diagnostics\/tests\/widal-test-slide-method-156\" target=\"_blank\" rel=\"noreferrer noopener\"><strong>widal blood test<\/strong><\/a> for typhoid checks for antibodies which your body produces against the bacteria. It may be used commonly in areas where the disease is spreading fast. However, the results may be affected if there was any past infection or other Salmonella diseases<sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a>,<a href=\"https:\/\/www.ncbi.nlm.nih.gov\/pmc\/articles\/PMC8065732\/\" target=\"_blank\" rel=\"noreferrer noopener\">6<\/a><\/sup>. It usually shows positive results only after 7 to 10 days of infection, and also may miss real cases or show false positives due to low sensitivity and low specificity<sup><a href=\"https:\/\/www.ncbi.nlm.nih.gov\/books\/NBK557513\/\" target=\"_blank\" rel=\"noreferrer noopener\">2<\/a>,<a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC3050682\" target=\"_blank\" rel=\"noreferrer noopener\">15<\/a><\/sup>.\u00a0<\/p>\n\n\n\n<h3 class=\"wp-block-heading\"><strong>3. Molecular Tests<\/strong><\/h3>\n\n\n\n<p>The Polymerase Chain Reaction (PCR) test may help identify the genetic material (DNA) of the typhoid bacteria and show which antibiotic may work and which may not (including fluoroquinolone antibiotics). It is given as an injection taken as per the doctor\u2019s advice at least 2 weeks before travel (as protection usually starts within 7\u201314 days), and repeat doses every 2 years<sup><a href=\"https:\/\/immunisationhandbook.health.gov.au\/contents\/vaccine-preventable-diseases\/typhoid-fever\" target=\"_blank\" rel=\"noreferrer noopener\">5<\/a><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC10425215\/\" target=\"_blank\" rel=\"noreferrer noopener\">,26<\/a><\/sup>.\u00a0<\/p>\n\n\n\n<p>Immunity is usually achieved within 7\u201314 days with the<strong> <a href=\"https:\/\/pharmeasy.in\/molecules\/purified-vi-capsular-polysaccharide-salmonella-typhi-conjugated-tetanus-toxid-8982\" target=\"_blank\" rel=\"noreferrer noopener\">Vi polysaccharide vaccine<\/a><\/strong>, but it\u2019s ideal to take it at least 2 weeks before exposure<strong>.<\/strong>\u00a0<\/p>\n\n\n\n<p>Vi conjugate vaccine (TCV): This is a vaccine with Vi polysaccharide but joined to a special protein, which is why it is called a conjugate vaccine. It can be given to people aged 6 years and older. One capsule is taken every other day (on day 1, 3, 5, and 7), for a total of 4 capsules. It should be taken at least 1 week before travel, and a booster dose may be given once in 5 years.
